Epidemiology of U.S. Army cold weather injuries, 1980-1999.

BACKGROUND: Cold weather injuries (CWI) are of great military concern due to their wide-ranging impact on military readiness. Previous short-term studies have identified CWI to be more prevalent in African-Americans, infantrymen, and lower-ranking soldiers. The purpose of this cross-sectional study was to determine the occurrence of CWI hospitalizations in the U.S. Army from 1980 to 1999, and to identify possible trends, high-risk groups and/or activities. METHODS: The U.S. Army Research Institute of Environmental Medicine Total Army Injury and Health Outcomes Database was searched for hospit

Effect of lower body negative pressure against orthostatic intolerance induced by 21 days head-down tilt bed rest.

BACKGROUND: Exposure to actual or simulated weightlessness is known to induce orthostatic intolerance in humans. Many different methods have been suggested to counteract orthostatic hypotension. The repetitive or prolonged application of lower body negative pressure (LBNP) has shown beneficial effects to counter orthostatic intolerance, but devoting so much time to countermeasures is not compatible with space mission objectives or costs. The purpose of the present study was to assess the effects of brief LBNP sessions against orthostatic intolerance during a 21-d head-down tilt (HDT) bed rest.

Pulmonary barotrauma-induced cerebral arterial gas embolism with spontaneous recovery: commentary on the rationale for therapeutic compression.

Pulmonary barotrauma-induced cerebral arterial gas embolism (CAGE) continues to complicate compressed gas diving activities. Inadequate lung ventilation secondary to inadvertent breath holding or rapid buoyant ascent can quickly generate a critical state of lung over-pressure. Pulmonary over-pressurization may also occur as a consequence of acute and chronic pulmonary pathologies. Resulting barotrauma frequently causes structural failure within the terminal distal airway. Respiratory gases are then free to embolize the systemic circulation via the pulmonary vasculature and the left heart. The

Percutaneous aspiration of fluid for management of peritonitis in space.

BACKGROUND: As a medical emergency that can affect even well-screened, healthy individuals, peritonitis developing during a long-duration space exploration mission may dictate deviation from traditional clinical practice due to the absence of otherwise indicated surgical capabilities. Medical management can treat many intra-abdominal processes, but treatment failures are inevitable. In these circumstances, percutaneous aspiration under sonographic guidance could provide a "rescue" strategy. HYPOTHESIS: Sonographically guided percutaneous aspiration of intra-peritoneal fluid can be performed i

Circadian rhythms in airline pilots submitted to long-haul transmeridian flights.

BACKGROUND: Circadian rhythms shift out of phase after transmeridian flights. Desynchronization between body rhythms and the environment is linked to jet lag, which depends on age, flight direction, and number of time zones crossed. METHODS: To investigate this problem in airline pilots, we performed a multivariate analysis of their circadian systems during Madrid-Mexico-Madrid flights (-7 time zones, n = 12) and Madrid-Tokyo-Madrid flights (+8 time zones, n = 21). Telemetry was used to record pilots' activity, skin temperature, and heart rate, obtaining 6 d of continuous data, including 2 d

The role of the central nervous system in heatstroke: reversible profound depression of cerebral activity in a primate model.

BACKGROUND: The neurological manifestations of heatstroke victims vary. The exact sequence of the central nervous system (CNS) changes during lethal hyperthermia has only been partially explored, and the data covering the post-resuscitation CNS changes, which in most cases lead to secondary cardiac arrest, are insufficient. HYPOTHESIS: Following heating of the organism to cardiopulmonary arrest, successful resuscitation may be achieved by standard cardiopulmonary resuscitation (CPR), plus glucose and surface cooling. There is a characteristic sequence of neurological responses to hyperthermia

The exercise and environmental physiology of extravehicular activity.

Extravehicular activity (EVA), i.e., exercise performed under unique environmental conditions, is indispensable for supporting daily living in weightlessness and for further space exploration. From 1965-1996 an average of 20 h x yr(-1) were spent performing EVA. International Space Station (ISS) assembly will require 135 h x yr(-1) of EVA, and 138 h x yr(-1) is planned for post-construction maintenance. The extravehicular mobility unit (EMU), used to protect astronauts during EVA, has a decreased pressure of 4.3 psi that could increase astronauts' risk of decompression sickness (DCS). Exercise

Relaxed acceleration tolerance in female pilot trainees.

BACKGROUND: Female pilots now fly many types of aircraft including military fighters capable of maneuvers that produce high, sustained acceleration in the +Gz axis. Although women have participated as subjects in various centrifuge studies, little is known about the acceleration tolerance of female pilots. METHODS: Between April 1995 and December 1997, 17 female pilot trainees were studied at the Institute of Aerospace Medicine, Bangalore, India. The subjects were 23.2 +/- 1.4 yr old and led physically active lives. Their relaxed +Gz tolerance limits (defined as peripheral light loss) were te

The correlation between aerobic fitness and motion sickness susceptibility.

Susceptibility to motion sickness has been linked to aerobic fitness in several studies, however, these studies have not elucidated the underlying physiological mechanism by which increased aerobic fitness is related to a decreased ability to tolerate motion sickness stimuli. This pilot study provides further evidence of a relationship between aerobic fitness and motion sickness susceptibility. It also suggests that aerobic capacity is more specifically linked to signs and symptoms of vasomotor origin including stomach discomfort, nausea and/or vomiting, headache, and diaphoresis. By independe

Effect of frequency and direction of horizontal oscillation on motion sickness.

BACKGROUND: Low frequency horizontal oscillation can cause motion sickness in some transportation systems, but the influence of the frequency, magnitude, direction, and duration of oscillation are poorly understood. HYPOTHESIS: It was hypothesized that motion sickness was independent of the direction of horizontal oscillation (fore-and-aft or lateral) and that there was no difference in the motion sickness produced by different frequencies of horizontal oscillation (between 0.2 and 0.8 Hz) when subjects were exposed to the same peak velocity of motion at each frequency. METHOD: The 192 subje

Transient and maintained changes of the spontaneous occipital EEG during acute systemic hypoxia.

BACKGROUND: It has been reported that during systemic hypoxia the spectral power of spontaneous EEG (closed eyes) increases, except for the power in the alpha band, which decreases. HYPOTHESIS: It is surmised that after a sudden decrease in inspired oxygen, the EEG parameters and SaO2 show similar temporal characteristics. METHODS: Normobaric acute hypoxia in 14 healthy subjects was evoked by breathing a hypoxic gas. Frequency spectra were calculated from occipital recordings and quantified together with the parameters of the spectral alpha peak. RESULTS: During the first 20 min of exposure

The effect of aircraft control forces on pilot performance during instrument landings in a flight simulator.

BACKGROUND: Pilots may have difficulty controlling aircraft at both high and low force levels due to larger variability in force production at these force levels. The aim of this study was to measure the force variability and landing performance of pilots during an instrument landing in a flight simulator. METHODS: There were 12 pilots who were tested while performing 5 instrument landings in a flight simulator, each of which required different control force inputs. Pilots can produce the least force when pushing the control column to the right, therefore the force levels for the landings wer

In-flight psychiatric emergencies.

BACKGROUND: Although studies have been published regarding airline in-flight medical emergencies, there is a lack of any specific studies about in-flight psychiatric emergencies. This study seeks to ascertain the incidence of in-flight psychiatric emergencies, their associated factors, and outcomes. METHODS: All in-flight calls for physician consultation to MedAire, a leading medical resource for airlines, during 1997 were reviewed for psychiatric symptomatology. RESULTS: Of 1375 consultations, 3.5% were characterized as psychiatric, with a large majority (90%) of these cases presenting prim

Effect of "inside-out" and "outside-in" attitude displays on off-axis tracking in pilots and nonpilots.

BACKGROUND: Pilots employing helmet-mounted displays spend sustained periods of time looking off-axis, necessitating the inclusion of attitude symbology on the helmet to maintain spatial awareness. We examined how fundamentally different attitude references, a moving-horizon ("inside-out") or a moving-aircraft ("outside-in"), affected pilot and nonpilot attitude control when looking on- or off-axis. Both a rear-view and a side-view outside-in perspective were depicted to investigate the effect of control-display compatibility. METHODS: Subjects performed a compensatory pitch-roll tracking tas
