We present a case of a traumatic skull base internal carotid artery (ICA) pseudoaneurysm treated with endovascular flow diversion stenting. The patient was a 27-year-old male who was involved in a motorcycle accident suffering multiple traumatic injuries including a large skull base fracture that extended through the carotid canal. Computed tomography angiography revealed a 2-cm right ICA pseudoaneurysm. Once the patient was stable, a digital subtraction angiography demonstrated enlargement of the pseudoaneurysm. After his other injuries were addressed by trauma and orthopedic surgery, at day

Contrast extravasation on computed tomography angiography (CTA) is rare but becoming more common, with increasing use of CTA for various cerebral vascular diseases. We report on two cases of spontaneous subarachnoid hemorrhage (SAH) in which the CTA showed a saccular lesion of the upper basilar trunk suggesting a ruptured aneurysm. However, no vascular lesion was observed on immediate subsequent digital subtraction angiography (DSA). In one case, repeated follow up DSA was also negative. The patient was treated conservatively and discharged with no neurologic deficit. In the other case, the pa

Headache can be attributed to cranial or cervical vascular disorders including ischemic stroke or transient ischemic attack, non-traumatic intracranial hemorrhage, unruptured vascular malformation, arteritis, carotid-vertebral artery pain, and cerebral venous thrombosis. Here, we present a case report of unruptured saccular aneurysm with migraine. The patient was a previously healthy 32-year-old man with repeated episodes of headache for 6 years. Findings for computed tomography and magnetic resonance imaging of the head were normal. Head magnetic resonance angiography revealed a small, nodule

Calcification of the portal venous system is a rare entity that can be incidentally discovered during computed tomography (CT). We describe a case of extensive calcifications in the portal venous system in a middle-aged male patient with hepatocellular carcinoma (HCC). This patient presented with epigastric pain that had no obvious origin prior to admission. Laboratory examinations were positive for hepatitis B surface antigen and α-fetoprotein, and severe esophageal and gastric varices were detected during gastroscopy. Abdominal X-ray plain film showed well-defined linear and track-like calci

Hepatic vein stenosis is a rare but serious complication following liver transplantation. Multiple modalities can be utilized to image the hepatic vasculature. Magnetic resonance venography (MRV) provides certain advantages over ultrasound, computed tomography angiography and digital subtraction venography. MRV utilizes the same imaging principles of magnetic resonance angiography in order to image the venous system. Blood pool contrast agents, specifically gadofosveset trisodium, allow for steady state imaging up to 1 h following injection, with improved visualisation of vital venous structur

We report a rare case of posterior communicating artery occlusion mimicking a cerebral aneurysm. A 62-year-old man was admitted to Towakai Hospital with sudden onset of left motor weakness. He had developed thunderclap headache five days before. Computed tomographic scan and lumbar tap were negative for subarachnoid hemorrhage (SAH). Digital subtraction angiography and three-dimensional computed angiography showed aneurysmal protrusion at the junction of the right internal carotid (IC) artery and posterior communicating artery (PcomA). Because minor bleeding from IC-PcomA junction aneurysm was

Clinical findings and the correct assessment of the morphology and degree of stenosis is a decidant point in the conservative and operative therapy of carotid stenosis. Diagnosis should be reached fast, sure and cost-effective with a high sensitivity and specifity. One hundred and fifty-nine patients with cerebrovascular disease underwent digital subtraction angiography (DSA), color flow Doppler (CFD) and CT angiography (CTA).We detected 213 severe (> 80%) stenoses (ICA: 151, ECA: 45, CCA: 17), and 32 occlusions (ICA: 27, ECA: 3, CCA: 2). The 3 methods showed all occlusions (sensitivity: 100%).

Our purpose was to evaluate a dedicated head and neck coil for demonstration of supra-aortic arteries with optimised magnetic resonance angiography techniques. We performed 47 examinations with a 1.5-T system. We used coronal 3D fast imaging with steady precession (FISP), axial 3D tilted optimised nonsaturating excitation (TONE) and 2D fast low-angle shot (FLASH) for the carotid bifurcation, axial 3D TONE with or without magnetisation transfer (MT) for intracranial arteries, and axial 3D FISP or TONE for the aortic arch. Evaluation included visual assessment of image quality and grading of ste

A pilot study was undertaken to evaluate the role of helical computed tomography in the assessment of abdominal aortic pathology. A total of 17 patients underwent intra-arterial digital subtraction of angiography (IADSA) and helical computed tomography, with eight patients undergoing subsequent operative intervention. A comparison of radiological findings between IADSA and helical computed tomography (CT) was made and, where applicable, a comparison was made with operative findings. Pathology included abdominal aortic aneurysm (AAA) (n = 12), thoraco-abdominal aneurysm (n = 2) and dissection (

Reports on simultaneous revascularization operations for concomitant stenoses of the carotid and coronary arteries are many in Europe and America but few in Japan. Herein we report two cases of successful combined carotid endarterectomy and coronary artery bypass grafting. The first case was a 56-year-old male who had effort angina and a right carotid bruit with tinnitus. He was revealed to have severe stenosis of the right internal carotid artery as well as significant lesions of the right coronary artery. The second case was a 69-year-old male who had unstable angina and a left carotid bruit

In a prospective study involving 52 patients, magnetic resonance angiography (MRA) was compared with arterial digital subtraction angiography (IA-DSA). MRA was performed within three days of the IA-DSA. It was carried out without knowledge of the findings on IA-DSA. Of 38 stenoses of the carotid arteries or their branches, demonstrated by IA-DSA, 33 could be seen on MRA; in four cases the stenosis was outside the imaging area of the coil. Sixteen out of 17 carotid occlusions were diagnosed by MRA. There was one false positive. In the vertebral artery territory, eleven out of 13 stenoses and th

BACKGROUND: Spontaneous obliteration of cerebral arteriovenous malformations (AVMs) is a rare phenomenon. Hereditary Hemorrhagic Telangiectasia (HHT) is a predisposal genetic condition for AVMs development in all organs.
CASE ILLUSTRATION: We report the case of a 34 years old woman with HHT family history. After radiosurgical treatment of a symptomatic evolving cerebellar AVM, late control subtracted digital angiography (DSA) demonstrated the complete obliteration of this AVM but also spontaneous obliteration of 3 fronto-parietal AVMs without any hemorrhagic sign on MRI.
CONCLUSION: To our k

Symptomatic cerebral vasospasm after clipping of unruptured intracranial aneurysms (UIA) is extremely rare. Furthermore, its pathophysiology and risk factors are poorly understood. Delayed diagnosis and treatment can lead to severe neurological deficits. We report the case of a 53-year-old woman who developed delayed symptomatic cerebral vasospasm seven days after clipping of an unruptured left middle cerebral artery (MCA) aneurysm. The patient presented with right-sided hemiparesis and motor aphasia. Digital subtraction angiography revealed vasospasms from the M1 to M2 segments. Treatment wit

The Gerbode defect is a rare left ventricle-to-right atrium shunt, often misdiagnosed as tricuspid regurgitation, and may be either congenital or acquired. We report two cases: one acquired Gerbode defect following redo mechanical mitral valve replacement surgery, presenting with dyspnea and progressive heart failure; and one incidental congenital Gerbode defect in an asymptomatic patient. In both cases, Cardiac Magnetic Resonance (CMR) clarified shunt anatomy and informed management decisions. The first patient underwent successful percutaneous closure under Digital Subtraction Angiography (D

A boy in his mid-teens presented with progressively increasing bleeding from the right eye and nostril intermittently over a period of 6 weeks. A complete ophthalmic examination revealed nothing significant. His otorhinological examination and haematological investigations were within normal limits. The patient came a month later with frank bleeding from the right eye. Ophthalmic examination revealed hyperaemia and maceration of the right lower palpebral conjunctiva. A histopathological examination of conjunctival scrapings from the site showed abnormal dilated blood vessels suggestive of a va

Several reports indicate that arterial spin labelling (ASL) MRI is useful for the diagnosis, identification of cortical venous reflux, and assessment of therapeutic effect in dural arteriovenous fistula (dAVF). However, there is no reports indicating the utility of ASL in the identification of venous sinus obstruction. We herein report the case of a 72-year-old woman who presented with diplopia and right trigeminal neuralgia due to bilateral cavernous sinus dAVF. Digital subtraction angiography (DSA) showed temporal occlusion of the inferior petrosal sinus (IPS) and ASL indicated hyperintense
