Various causes and clinical characteristics in vertigo in children with normal eardrums.

OBJECTIVE: The differential diagnosis of vertigo in children is extensive. Otitis media and middle ear effusion could be the most common cau … tigo in children requires a questionnaire for extensive and complete history taking, audiograms and vestibular function tests. And in selected cases, electroencephalography, hematological evaluation, imaging of the brain or temporal bone should be performed.

Numerical simulations to determine the stimulation of the crista ampullaris during the Head Impulse Test.

The Head Impulse Test, the most widely accept test to assess the vestibular function, comprises rotations of the head based on idealized orientations of the semicircular canals, instead of their individual arrangement specific for each patient. In this study, we show how computational modelling can help personalize the diagnosis of vestibular diseases. Based on a micro-computed tomography reconstruction of the human membranous labyrinth and their simulation using Computational Fluid Dynamics and Fluid-Solid Interaction techniques, we evaluated the stimulus experienced by the six cristae ampull

Inner Ear Schwannoma Treated With Intratympanic Gentamicin for Recurrent Ménière's Disease-Like Vertigo: A Case Report.

Inner ear schwannoma (IES), also known as intralabyrinthine schwannoma, is a rare benign tumor typically presenting with hearing loss, tinnitus, and vestibular symptoms. In some cases, vestibular manifestations mimic Ménière's disease, characterized by recurrent vertigo attacks in which endolymphatic hydrops has been implicated in the underlying pathophysiology. Herein, we report the case of a 44-year-old woman with left-sided IES who developed intractable recurrent vertigo. Despite profound hearing loss in the affected ear, vestibular function testing revealed residual activity in all three s

Linear path integration deficits in patients with abnormal vestibular afference.

Effective navigation requires the ability to keep track of one's location and maintain orientation during linear and angular displacements. Path integration is the process of updating the representation of body position by integrating internally-generated self-motion signals over time (e.g., walking in the dark). One major source of input to path integration is vestibular afference. We tested patients with reduced vestibular function (unilateral vestibular hypofunction, UVH), patients with aberrant vestibular function (benign paroxysmal positional vertigo, BPPV), and healthy participants (cont

[Amplitude summation and the pendulum test as elements of the otoneurologic examination].

Vestibular tests were performed in more than 600 otoneuological patients. The data analysed included the otoneurological history, audiograms, observation of spontaneous nystagmus, caloric and swing tests, and recording by electronystagmography. The tracings were subjected to summation of the amplitudes of the slow-phase nystagmus and classification of the recording as peripheral (small amplitude, high frequency) or central (dysrhythmia with bouts of high-frequency nystagmus). The correlations between spontaneous nystagmus and directional preponderance in caloric and rotation tests are complex,

Proof of Concept for an "eyePhone" App to Measure Video Head Impulses.

OBJECTIVE: Differentiating benign from dangerous causes of dizziness or vertigo presents a major diagnostic challenge for many clinicians. Bedside presentations of peripheral vestibular disorders and posterior fossa strokes are often indistinguishable other than by a few subtle vestibular eye movements. The most challenging of these to interpret is the head impulse test (HIT) of vestibulo-ocular reflex (VOR) function. There have been major advances in portable video-oculography (VOG) quantification of the video HIT (vHIT), but these specialized devices are not routinely available in most clini

Vestibular evoked myogenic potentials of haemodialysed patients with end stage renal disease.

End stage renal disease (ESRD) can cause malfunction of multiple organs, including auditory and vestibular systems. During recent years, a significant amount of research has demonstrated the direct involvement of the otolith organs in stabilizing body and gaze which led to the development of specific functional tests. Stable gaze and body are more important in patients with ESRD, as they have an increased risk of bone fracture. The aim of this study was to investigate saccule and related neural pathways in haemodialysed patients with chronic renal failure. Twenty patients (40 ears) with ESRD w

Association of audiovestibular findings with fluid-attenuated inversion recovery magnetic resonance imaging in patients with vestibular schwannoma.

BACKGROUND: Vestibular schwannoma (VS) is a benign tumor of the vestibular nerve. Flair-attenuated inversion recovery (FLAIR) of magnetic resonance imaging (MRI) images are sensitive in detecting high protein contents of fluids. OBJECTIVES: To investigate the association between signal intensity (SI) on FLAIR images and audiovestibular findings in patients with VS. METHODS: Medical records of twenty-five patients with VS were retrospectively analyzed. RESULTS: Larger tumors were associated with increased FLAIR SI of the cochlea, vestibule, and semicircular canal (SCC) on the affected side c

Endolymphatic hydrops imaging and correlation with clinical characteristics, audiovestibular function and mental impairment in patients with Meniere's disease.

PURPOSE: MR imaging was used to visualize the vestibular and cochlear endolymphatic hydrops in patients with Meniere's disease (MD). The relationship between the degree of hydrops and clinical characteristics, audiovestibular function, anxiety and depression state in MD patients. METHODS: 70 patients with definitely or probably unilateral Meniere's disease received bilateral intratympanic gadolinium administration and MR scanning. The degree of bilateral vestibular and cochlea hydrops were analyzed and evaluated by three-dimensional real inversion recovery (3D-real IR) sequence, and the corre

Comparison of the Motor Performance and Vestibular Function in Infants with a Congenital Cytomegalovirus Infection or a Connexin 26 Mutation: A Preliminary Study.

OBJECTIVES: Hearing-impaired children are at risk for vestibular damage and delayed motor development. Two major causes of congenital hearing loss are cytomegalovirus (CMV) infection and connexin (Cx) 26 mutations. Comparison of the motor performance and vestibular function between these specific groups is still underexplored. The objective of this study was to investigate the impact of congenital (c)CMV and Cx26 on the motor performance and vestibular function in 6 months old infants. DESIGN: Forty children (mean age 6.7 months; range 4.8 to 8.9 months) participated in this cross-sectional d

Prolonged bone-conducted vibration in superior semicircular canal dehiscence and in otosclerosis: comparison of the 3D eye movement evaluation.

Vibration-induced nystagmus, as clinical sign, was recently introduced in outpatient clinical practice for the study and evaluation of otoneurological patients. This response, which can only be evoked by bone conducted vibratory stimulation in the mastoid region or at the location on the forehead in the midline at the hairline, was essentially designed for patients with persistent unilateral vestibular deficit and was interpreted as the result of excitatory functional activity of the vestibular system on the non-affected side. Vibratory stimulation is, in fact, considered to reach both systems

Comparison of VEMPs, VHIT and caloric test outcomes after vestibular neurectomy in Menière's disease.

OBJECTIVE: Selective unilateral vestibular neurectomy (VN) is considered a reliable surgical treatment in case of recurrent vertigo in Menière's disease (MD) because of hearing preservation and a minimally invasive posterior fossa retrosigmoid approach. The present study aimed to assess the quality of life and the long-term vestibular function in patients submitted to yearly follow-up after VN because of intractable MD. METHODS: Retrospective series of 15 MD patients undergoing retrosigmoid VN for recurrent vertigo. Outcome measures included cVEMPs and oVEMPs (cervical and ocular vestibular e

Predictors of vertigo in patients with untreated vestibular schwannoma.

OBJECTIVES: Previous studies have shown that vertigo is the most powerful negative predictor of quality of life in patients with vestibular schwannomas, but the variability in vertigo symptom severity is still poorly understood. We wanted to find out whether vertigo could be related to objective parameters such as tumor size, location, vestibular nerve function, hearing, and postural stability in patients with untreated vestibular schwannomas. STUDY DESIGN: Baseline data from prospective cohort study. SETTING: Tertiary referral center. PATIENTS: Four hundred thirty-four consecutive patients

Advances in the diagnosis and management of acute vertigo.

BACKGROUND: Patients presenting to the emergency department with acute vertigo pose a diagnostic challenge. While 'benign' peripheral vestibulopathy is the most common cause, the possibility of a posterior circulation stroke is paradoxically the most feared and missed diagnosis in the emergency department. OBJECTIVES: This review will attempt to cover the significant advances in the ability to diagnose acute vertigo that have occurred in the last two decades. The review discusses the role of neurological examinations, imaging and specific oculomotor examinations. The review then discusses the

[Vestibular testing in Ménière's syndrome].

Extensive vestibular examination was done in fifty patients with unilateral Menière's disease in the interval between the attacks of vertigo. Horizontal and vertical eye-movements were recorded by electro-oculography. In 80% of the significant difference was found in the slow phase velocity of the nystagmus due to caloric stimulation between the normal and pathological ear (mean difference 41% +/- 24,2). A spontaneous nystagmus was found with closed eyes in 58% of the patients with a mean value of 2,5 degrees/sec +/- 1,6 and a positional nystagmus in 76% with a mean value of 2,3 degrees/sec +/
