The sociomateriality of prescribing feedback interventions: A scoping review.

OBJECTIVES: Prescribing errors frequently threaten patient safety, with early career doctors writing most prescriptions. Prescribing feedback interventions can improve the prescribing capabilities of early career doctors. These typically involve interactions between people and materials; however, these relations tend to be under-researched due to a general focus on human behaviours. This study aimed to trace the sociomaterial relations that produce and sustain prescribing feedback interventions for early career doctors in hospital settings. METHODS: We conducted a scoping review of eight data

Clinician Fidelity with Hypertension Quality Improvement Program is Associated with Blood Pressure Control within a Clinician-Patient Panel.

BACKGROUND: Differences in clinician fidelity with hypertension quality improvement (QI) interventions may influence blood pressure (BP) control. METHODS: This analysis included data from 35,115 patients with 89,230 clinical encounters during a calendar year conducted by 87 physicians, 7 advanced practice nurses, and 173 residents to examine the association of fidelity measures with QI interventions and BP control for a clinician-patient panel. BP control (BP < 140/90 mm Hg) was based on vital signs at last clinic visit. The QI program fidelity measures included (1) documented automated offic

Education and regular Feedback for Antimicrobial Stewardship in nursing homes: EduFAST study protocol.

BACKGROUND: The aging population has led to an increased number of people in nursing homes (NHs). The use of antimicrobials in this setting carries an increased risk of adverse events and interactions, particularly in individuals with advanced age, comorbidities, and polypharmacy. Antimicrobial Stewardship Programs (ASPs) have been shown to reduce the consumption of these drugs, improve prescribing practices, and have a positive ecological impact. However, the implementation of ASPs in NHs remains limited. OBJECTIVE: To develop and evaluate the impact of EduFAST web platform, which is designe

A pilot point-of-care kidney disease clinic in primary care to pharmacologically optimise people with chronic kidney disease (PROTECT KIDNEY).

BACKGROUND: Chronic kidney disease (CKD) is increasing in prevalence and is associated with substantial morbidity and mortality. Early initiation of cardiorenal protective medications is recommended to improve outcomes. Barriers to implementation include renal function monitoring and resources to initiate and titrate treatment. We aimed to evaluate the feasibility and acceptability of a protocolled point-of-care testing (POCT) pathway to optimise people living with proteinuric CKD in primary care. METHOD: A pilot quality improvement study conducted across three general practices in Greater Lo

Mixed pain: clinical practice recommendations.

Mixed pain, defined by the concurrent involvement of nociceptive, neuropathic, and sometimes nociplastic mechanisms, poses a significant diagnostic and therapeutic challenge within modern pain medicine. This complex pain phenotype is increasingly recognized as a prevalent and burdensome clinical entity, yet it remains substantially underdiagnosed and sub-optimally managed across diverse healthcare settings. Epidemiological data indicate that mixed pain affects a substantial proportion of patients with chronic pain syndromes and is consistently associated with more severe symptomatology, prolon

Predicting postoperative delirium assessed by the Nursing Screening Delirium Scale in the recovery room for non-cardiac surgeries without craniotomy: A retrospective study using a machine learning approach.

Postoperative delirium (POD) contributes to severe outcomes such as death or development of dementia. Thus, it is desirable to identify vulnerable patients in advance during the perioperative phase. Previous studies mainly investigated risk factors for delirium during hospitalization and further used a linear logistic regression (LR) approach with time-invariant data. Studies have not investigated patients' fluctuating conditions to support POD precautions. In this single-center study, we aimed to predict POD in a recovery room setting with a non-linear machine learning (ML) technique using pr

Appropriateness of small molecule agents for patients with IBD of childbearing age - a RAND/UCLA appropriateness panel.

BACKGROUND: Many women of childbearing age with inflammatory bowel disease (IBD) require advanced therapies. While biologics are largely low risk during pregnancy, the novel small molecules tofacitinib, filgotinib, upadacitinib and ozanimod (TFUO) have shown concerning teratogenic effects, and decreased fertility in animal studies. Therefore, their use in women of childbearing age needs careful consideration. DESIGN: RAND/University of California Los Angeles (UCLA) Appropriateness Method (RAM). OBJECTIVE: To evaluate the appropriateness of TFUO in women of childbearing age. METHODS: We conv

The Relationship Between Rejection of Care Behaviors and Pain and Delirium Severity in Hospital Dementia Care.

BACKGROUND AND OBJECTIVES: Rejection of care is common in hospitalized persons living with dementia. However, distinguishing between rejection of care behaviors related to care practices or other causes, such as pain or delirium, is challenging. The purpose of this study is to further understand the relationship between rejection of care and pain and delirium in hospitalized patients with dementia by identifying which rejection of care behaviors are associated with pain and delirium. RESEARCH DESIGN AND METHODS: Care encounters between hospitalized patients with dementia (n = 16) and nursing

Increasing Provider Self-Efficacy to Manage Chronic Cancer Pain.

BACKGROUND: Aims: Pain is a significant problem for those with a cancer diagnosis. Oncology providers often use opioid therapy to manage cancer-related pain. Accessing opioid therapy has grown increasingly difficult because of the opioid epidemic. The American Society of Clinical Oncology (ASCO) released guidelines to optimize pain management for patients with oncologic diagnoses. The goal of this quality improvement project was to create an educational session and evaluate self-efficacy in providers who manage chronic cancer pain. DESIGN: The Plan-Do-Study-Act was used as the framework for t

Neonatal Retinoblastoma.

From 7% to 10% of all retinoblastomas and from 44% to 71% of familial retinoblastomas in developed countries are diagnosed in the neonatal period, usually through pre- or post-natal screening prompted by a positive family history and sometimes serendipitously during screening for retinopathy of prematurity or other reasons. In developing countries, neonatal diagnosis of retinoblastoma has been less common. Neonatal retinoblastoma generally develops from a germline mutation of RB1, the retinoblastoma gene, even when the family history is negative and is thus usually hereditary. At least one-hal
