Better fitting of cochlear implants: modeling loudness for acoustic and electric stimuli.

It is becoming more common for cochlear implant (CI) recipients to have some useful acoustic hearing post-operatively. Such CI users may benefit from simultaneous use of an acoustic hearing aid (HA). To provide a satisfactory fitting of both types of devices, it is helpful to make the loudness perceived via each device as similar as possible. In practice, this is usually attempted by adjusting the sensitivity and/or gain of each device during fitting, but it is often not clear whether or how well the loudness of the devices has been matched for each user. In this study, established numerical m

Sensitivity to interaural time differences with combined cochlear implant and acoustic stimulation.

The interaural time difference (ITD) is an important cue to localize sound sources. Sensitivity to ITD was measured in eight users of a cochlear implant (CI) in the one ear and a hearing aid (HA) in the other severely impaired ear. The stimulus consisted of an electric pulse train of 100 pps and an acoustic filtered click train. Just-noticeable differences (JNDs) in ITD were measured using a lateralization paradigm. Four subjects exhibited median JNDs in ITD of 156, 341, 254, and 91 mus; the other subjects could not lateralize the stimuli consistently. Only the subjects who could lateralize ha

Survey of prophylactic antibiotic use amongst UK cochlear implant surgeons.

Cochlear implant patients are at an increased risk of pneumococcal meningitis. Recent government guidelines require all implant patients to undergo pneumococcal vaccination. The guidelines also suggest antibiotic prophylaxis but no clear guidelines regarding which antibiotic to use or for how long were issued. We asked each implant centre within the UK to describe their antibiotic protocol for cochlear implantation. Our results have showed that 100% of UK implant surgeons use antibiotic prophylaxis. The type of antibiotic and duration vary significantly between centres. Interestingly, however, t…

Auditory performance of cochlear implant children aged 2-5 years.

OBJECTIVE: To assess the categories of auditory performance in prelingual deaf children after implantation. STUDY DESIGN: Prospective study … valuate the outcome of auditory receptive abilities in young congenital deaf children who underwent cochlear implantation. The accessible outcome measurement will provide information for parents and professionals to obtain a hierarchical scale on which the children's auditory ability with other more formal measures may be inappropriate.

Effects of insertion depth of cochlear implant electrodes upon speech perception.

OBJECTIVE: To investigate whether the insertion depth of a cochlear implant array affects postoperative speech perception. DESIGN: The subjects were 48 postlingually deaf adults who received either the Nucleus 22 or the Nucleus 24 cochlear implant with a straight array. A postoperative radiograph of the cochlear electrode was used to estimate insertion depth, as either the angle of the electrode tip (angle) or the intracochlear length of the electrode (length). Other estimates of insertion depth included the numbers of active electrodes and channels used by the speech processor. Electrode dep

Clinical relevance of the distance between the cochlea and the facial nerve in cochlear implantation.

HYPOTHESIS: To elucidate possible mechanisms of facial nerve costimulation after cochlear implantation that are supposed to result from the close cochlea to facial nerve contact. BACKGROUND: One of the postoperative complications of cochlear implantation is facial muscle twitching, which has preferentially been found in otosclerotic patients. It impairs hearing benefits because of deactivation of electrodes and can still not be adequately prevented. METHODS: A total of 13 temporal bones were dissected to quantify where the labyrinthine portion of the facial nerve is closest to the scala tymp…

Aspects of temporal bone anatomy and pathology in conjunction with cochlear implant surgery.

Cochlear implantation is a treatment for patients with severe sensorineural hearing loss/deafness, who get no help from ordinary hearing aids. The cochlear implant is surgically placed under the skin near the ear and a very thin electrode array is introduced into the cochlea of the inner ear, where it stimulates the remaining nerve fibers (1,2). The operation is complicated; it is performed with the aid of a microscope, and involves drilling very close to vital vessels and important nerves. The method was introduced in Sweden in 1984 by Professor Göran Bredberg, then at Stockholm Söder Hospita…

Further experience with fat graft obliteration of mastoid cavities for cochlear implants.

Obliteration of old mastoids and wet middle ears with autologous abdominal fat seems to be a reliable technique to render chronically discharging mastoid cavities or open middle ears dry and closed. This paper is the third in the series and looks at the intermediate results at five years. Of the 16 patients (one bilateral) 94.1 per cent of the ears are still dry and uninfected with closed external meati. Recurrent cholesteatoma was found in two patients at implantation and removed.

Electrical middle ear muscle reflex: use in cochlear implant programming.

Programming of multichannel cochlear implants (CIs) requires subjective responses to a series of sophisticated psychophysical percepts. It is often difficult for young prelinguistically deaf children to provide adequate responses for device fitting. This is especially true in setting levels of maximum comfortable loudness, whereby failure to indicate growth of loudness may result in elevation of stimulus levels to the threshold of pain. The acoustic or stapedial muscle reflex has been used previously to provide objective confirmation of acoustic stimulation, and there have been attempts to use

[Indications for middle ear obliteration within the scope of cochlear implant management].

BACKGROUND: Cochlear implants have gained worldwide acceptance as a reliable method of rehabilitation of profoundly hearing-impaired patients. Due to thorough patient selection major postoperative complications rarely occur and are flap related in most cases. Deafness can develop during chronic suppurative otitis media, either coincidentally or secondary to the medical treatment; normally this condition is regarded as a contraindication for cochlear implantation. In cases with a mastoid cavity after surgical treatment for cholesteatoma, the electrode covered only by the epithelial lining will …

An integrity test battery for the Nucleus Mini 22 Cochlear Implant System.

The probability of system failures increases as the number of cochlear implants increases throughout the world. Whether a malfunction is a technical or physiological problem remains to be defined, particularly in very young children, while a psychogenic hearing disorder after implantation must not be excluded in adults. The battery of objective measurements used clinically at the Medizinische Hochschule, Hannover has provided useful diagnostic information for distinguishing possible causes of failure. In a normally functioning device, an electrical signal equivalent to the biphasic rectangular

[Loudness scaling in cochlear implant patients within the scope of preoperative evaluation].

The electrical stimulation of the acoustic nerve is essential for evaluating patients for cochlear implants. Different tests are used to assess the performance of the cochlear nerve in a deaf ear. The use of category scaling of loudness leads to a more efficient test routine.

Cochlear implants and otitis media: considerations in two cleft palate patients.

Cochlear implantation is a safe and effective means of rehabilitating profound sensory deafness. The implications of coincident chronic middle ear disease upon the operative procedure and auditory results have received little attention in the literature. We describe two patients, each with congenital cleft lip and palate and secondary chronic otitis media with effusion, who received multichannel cochlear implants following bacterial meningitis. One patient (a three-year-old girl) was implanted in the standard fashion while the other (a 38-year-old-man) underwent middle ear and mastoid oblitera…

Temporal bone histopathology of a patient with a nucleus 22-channel cochlear implant.

The temporal bone histopathology of a patient with profound sensorineural hearing loss attributable to cochlear otosclerosis is presented. The patient had a Nucleus 22-channel cochlear prosthesis implanted in the right ear prior to his demise. Unintentional scala media insertion of the electrode array resulted from labyrinthitis ossificans obliteration of the scala tympani. Inadvertent facial nerve stimulation necessitated program exclusion of electrodes 11 to 16 located in proximity to the labyrinthine and geniculate segments of the facial nerve. The patient received significant benefit from

Previous experience as a confounding factor in comparing cochlear-implant processing schemes.

It is of great importance to compare the relative merits of different cochlear-implant speech-processing strategies. Some groups have compared different strategies within single subjects, but usually the subject has prior experience with one strategy, and no allowance is made for this prior experience. We show in the present study that this is inappropriate. We tested one subject using the Melbourne (Cochlear Corp.) multichannel implant with the device set to process sounds in two different ways. In the first processing scheme, the device functioned normally, extracting information about voici
