Possible drug interaction between warfarin and combination of levamisole and fluorouracil.

OBJECTIVE: To report a possible drug interaction between the combination of fluorouracil (5-FU), levamisole, and warfarin. CASE SUMMARY: An elderly patient with chronic atrial fibrillation and prosthetic valve replacements had been taking warfarin 22.5 mg/wk. Following the diagnosis of colon cancer (Duke's classification D), a chemotherapy regimen of 5-FU and levamisole was started. Within four weeks after initiation of chemotherapy, the International Normalization Ratio (INR) increased from 3.04 to 39.56. Warfarin was discontinued and restarted at 7.5 mg/wk. Discontinuation of levamisole and

The interaction of antiarrhythmic drugs and the energy for cardioversion of chronic atrial fibrillation.

Antiarrhythmic drugs may alter the energy for cardioversion of ventricular arrhythmias. This study compares the energy necessary for cardioverting chronic atrial fibrillation in 57 patients taking type Ia, Ic, or type III antiarrhythmic drugs. Patients taking Ia (n = 22) or III (n = 14) drugs had a median energy for cardioversion of 100 joules, while the patients taking Ic (n = 17) drugs had a median energy of 200 joules (P = 0.03). There were no differences in the frequency of unsuccessful cardioversion. There were no serious adverse events in any of the three groups, although three patients

In vitro and in vivo interactions of drugs used in multidrug therapy in leprosy.

Interactions of different drugs commonly used in multiple drug therapy were evaluated using both in vitro culture (cell-free as well as macrophage) system and mouse footpad. No additive effects were obtained in the in vitro system when dapsone was combined with either rifampicin or clofazimine, while a strong antagonism was observed when clofazimine was combined with rifampicin but not with rifabutin. In the mouse footpad system, a strong synergism was obtained when clofazimine was combined with either rifampicin or rifabutin, but significant antagonism was observed with the combination of clo

Drug interaction. Effects of salicylate on pharmacokinetics of valproic acid in rats.

The effects of salicylic acid on the pharmacokinetics of valproic acid were investigated in bile-exteriorized rats. A 50 mg/kg bolus dose of sodium valproate was injected iv to Long Evans rats with and without (control) prior treatment by constant infusion of salicylate to keep it at steady state plasma level (about 250 micrograms/ml). The plasma elimination of valproic acid followed a monoexponential decline in both salicylate-treated and control rats. A significant increase (p less than 0.01) in the disposition rate constant (kel), the volume of distribution (Vd), and the total clearance (Cl

Antiallergic drugs and the immune response. Interactions and possible clinical relevance.

Since the pharmacological treatment of allergic diseases is used in patients with alteration of the immune response due to atopy and possible concomitant infections, we have investigated the possible effects of such drugs as cromolyn, theophylline, ketotifen, oxatomide, astemizole, fenoterol, pirenzepine, and rosaprostol on the in vitro immune response, in order to obtain experimental data and, as a consequence, clinical conclusions. In a series of investigations by our group and other authors the following immunological parameters have been considered: T cell activation induced by different p

Interaction of carbamazepine and other drugs with adenosine (A1 and A2) receptors.

The tricyclic anticonvulsant carbamazepine (CBZ) is effective in pain and affective disorder, but the mechanism of action for this drug has not been defined. Recently it was reported that CBZ had interaction with adenosine receptor, which is related to the inhibition of release of neurotransmitter. In the present study, we investigated the in vitro effects of CBZ and other drugs upon adenosine receptor binding using 3H-L-phenylisopropyladenosine (A1) and 3H-N-ethylcarboxamidoadenosine (A2). The following results were obtained: CBZ and its derivative oxcarbazepine inhibit 3H-PIA binding at ther

Characterization of interactions of phenothiazines and related drugs with lipids by UV-spectrophotometry.

The UV-spectrum of chlorpromazine undergoes a red shift in the presence of vesicles of biological membranes or phospholipids, triglycerides, serum lipoproteins or fatty acids. The resulting difference spectrum has two positive peaks at about 260 and 320 nm and two negative peaks at 250 and 290 nm. This interaction signal, which was elicited in the presence of as little as 3 muM oleic acid, was dependent on the concentrations of both ligand and binder. It was abolished by 8 M urea, diminished by temperature increase up to 70 degrees C, but not changed by varying the ionic strength from 0 to 0.5

Interaction between centrally acting hypotensive drugs and tricyclic antidepressants.

From clinical experience it is known that the hypotensive action of clonidine and alpha-methyl-DOPA is antagonized by desipramine. This antagonism was investigated in detail in chloralose-anaesthetized cats. Both the centrally acting hypotensive agents and the tricyclic antidepressants where infused into the left vertebral artery. (1) The central hypotensive action clonidine was antagonized by desipramine, imipramine, amitriptyline, protriptyline and mianserine. For the inhibition of the hypotensive action of clonidine by protriptyline a parallel shift of the dose-response curve was obtained,

The Use of Transporter Probe Drug Cocktails for the Assessment of Transporter-Based Drug-Drug Interactions in a Clinical Setting-Proposal of a Four Component Transporter Cocktail.

Probe drug cocktails are used clinically to assess the potential for drug-drug interactions (DDIs), and in particular, DDIs resulting from coadministration of substrates and inhibitors of cytochrome P450 enzymes. However, a probe drug cocktail has not been identified to assess DDIs involving inhibition of drug transporters. We propose a cocktail consisting of the following substrates to explore the potential for DDIs caused by inhibition of key transporters: digoxin (P-glycoprotein, P-gp), rosuvastatin (breast cancer resistance protein, BCRP; organic anion transporting polypeptides, OATP), met…

Strategies for modelling the intestinal mucus interface: Focus on drug transport and host-microbiota interactions.

The intestinal mucus barrier is a complex, dynamic hydrogel that plays a central role in host-microbiota interactions, nutrient transport, and protection against pathogens. Understanding its physicochemical properties has become essential for developing predictive in vitro and in silico models for drug delivery, microbiome research, and disease modelling. Over the past decade, advances in polymer chemistry, glycoscience, and microfluidics have enabled the development of increasingly sophisticated mucus models that mimic structural, rheological, and biochemical features of native intestinal muc

Bispecific antibody-drug conjugates in cancer: therapeutic-window determinants, tumour-immune interactions, and clinical translation.

Antibody-drug conjugates (ADCs) have established targeted payload delivery as an important anticancer strategy, but their efficacy remains constrained by antigen heterogeneity, incomplete internalisation, resistance and dose-limiting toxicity. Bispecific antibody-drug conjugates (BsADCs) add a second antigen- or epitope-binding specificity with the aim of broadening tumour coverage or altering receptor trafficking. The field crossed a major translational threshold in 2026, when izalontamab brengitecan (iza-bren; BL-B01D1), an EGFR×HER3 BsADC, received its first regulatory approvals in China fo

Managing High-Risk Drug Interactions With Nirmatrelvir/Ritonavir: Experience From a Dedicated Pharmacology Advice Service.

Nirmatrelvir boosted with ritonavir is a key antiviral against COVID-19, yet ritonavir's potent CYP3A4 inhibition exposes patients to numerous drug-drug interactions. To make prescribing safer, a national proactive system providing expert pharmacology advice was implemented in France. This retrospective study reports the experience of the pharmacology department at Rennes University Hospital, which provided these expert services from February 2022 to December 2024. We evaluated 455 consecutive patients (median age 75 years, median eight co-medications per patient, 3813 prescription lines). Nir…

MLI-DTA: An interpretable multimodal framework with multi-level interactions for drug-target affinity prediction.

Drug-target affinity prediction provides a computational basis for virtual screening and drug repositioning optimization by estimating the binding strength between compounds and target proteins. Existing deep learning methods have evolved from early SMILES/amino acid sequence modeling to graph neural networks and multi-modal fusion. However, there are still three challenges: one-dimensional sequence models struggle to explicitly represent atomic connections and residue interactions; some graph models incorporate molecular or protein contact graphs, but often fail to properly align the semantic

Drug-gene interactions and the risk of diabetic microvascular complications: A population-based cohort study.

AIMS: Drug-gene interactions (DGIs) modify drug response and safety, yet their influence on diabetic microvascular complications remains unclear. This study aimed to elucidate the role of DGIs in these complications. MATERIALS AND METHODS: Using UK Biobank (UKB) data, we identified medications frequently prescribed to individuals with diabetes and defined DGIs based on the Food and Drug Administration (FDA) and the Clinical Pharmacogenetics Implementation Consortium (CPIC) guidelines. Associations between DGIs and diabetic microvascular complications were evaluated using Cox proportional haza

Preventing drug interactions with antiretrovirals in the aging HIV population: focusing on population pharmacokinetic modeling.

INTRODUCTION: As people with HIV live longer given the success of antiretroviral therapy, the population of older individuals with HIV is rapidly expanding, bringing new challenges related to aging, multimorbidity, and polypharmacy. Age-associated physiological changes - including alterations in renal and hepatic function, body composition, plasma protein binding, and gastrointestinal absorption - can substantially influence drug pharmacokinetics (PK). In individuals with HIV, these changes may be further compounded by chronic inflammation, immune activation, and long-term antiretroviral expos
