Conduction System Versus Biventricular Pacing in Pacing-Induced Cardiomyopathy: Systematic Review and Meta-Analysis.

BACKGROUND: Biventricular pacing (BVP) and conduction system pacing (CSP) can enhance pacing-induced cardiomyopathy (PICM) patients' clinical outcomes. Although BVP is a class I recommendation in guidelines, CSP may provide a superior physiological alternative to BVP. This meta-analysis compares the efficacy and safety of CSP and BVP in PICM patients. METHODS: Databases including PubMed, Cochrane Library, Web of Science, and Embase were searched from their establishment to June 2025. Data analysis was performed using Stata 17. RESULTS: Twenty observational studies involving 821 patients with

Retrograde Snaring for Left Ventricular Lead Placement in the Presence of a Persistent Left Superior Vena Cava.

Left ventricular lead positioning is technically demanding in cardiac resynchronization therapy (CRT) device implantation, especially in patients with complex cardiac venous anatomies. We report a case in which retrograde snaring was employed to successfully deliver the left ventricular lead through a persistent left superior vena cava for CRT implantation.

Septal scar as a barrier to left bundle branch area pacing.

BACKGROUND: The use of left bundle branch area pacing (LBBAP) for bradycardia pacing and cardiac resynchronization is increasing, but implants are not always successful. We prospectively studied consecutive patients to determine whether septal scar contributes to implant failure. METHODS: Patients scheduled for bradycardia pacing or cardiac resynchronization therapy were prospectively enrolled. Recruited patients underwent preprocedural scar assessment by cardiac MRI with late gadolinium enhancement imaging. LBBAP was attempted using a lumenless lead (Medtronic 3830) via a transeptal approach…

Left Bundle Branch Area Pacing After Subaortic Membrane Resection.

BACKGROUND: Left bundle branch area pacing (LBBAP) is an effective alternative to biventricular pacing for cardiac resynchronization therapy, particularly when coronary sinus anatomy is unfavorable. CASE SUMMARY: A 44-year-old man developed new-onset left bundle branch block and cardiomyopathy following surgical resection of a subaortic membrane. Left ventricular ejection fraction decreased to 30% despite optimal medical therapy. Conventional biventricular pacing failed due to repeated left ventricular lead dislodgment. LBBAP was performed using a deep septal pacing lead targeting the left bu…

Left Bundle Branch Area Pacing to Overcome Coronary Sinus Anatomy-Related Technical Problems Encountered during Implantation of Biventricular CRT-A Case Report.

The results of clinical trials show that up to one-third of patients who are eligible for cardiac resynchronization therapy (CRT) do not benefit from biventricular pacing. The reasons vary, including technical problems related to left ventricle pacing lead placement in the appropriate branch of the coronary sinus. Herein, we present a case report of a patient with heart failure with reduced ejection fraction and left bundle branch block, in whom a poor coronary sinus bed made implantation of classic biventricular CRT impossible, but in whom, alternatively, rescue-performed left bundle branch a…

Strangulation of the heart presenting as sudden cardiac death: A deadly but forgotten complication of epicardial pacing device.

Strangulation of the heart appears to be an exceedingly rare mechanical phenomenon that can develop with epicardial pacing wires in individuals undergoing somatic growth. Over the passage of time, owing to size mismatch between a fixed wire length and dynamically growing heart, the epicardial wire may embed around the heart resulting in the progressive over-pressurizing of involved cardiac structures. Depending on the location of the peak constriction pressures, cardiac strangulation may damagingly affect coronary circulation, heart valves function, or physiologic filling of the heart chambers

Reversible Biventricular Dysfunction Due to Right Ventricular Pacing in a Patient With Prior Cancer Treatment.

BACKGROUND: We report the case of a 54-year-old woman with a history of breast cancer treated with chemotherapy, surgery, and thoracic radio … without pericardial constrictive physiology. DISCUSSION: Given the patient's lack of candidacy for cardiac resynchronization therapy, medical treatment was optimized, leading to significant functional improvement and recovery of biventricular function within 6 months. TAKE-HOME MESSAGE: This case highlights the potential reversibility of pacing-induced cardiomyopathy with optima …

Revisiting Abnormalities of Ventricular Depolarization: Redefining Phenotypes and Associated Outcomes Using Tree-Based Dimensionality Reduction.

BACKGROUND: Abnormal ventricular depolarization, evident as a broad QRS complex on an ECG, is traditionally categorized into left bundle-bra … future cardiovascular disease risk differentially. Additionally, for subjects with LBBB undergoing cardiac resynchronization therapy, tree position predicted cardiac resynchronization therapy response independent of covariates, including QRS duration (adjusted odds ratio [aOR], 0.47 [0.25-0.86], P<0.05). CONCLUSIONS: Our findings challenge the current paradigm, highlighting the …

Sudden-onset severe presyncope in a 67-year-old man.

A 67-year-old man presented to the emergency department with sudden onset of severe presyncope. He reported that he had a permanent pacemaker implanted in 2006 following atrioventricular node ablation for persistent atrial fibrillation (AF). After suffering increasing shortness of breath, he underwent upgrade to cardiac resynchronisation therapy (CRT) in 2016. He denied any recent falls, interventions or changes in medication. ECG monitoring showed AF with a broad ventricular escape rhythm at around 25 bpm with pauses of up to 3 s. Placement of a magnet over the device resulted in pacing (figu

Multiparametric Implantable Cardioverter-Defibrillator Algorithm for Heart Failure Risk Stratification and Management: An Analysis in Clinical Practice.

BACKGROUND: The HeartLogic algorithm combines multiple implantable cardioverter-defibrillator sensors to identify patients at risk of heart failure (HF) events. We sought to evaluate the risk stratification ability of this algorithm in clinical practice. We also analyzed the alert management strategies adopted in the study group and their association with the occurrence of HF events. METHODS: The HeartLogic feature was activated in 366 implantable cardioverter-defibrillator and cardiac resynchronization therapy implantable cardioverter-defibrillator patients at 22 centers. The median follow-u…

Treatment of atrial fibrillation with a dual defibrillator in heart failure patients (TRADE HF): protocol for a randomized clinical trial.

BACKGROUND: Heart failure(HF) and atrial fibrillation(AF) frequently coexist in the same patient and are associated with increased mortality and frequent hospitalizations. As the concomitance of AF and HF is often associated with a poor prognosis, the prompt treatment of AF in HF patients may significantly improve outcome. METHODS/DESIGN: Recent implantable cardiac resynchronization (CRT) devices allow electrical therapies to treat AF automatically. TRADE-HF (trial registration: NCT00345592; http://www.clinicaltrials.gov) is a prospective, randomized, double arm study aimed at demonstrating t

Development of a technique for left ventricular endocardial pacing via puncture of the interventricular septum.

BACKGROUND: Left ventricular (LV) pacing through the coronary sinus is the standard approach for cardiac resynchronization therapy. When this route is unavailable, the alternatives have major limitations. LV endocardial pacing through the interventriuclar septum may offer a simpler solution. We describe an initial case series to demonstrate technical feasibility and to describe our refinement of the puncture technique. METHODS AND RESULTS: Ten patients with previous failed coronary sinus lead implant or with nonresponse to cardiac resynchronization therapy and a suboptimal LV lead position we…
