Emergent Treatment of Acute Cholangitis and Acute Cholecystitis.

Acute pathology of the biliary tract including cholangitis and cholecystitis can lead to biliary sepsis if early decompression is not performed. This article provides an overview of the presenting signs and symptoms and role of interventional radiology in the management of patients with acute cholangitis or acute cholecystitis. It is especially important to understand the role of IR in the context of other treatment options including medical management, endoscopy, and surgery.

Endoscopic transpapillary stenting for the management of acute cholecystitis.

INTRODUCTION: Cholecystectomy is the gold standard treatment of acute cholecystitis. Patients who are considered not to be candidates for cholecystectomy are commonly recommended to undergo percutaneous transhepatic gallbladder drainage (PTGBD) tube placement; however, external drainage is undesirable for many patients. Endoscopic transpapillary stent placement (ETSP) has been described as an alternative method for decompression of the gallbladder. Data in support of this technique is limited to a handful of observational studies with variable indications. Our study sought to expand on the ava…

Acute Acalculous Cholecystitis with Empyema due to Salmonellosis.

Empyema of acalculous gallbladder in the setting of salmonellosis represents an extremely rare and life-threatening clinical condition in adults. In this unique case report, we deal with a previously healthy patient who developed acalculous cholecystitis and empyema due to infection by Salmonella. He underwent explorative laparotomy in emergency setting, and cholecystectomy was performed due to his toxic clinical condition. Empyema of gallbladder was revealed and cultures were collected. A combination of antibiotics (ciprofloxacin and metronidazole) was set, and the patient was discharged 8 da…

Timing of cholecystectomy as the therapy for acute calculous cholecystitis.

INTRODUCTION: Acute calculous cholecystitis is a common disease treated mostly by surgical therapy - laparoscopic cholecystectomy (CHE), particularly upon the common failure of conservative therapy. Timing of the surgery is essential for the development of perioperative complications. METHODS: We carried out a retrospective study with patients hospitalised at our Department of Surgery, University Hospital Královské Vinohrady between January 2013 and December 2015 for the treatment of acute calculous cholecystitis. We had a set of 209 patients. We looked for the presence of perioperative compl…

Utility of liver function tests in acute cholecystitis.

BACKGROUNDS/AIMS: Common bile duct stones (CBDS) affect the management of acute cholecystitis (AC). This study aims to investigate the utility of liver function tests (LFTs) in predicting the presence of CBDS in AC patients. METHODS: Retrospective cohort study of adult patients with AC found in the American College of Surgeons National Surgical Quality Improvement Program database from 2008 to 2016. Patients were classified into two groups, without CBDS (AC-) and with CBDS (AC+). LFT results namely total bilirubin, SGOT and ALP were collected and categorized into normal and abnormal with the …

Computed tomography imaging of complications of acute cholecystitis.

Acute cholecystitis (AC) is a frequent complication of biliary cholelithiasis. Although ultrasound is the first diagnostic imaging procedure, frequently the initial imaging modality is computed tomography (CT). Therefore, familiarization of CT findings in AC and potential related complications are extremely important. This pictorial essay reviews a broad spectrum of complications related to AC and its key findings in CT.

Prediction of difficult laparoscopic cholecystectomy for acute cholecystitis.

BACKGROUND: No report has described the predictive factor of surgical difficulty for laparoscopic cholecystectomy (LC) by preoperative computed tomography (CT) findings. This study aimed to investigate whether dynamic CT findings can predict the difficulty of LC for acute cholecystitis. MATERIALS AND METHODS: Fifty-seven patients who underwent emergency LC and dynamic CT preoperatively were enrolled. Difficult LC (DLC) was defined as any patient with an operative time ≥3 h, bleeding volume ≥300 mL, common bile duct injury, partial cholecystectomy, the need for a second surgeon, and/or convers…
