Magnetic resonance diagnosis of lipomatous hypertrophy of the atrioventricular grooves mimicking a tumour on echocardiography.

Echogenic structures in the atrioventricular grooves may cause diagnostic problems and may be misinterpreted as a tumour. Images of lipomatous hypertrophy of the atrioventricular grooves diagnosed by magnetic resonance but mimicking a tumour on echocardiography are presented.

Coronary artery stenosis after radiofrequency catheter ablation of accessory atrioventricular pathways in children with Ebstein's malformation.

BACKGROUND: Complications concerning the coronary arteries that are directly related to radiofrequency catheter ablation procedures have not been reported in children. Coronary artery lesions, however, have been demonstrated after the endocardial application of radiofrequency current in young animals. METHODS AND RESULTS: Two boys with Ebstein's anomaly of the tricuspid valve developed clinically asymptomatic coronary artery stenosis after radiofrequency catheter ablation of right-sided accessory atrioventricular pathways with standard catheter technology. CONCLUSIONS: The complication of co

Paradoxical AV delay shortening of a pacemaker.

Paradoxical shortening of the paced AV delay (atrial paced-ventricular paced or Ap-Vp interval) was observed at rest in the DDD and DDDR modes in three patients with implanted CPI Vigor DR pacemakers programmed with a long AV delay and a relatively narrow difference between the lower and upper rates. This behavior is related to the VA extension algorithm designed to prevent the sensor-driven atrial pacing rate from exceeding the programmed upper rate whenever a sensed conducted QRS complex continually follows an atrial stimulus. We found that this algorithm also becomes manifest at rest and ma

Catheter ablation in supraventricular tachycardia.

The evolution of catheter ablation for the treatment of supraventricular tachycardias represents a major advance in the management of cardiac arrhythmias. Excellent results in the majority of patients undergoing the procedure, together with a low rate of early complications and a brief hospitalization, make catheter ablation a highly cost-effective permanent cure. At present, however, its place in relation to alternate therapies in the management of supraventricular tachycardias has not been clearly established owing to unresolved risk-benefit issues. Continuing technical advances will likely

Radiofrequency catheter ablation of concealed atrio-His bypass tract involved in paroxysmal supraventricular tachycardia.

In a patient with paroxysmal supraventricular tachycardia and without any evidence for preexcitation syndrome or dual atrioventricular (AV) nodal pathways, the tachycardia reentry circuit consisted of the AV node as an antegrade limb of the circuit and a concealed atrio-His bypass tract located in the posterior septum as a retrograde limb. During the tachycardia, the atrial potentials in the septal region and coronary sinus were inscribed in the QRS complex, and the earliest atrial activation site was located in the posterior septum. Ventricular extrastimulation at critically short intervals r

[The first experience with surgical treatment of supraventricular tachycardia].

The article analyses the late-term results of operations on 46 patients for paroxysms of supraventricular tachyarrhythmias (SVTA) which were refractory to prophylactic administration of antiarrhythmics. Transvenous electrodestruction of the atrio-ventricular junction were carried out in 39 patients. The results were good in 34 and satisfactory in 3 patients; conductivity and paroxysms reappeared in 2 patients. ++Anti-tachycardial ECS were implanted in 6 patients with good results. One patient underwent electrodestruction of an accessory conduction tract of the type of Kent's bundle with a stab

Electrophysiologic and histologic observations of chronic atrioventricular block induced by closed-chest catheter desiccation with radiofrequency energy.

Direct-current or laser energy has been used to induce atrioventricular (AV) block, but certain complications associated with this type of energy have been reported. We have previously documented that radiofrequency (RF) energy can effectively and safely induce acute AV block in closed-chest dogs during the 4-7 days of follow-up. This study was undertaken to determine if the ablation was permanent and to define the chronic pathology and site of AV block. Complete AV block was successfully achieved in four dogs immediately after ablation with a bipolar "standard" RF output (750 kHz) delivered b

Differential effects of sympathetic activity on AV junctional automaticity and AV conduction.

Rapid ventricular response during episodes of supraventricular tachycardia are often followed, on abrupt cessation of the tachycardia, by prolonged pauses terminated by a sluggish and sometimes erratic escape of a supraventricular pacemaker. Such chronotropic-dromotropic paradoxes are readily reproduced in the animal laboratory following elimination of the sinus node and bilateral decentralization of the stellate ganglia and vagi. This study examined whether left stellate stimulation (0.5, 1, 2, 4, 8 and 16 Hz) or lack thereof differentially affected AV junctional automaticity and AV conductio

Increasing pre-excitation ("concertina effect") during vasospastic angina.

A patient with Wolff-Parkinson-White syndrome and variant angina developed progressive ST-segment elevation in the inferior leads after hyperventilation-induced right coronary artery spasm. At the same time, increasing pre-excitation ("concertina effect") developed with gradual prolongation of the AH interval on His bundle ECG. The ECG changes promptly disappeared after sublingual nitroglycerin, with termination of the chest pain. Transient ischemia induced by coronary artery spasm can be an etiology of increasing pre-excitation.

Surgical treatment of accessory atrioventricular pathways and symptomatic tachycardia in children and young adults.

Twenty-seven patients aged 21 years or younger (mean 15) with symptomatic tachycardia underwent operation for ablation of an accessory atrioventricular pathway. Six patients had associated Ebstein's malformation of the tricuspid valve. Supraventricular tachycardia had been present for a mean of 5 years. At electrophysiologic study, 4 patients were found to have 2 accessory pathways. Left ventricular free wall pathways were found in 14 patients, right ventricular free wall pathways in 10 and septal pathways in 6. Successful initial ablation of all the pathways was achieved in 26 of the 27 patie

Atrioventricular nodal reentrant tachycardia.

This entity represents the most common type of recurrent, regular, narrow QRS tachycardia in the absence of preexcitation syndrome. Electrophysiologic basis for the arrhythmia is reentry within the AV node resulting from dissociation between intranodal pathways. A better understanding of these intranodal pathways helps in the selection of a rational approach to management of these cases.

Amiodarone in atrial fibrillation.

Twenty-seven patients with atrial fibrillation without any concomitant conduction abnormality have been treated with oral amiodarone in a daily maintenance dose of 200 mg. The drug has been used for three purposes: 1) to block atrioventricular conduction, thereby decreasing the ventricular rate during atrial fibrillation (9 patients), 2) as prophylaxis against paroxysmal atrial fibrillation (8 patients), 3) as prophylaxis against recurrence of atrial fibrillation after DC conversion to sinus rhythm (13 patients). All patients were considered refractory to other antiarrhythmic drugs in these re

Verapamil in prophylaxis of paroxysmal atrioventricular nodal reentrant tachycardia.

We examined by electrophysiologic study and clinical follow-up the use of verapamil as a prophylactic agent in 13 patients with refractory paroxysmal supraventricular tachycardia due to atrioventricular (AV) node reentry. Electrophysiologic variables and initiation and maintenance of AV nodal reentry were studied by programmed electrical stimulation. Observations were made before and after intravenous administration of verapamil, 0.15 mg/kg. Twelve of the 13 patients had previously not been controlled by other antiarrhythmic agents. Before verapamil, AV nodal reentry was induced in all 13 pati

Sudden death in hypertrophic cardiomyopathy: associated accessory atrioventricular pathways.

Sudden death is a known but unpredictable complication of hypertrophic cardiomyopathy. We describe two patients who both had a strong family history of the disorder complicated by sudden death. Necropsy disclosed accessory bypass tracts, concealed in one and previously suspected in the other. One died from ventricular fibrillation and the other, who died outside hospital, had previously complained of palpitation. Arrhythmia complicating pre-excitation appears to be one of the factors responsible for sudden death in hypertrophic cardiomyopathy.

A study on the irregularity of the sequence of R-R intervals in chronic atrial fibrillation in man based on the time series analysis and the information theory.

The mechanism of the irregularity of R-R intervals and especially the genesis of the long R-R intervals in atrial fibrillation were clarified. From the point of view of the present study, the sequence of successive R-R intervals can be regarded as a stochastic process, and consequently mathematical analyses can be applied, and the following conclusion could be obtained: the stability of the data, mathematically called stationarity, is present in the sequence of R-R intervals, and these intervals are independently and identically distributed. The probability density function is an Erlangian dis

Involvement of the cardiac conducting system in panarteritis nodosa.

Histopathological observations on the conduction system of the heart were carried out in three cases of panarteritis nodosa. This specialized tissue was involved in each case secondary to ischemia and/or periarterial extension of the inflammatory process affecting the nutrient arteries of the conducting system. The high risk of disturbances in impulse formation and conduction in patients with panarteritis is emphasized as well as the need for appropriate clinical investigation (protracted cardiac monitoring and control by catheter recording and stimulation) in order to secure early detection a
