AICA syndrome with facial palsy following vertigo and acute sensorineural hearing loss.

We report a case of infarction of the anterior inferior cerebellar artery (AICA) with peripheral facial palsy following vertigo and acute sensorineural hearing loss. A 39-year-old female presented with vertigo and sudden hearing loss, tinnitus, and aural fullness of the right ear. An audiogram revealed a severe hearing loss at all tested frequencies in the right ear. Spontaneous nystagmus toward the left side was also observed. Otoneurological examinations showed sensorineural hearing loss of the right ear and horizontal and rotatory gaze nystagmus toward the left side, and a caloric reflex te…

[Association between endolymphatic hydrops and benign paroxysmal positional vertigo: coincidence or causality?].

INTRODUCTION: The aim of this study was to determine the prevalence of benign paroxysmal positional vertigo (BPPV) in patients diagnosed with endolymphatic hydrops (EH) and to analyze whether this association is a factor for poor prognosis in these patients. MATERIAL AND METHODS: A retrospective study was carried out in our department among the patients definitively diagnosed as having Ménière's disease according to the criteria of the American Academy of Otolaryngology and BPPV by the usual triggering manoeuvres. RESULTS: The prevalence of BPPV in our patients with EH was 5.1%. The rate of …

One-year follow-up of cognitive behavioral therapy for phobic postural vertigo.

BACKGROUND: Phobic postural vertigo is characterized by dizziness in standing and walking despite normal clinical balance tests. Patients sometimes exhibit anxiety reactions and avoidance behavior to specific stimuli. Different treatments are possible for PPV, including vestibular rehabilitation exercises, pharmacological treatment, and cognitive behavioral therapy. We recently reported significant benefits of cognitive behavioural therapy for patients with phobic postural vertigo. This study presents the results of a one-year follow-up of these patients. METHODS: Swedish translations of the …

Enlarged vestibular aqueduct (EVA) related with recurrent benign paroxysmal positional vertigo (BPPV).

The vestibular aqueduct is a bony canal related to the bony labyrinth of the inner ear and represents the non-sensory components of the endo … to stepwise progressive or sudden. Vestibular disturbances, ranging from mild imbalance to episodic vertigo, are rarely described in the literature. Benign paroxysmal positional vertigo (BPPV) is a labyrinthine disorder with a typical behavior: intense crises of rotational vertigo induced by postural changes of the head, with short duration and usually go …

Subjective visual horizontal in patients with posterior canal benign paroxysmal positional vertigo.

CONCLUSION: Patients with posterior canal benign paroxysmal positional vertigo (p-BPPV) could have mild excitation in the vestibular system on the affected side. OBJECTIVE: p-BPPV is considered to be caused by small particles dislodged from the utricular macula. While subjective visual horizontal (SVH) mainly reflects utricular function, there is less documentation on SVH in patients with p-BPPV. The purpose of this study was to clarify whether p-BPPV could affect SVH. PATIENTS AND METHODS: Twenty-eight patients with unilateral idiopathic p-BPPV were enrolled. For comparison, 17 patients wit…

Inner ear and facial nerve complications of acute otitis media, including vertigo.

Acute otitis media is one of the most common diagnoses made in children in the United States. Intracranial and extracranial (intratemporal) complications have greatly decreased in the antibiotic era, but still remain a challenge when they arise. This article addresses two intratemporal complications with significant associated morbidity: facial nerve paralysis/paresis, and labyrinthitis. Epidemiology, pathology, clinical diagnosis, and treatment options are discussed, focusing on an evidence-based approach to diagnosis and management. In addition, the future of treatment and current questions

Vestibular evoked myogenic potentials in benign paroxysmal positional vertigo and Meniere's disease.

The objective was to investigate vestibular evoked myogenic potentials (VEMP) in benign paroxysmal positional vertigo (BPPV) and Meniere's disease, and to determine if this type of testing is valuable for assessing the vestibular system. A prospective controlled clinical study was designed in a tertiary referral center setting. The 62 participants included 17 healthy controls and 45 other subjects selected from patients who presented with the complaint of vertigo (25 diagnosed with BPPV and 20 diagnosed with Meniere's disease). The main outcome measures of VEMP were recorded in all subjects an…

Experimental study of speed-dependent positional nystagmus in benign paroxysmal positional vertigo.

OBJECTIVE: One of the clinical characteristics of benign paroxysmal positional vertigo (BPPV) is that the more quickly the head position changes, the more severe the vertigo. This suggests that the velocity of the head change is critical in determining the occurrence and severity of vertigo. The aim of this study was to examine factors determining the symptoms of BPPV using models of canalolithiasis and cupulolithiasis. MATERIAL AND METHODS: Canalolithiasis and cupulolithiasis models were prepared using the bullfrog posterior semicircular canal (PSC). The ampullary nerve discharges were compa…

Unexpected complication of posterior canal occlusion surgery for benign paroxysmal positional vertigo.

OBJECTIVE: The purpose of this report was to illustrate how an unusual complication of posterior canal occlusion surgery for benign paroxysmal positional vertigo (BPPV) may be recognized and prevented. Recurrence of BPPV after occlusion surgery of the posterior semicircular canal has not previously been reported in the literature, to the authors' knowledge. Failure of occlusion not only permits the continued symptoms of BPPV but also burdens the patient with the additional morbidity of a fistula of the PSCC. The authors describe the successful treatment of a patient with recurrent and incapaci…

Unpredictable hearing loss after intratympanic gentamicin treatment for vertigo. A new theory.

A new hypothesis is advanced suggesting that unpredictable cases of profound hearing loss after intratympanic gentamicin treatment (IGT) may be caused by decreased patency of the communication routes between the inner ear and the cerebrospinal fluid, primarily of the cochlear aqueduct. A tympanic displacement analyzer, which can indirectly analyze inner ear and intracranial pressure changes and can also evaluate the efficiency of communication between these two compartments, was used. Two cases are presented: in the first, a patient who became deaf after IGT showed signs of decreased patency o

Canalith repositioning procedure for relief of post-stapedectomy benign paroxysmal positional vertigo.

A 41-year-old woman underwent a successful stapedectomy for relief of conductive hearing loss due to otosclerosis. Post-operatively, she developed persistent symptoms of benign paroxysmal positional vertigo (BPPV). Eight weeks later, a canalith repositioning procedure was performed in the office, with immediate complete resolution of her vertigo. Anatomic and pathophysiologic implications of these observations will be discussed.

Spinocerebellar ataxia type 6 with positional vertigo and acetazolamide responsive episodic ataxia.

The SCA6 mutation, a small expansion of a CAG repeat in a calcium channel gene CACNA1A, was identified in three pedigrees. Point mutations in other parts of the gene CACNA1A were excluded and new clinical features of SCA6 reported--namely, central positional nystagmus and episodic ataxia responsive to acetazolamide. The three allelic disorders, episodic ataxia type 2, familial hemiplegic migraine, and SCA6, have overlapping clinical features.

Cervical vertigo. Diagnostic and semiological problem with special emphasis upon "cervical nystagmus".

There is a strong evidence that vertigo can be produced by cervical disturbance, either via the neck-proprioceptor, or via the vascular supply by the vertebral artery. However, it is very difficult to confirm the cervical origin of the vertigo in a particular patient. "Cervical nystagmus" has been proposed as a diagnostic clue. But critical analysis, discussed in this paper, has raised doubt about its reliability. The most know type, the neck torsion nystagmus, appears to be in fact a "normal" cervico-ocular reflex. Its inconstancy and low gain makes that it is only present in 50% of the subje…

A study to determine whether a maneuver to restore positional nystagmus from benign paroxysmal positional vertigo fatigue in posterior-canal-type benign paroxysmal positional vertigo is sufficient for diagnosis.

BACKGROUND: Benign paroxysmal positional vertigo (BPPV) has a characteristic, called BPPV fatigue, in which positional nystagmus (PN) and dizziness symptoms disappear when the patient repeatedly assumes a head position that causes dizziness. AIMS/OBJECTIVES: We previously developed a new maneuver to restore PN that had disappeared because of BPPV fatigue in posterior-canal-type BPPV (pc-BPPV). This study aimed to demonstrate that the maneuver restores PN from BPPV fatigue to a level sufficient for diagnosing BPPV by macroscopic observation of PN. MATERIAL AND METHODS: Thirty patients with pc…

Diagnostic challenges in posterior circulation ischemic stroke presenting with acute dizziness and vertigo.

Posterior circulation stroke (PCS) is a common and clinically important cause of acute dizziness and vertigo, accounting for approximately 20-25% of all acute ischemic strokes. Because PCS frequently presents with non-specific, fluctuating, or isolated vestibular symptoms, it is about three times more likely to be initially missed in emergency care than anterior circulation stroke (ACS), with important consequences for acute treatment, secondary prevention, and patient outcomes. Diagnostic evaluation is further complicated by the low sensitivity of prehospital stroke scales for posterior fossa…

Vertigo-unsolved Mystery (Meniere's Disease) Care Redefined: Insights from a Tertiary Care Center.

BACKGROUND: Meniere's disease remains a poorly understood inner ear disorder since its first description by Prosper Meniere in the early 1800s. Characterized by episodic vertigo, fluctuating hearing loss, and tinnitus, it continues to challenge clinicians due to its unpredictable course and variable treatment response. Lifestyle modification alone often fails to adequately control vertigo episodes. AIM AND OBJECTIVES: To assess the effectiveness of oral pharmacotherapy in controlling vertigo and stabilizing hearing in patients with Meniere's disease, and to develop a practical, non-invasive t…
