Neonatal pulmonary hypertension: pathophysiology, classification, and etiology.

Pulmonary hypertension in the newborn infant has many potential causes, and multiple etiologies are common. In this article, the author outlines the basic principles in pathophysiology, classification, and etiology that should be well understood by those who care for neonates with disease processes in which pulmonary hypertension is an important feature.

Causes of stillbirth: a clinicopathological study of 243 patients.

The clinical and autopsy findings in all stillbirths during the years 1974-1979 at the Helsinki University Central Hospital were analysed. There were 243 stillborn infants of whom 200 died before labour. According to the autopsy findings asphyxia accounted for 38% and major malformations for 17% of the deaths, but because of fetal maceration no diagnosis could be made at autopsy in 43%. The cause of death suggested by the clinical findings was placental failure in 57%, cord complication in 12% and major malformations in 17%. The cause of death remained unsolved in 9%. The importance of routine

Cerebral palsy and mental retardation in relation to indicators of perinatal asphyxia. An epidemiologic overview.

Although intrapartum asphyxia is established as an important cause of perinatal loss, there is little consensus as to how much of the burden of neurologic handicap in the community is attributable to intrapartum and neonatal asphyxia, as measured clinically. A review of the available epidemiologic information suggests that the role of perinatal events in the genesis of severe mental retardation and cerebral palsy is not as large as popularly thought. Of all neurologic handicaps, cerebral palsy bears the closest relationship to adverse perinatal events, but at least 50% of all cases have no doc

Estimated cerebral blood flow in term infants with hypoxic-ischemic encephalopathy.

Estimated cerebral blood flow (eCBF) was measured sequentially in seven term infants with evidence of hypoxic-ischemic encephalopathy (HIE) and compared with that of normal-term infants. The eCBF was determined by a noninvasive method involving brief bilateral jugular venous occlusion with simultaneous measurement of occipitofrontal circumference. There was a significant decrease in eCBF on day 2 (30 +/- 4 ml/min/100 g brain weight) and on day 4 (36 +/- 5 ml/min/100 g brain weight) compared with control values (56 +/- 4 ml/min/100 g brain weight; 54 +/- 4 ml/min/100 g brain weight) (P less tha

Morbidity and mortality of advanced gestational age: post-term or postmature.

A 6-year retrospective review of 53 post-term infants admitted to a neonatal intensive care unit (NICU) was undertaken to determine whether morbidity and mortality related to uteroplacental insufficiency were confined to infants with the postmaturity syndrome. Objective identification of infants with significant intrauterine growth retardation (IUGR) was done using the ponderal index (PI). All neonatal deaths occurred in large, appropriately grown infants and were associated with anoxic encephalopathy. Morbidity was equally distributed between normally grown and malnourished infants. Developme

Heart rate variability in full-term normal and abnormal newborn infants during sleep.

Heart rate variability was studied in normal and abnormal newborn infants during sleep in relation to the EEG background. The background EEGs of the full-term newborn infants were classified into normal, minimal depression, mild depression, moderate depression, burst-suppression and flat patterns in order of increasing severity. Nonsequential and sequential histograms were produced with the aid of a computer from some 500 consecutive RR intervals of the ECG recorded on a magnetic tape during polygraphic recordings. The seuential curves mainly showed fast oscillation during quiet sleep and slow

Predictors and survival of birth asphyxia-related neonatal mortality in rural Ghana: a five-year retrospective cohort study (2018-2023).

BACKGROUND: Birth asphyxia remains a leading cause of neonatal mortality in low- and middle-income countries, particularly in rural settings with limited access to skilled intrapartum and neonatal care. Evidence on survival patterns and predictors of birth asphyxia-related neonatal mortality in rural Ghana is limited. This study assessed neonatal mortality among neonates admitted with birth asphyxia and predictors among neonates admitted with birth asphyxia in rural Ghana. METHODS: An institution-based multicentre retrospective cohort study was conducted using routinely collected clinical rec

Association between early hyponatremia and neurological diagnoses in infants with perinatal asphyxia.

UNLABELLED: To investigate the association between early hyponatremia and long-term neurodevelopmental diagnoses in infants with perinatal asphyxia. This retrospective study included infants born at ≥ 36 weeks of gestation with an ICD-10 diagnosis of perinatal asphyxia and/or hypoxic-ischemic encephalopathy. Data on sodium levels within the first 72 h of life, early postnatal weight change, and clinical follow-up were extracted from medical records. The outcome was death or any ICD-10 diagnoses indicating a neurodevelopmental disorder (NDD), including epileptic, paralytic, hearing, visual and

Early postnatal inhibition of neurosteroid synthesis changes the later-life adverse outcome of neonatal asphyxia in rats.

Birth asphyxia (BA) is a common cause of hypoxic-ischemic encephalopathy (HIE), neonatal seizures, and detrimental neurodevelopment. Brain levels of neurosteroids such as allopregnanolone rise in response to acute hypoxic stress, which is thought to represent an endogenous protective mechanism that reduces excitotoxicity in the developing brain. In the present study, we investigated how inhibition of neurosteroid synthesis by the steroid 5α-reductase inhibitor finasteride affects the adverse outcomes of BA/HIE. Intermittent asphyxia was induced in neonatal rats at postnatal day 11. Finasteride

Effect of therapeutic hypothermia on surfactant proteins, anti-inflammatory and pro-fibrotic mediators.

OBJECTIVES: To study the effect of hypothermia on surfactant proteins, anti-inflammatory and pro-fibrotic mediators DESIGN: Prospective, pragmatic study enrolling asphyxiated neonates without lung injury. Surfactant proteins (SP), Club cell secretory protein (CC-10), tumor growth factor-β1 (TGF-β1), human fibroblast growth factor-2 (hFGF) and anti-inflammatory interleukins (IL) were measured in broncho-alveolar lavage fluids obtained before, during and after hypothermia. RESULTS: Twelve neonates were studied. SP-A, -B, -C, -D and CC-10 levels were similar before, during and after hypothermia.

Assessing the influence of abdominal compression on time to return of circulation during resuscitation of asphyxiated newborn lambs: a randomised preclinical study.

OBJECTIVE: During neonatal resuscitation, the return of spontaneous circulation (ROSC) can be achieved using epinephrine which optimises coronary perfusion by increasing diastolic pressure. Abdominal compression (AC) applied during resuscitation could potentially increase diastolic pressure and therefore help achieve ROSC. We assessed the use of AC during resuscitation of asystolic newborn lambs, with and without epinephrine. METHODS: Near-term fetal lambs were instrumented for physiological monitoring and after delivery, asphyxiated until asystole. Resuscitation was commenced with ventilatio

A hybrid approach to skill retention following neonatal resuscitation training: Assessing effectiveness.

Perinatal death, a global health problem, can be prevented with simple resuscitation interventions that help the baby breathe immediately at birth. Latter-day Saint Charities (LDSC) and Safa Sunaulo Nepal (SSN) implemented a program to scale-up Helping Babies Breathe (HBB) training in Karnali Province, Nepal from January 2020-February 2021. The interventions were implemented using a hybrid approach with on-site mentoring in the pre/post COVID period combined with remote support and monitoring during the COVID period. This paper reports overall changes in newborn outcomes in relation to the uni
