While most anuran species are highly vocal, few of them seem to be endowed with a complex call repertoire. Odorrana tormota, combines a remarkable vocalization complexity with auditory sensitivity over an extended spectral range spanning from audible to ultrasonic frequencies. This species is also exceptional for its ability to modify its middle ear tuning by closing the Eustachian tubes (ET). Using scanning laser Doppler vibrometry, the tympanal vibrations were measured to investigate if the tuning shift caused by the ET closure contributes to intraspecific acoustic communication. To gain ins…

Wideband acoustic immittance (WAI) measures are noninvasive diagnostic measurements that require an estimate of the ear canal's area at the measurement location. Yet, physical measurements of the area at WAI probe locations are lacking. Methods to measure ear-canal areas from silicone molds were developed and applied to 169 subjects, ages 18-75 years. The average areas at the canal's first bend and at 12 mm insertion depth, which are likely WAI probe locations, were 63.4 ± 13.5 and 61.6 ± 13.5 mm2, respectively. These areas are substantially larger than those assumed by current FDA-approved WA…

Today, the standard method to predict output levels of active middle ear implants (AMEIs) before clinical data are available is stapes vibration measurement in human cadaveric ears, according to ASTM standard F2504-05. Although this procedure is well established, the validity of the predicted output levels has never been demonstrated clinically. Furthermore, this procedure requires a mobile and visually accessible stapes and an AMEI stimulating the ossicular chain. Thus, an alternative method is needed to quantify the output level of AMEIs in all other stimulation modes, e.g. reverse stimulati…

BACKGROUND: Bacterial infection, Eustachian tube dysfunction, allergies, and immunologic factors are major causes of otitis media with effusion (OME). However, the exact pathogenesis of OME is still unclear. This study evaluated whether allergy influences bacterial growth in middle ear effusions.
MATERIALS: Fifty-four samples were obtained from OME patients 3-10 years of age who underwent ventilation tube insertion and were divided into two groups based on the presence of allergy as determined using the multiple allergosorbent test (MAST). Streptococcus pneumoniae, Haemophilus influenzae, and…

The medical charts of 41 ears with congenital middle ear malformation with both an intact external ear canal and a mobile stapes footplate were reviewed retrospectively to study the Carhart effect. The operations were categorized as successful or unsuccessful according to the extent of decrease in the average air-bone gap. Statistically significant differences were observed between the 2 groups with respect to the changes in pure-tone average and the changes in the bone conduction (BC) threshold at 1 and 2 kHz. Linear regression analysis revealed weak correlations between the change in the BC …

It is unknown whether childhood ear disease could be present long before symptoms provoke an initial otoscopic examination. A newborn middle ear might or might not start in a pristine, privileged state. The clinician evaluating later infant and childhood ear disease is often unaware of the status of a patient's ear from the neonatal period, the earliest time at which the tympanic membrane can be evaluated. Adding to the physician's handicap, normative otoscopic and histologic data on the neonatal ear are incomplete. In order to test the hypothesis that disease in the neonatal middle ear may be…

Otoacoustic emissions (OAEs) evoked by clicks and tone bursts (TBs) were measured using a minor modification of the 1987 Bray and Kemp system in normal and hearing-impaired ears with high-frequency sensorineural hearing loss. Sixty ears of 60 subjects were tested. The average behavioral hearing threshold of 20 normally hearing ears was measured for the different "nonlinear" stimulus groups and defined as 0 dBnHL. Emissions were recorded in another 20 normally hearing ears and in 20 ears with steep high-frequency sensorineural hearing loss above 2kHz. An unfiltered click of 80 microseconds dura…

Hearing loss and dizziness are the most common sensory disorders worldwide. About 5% of the world's population is affected by disabling hearing loss, and the prevalence is projected to increase over the next several decades. Cochlear hair cells are essential in converting sound vibrations into electrical signals, which are then relayed centrally via the spiral ganglion neurons and lead to sound perception. The irreversible loss of cochlear hair cells is the most common pathology underlying sensorineural hearing loss in humans. In stark contrast, nonmammalian species such as chicken and zebrafi…

Despite being a highly advantageous trait, tympanic hearing was secondarily lost multiple times within tetrapods. Typically, loss of a sensory structure implies a shift to new environments where selection for another trait inhibits the development of said sensory structure, or a relaxation of selection on the sensory organ. However, the loss of tympanic hearing within North American sand lizards (Phrynosomatidae: Callisaurini) has eluded explanation. To explore the evolutionary reduction and loss of tympanic hearing within this clade, we created 3D models of the stapes to analyze morphological…

Body weight can have various effects on middle ear health. Pediatric obesity is associated with an increased risk and recurrence of otitis media with effusion. Adult obesity is associated with an increased risk of temporal bone cerebrospinal fluid leak and encephalocele, and weight loss can cause symptoms of patulous Eustachian tube dysfunction. With rising rates of obesity and widespread adoption of glucagon-like peptide 1 receptor agonists, it is increasingly important that patients are appropriately counseled on how weight changes affect the middle ear. In this review, we examine the litera…

BACKGROUND: Middle ear actinomycosis is a rare condition, with approximately 50 reported cases in the literature. It typically occurs due to dissemination through the Eustachian tube from the nasopharynx to the middle ear and mastoid cavity and presents as chronic suppurative otitis media (CSOM), requiring long-term antibiotics and surgical debridement.
CASE DESCRIPTION: We present two pediatric patients with middle ear actinomycosis. The first is a 7-year-old boy with Joubert syndrome who presented with right-sided persistent otorrhea 6 months after ventilation tube (VT) insertion. Histopath…

Neuroendocrine tumors originating in the middle ear are rare, and presentation with facial paralysis is extremely uncommon. A 19-year-old woman with a history of transient facial paralysis presented with a right-sided middle ear mass. Surgical exploration of the tympanic cavity was performed using a transcanal endoscopic approach. Histopathologic examination confirmed the diagnosis of a middle ear neuroendocrine tumor. After surgical excision of the tumor, no recurrence or facial paralysis occurred. Herein, the authors present this rare case, along with a literature review.

BACKGROUND: Total ear reconstruction in patients with microtia remains a complex aesthetic and technical challenge. Accurate positioning and customization of porous polyethylene implants are critical, yet current methods often rely on manual planning and intraoperative adjustments.
METHODS: At Schneider Children's Medical Center, high-resolution 3D facial scans or CT data were used to design patient-specific preoperative jigs and a custom single-piece implant. The jig guided precise ear positioning using facial landmarks and incorporated vascular mapping via CTA to identify branches of the su…

Petrous bone cholesteatoma, or cholesteatoma that extends beyond the middle ear and mastoid, represents a rare but destructive pathology. Diagnosis can be difficult before substantial morbidity is incurred, and patients can present with life-threatening complications. Determination of disease extent and the functional status of the facial nerve and cochleovestibular system are critical in surgical planning. Typically, surgery involves ablative procedures with the goal of complete disease resection given the low likelihood of preserved inner ear function. In experienced hands, disease control a…

This study delves into the intricate mechanics of human middle ear implants by examining a lumped parameter model with five degrees of freedom to estimate sound transfer. The ASTM standard, recognized globally as a benchmark, served as a reference for analysis, ensuring test accuracy and providing a comprehensive evaluation framework. To assess the implant's usability, numerical simulations were conducted and compared against both the ASTM standard and the experimental results obtained from temporal bone studies. This investigation uncovered the bistability of periodic responses induced by the…

OBJECTIVES: Middle ear cholesteatoma is a non-tumorous condition that typically leads to hearing loss, bone destruction, and other severe complications. Despite surgery being the primary treatment, the recurrence rate remains high. Therefore, exploring the molecular mechanisms underlying cholesteatoma is crucial for discovering new therapeutic approaches. This study aims to explore the involvement of N6-methyladenosine (m6A) methylation in long non-coding RNAs (lncRNAs) in the biological functions and related pathways of middle ear cholesteatoma.
METHODS: The m6A modification patterns of lncR…

The middle ear ossicles in modern mammals are repurposed from postdentary bones in non-mammalian cynodonts. Recent discoveries by palaeontological and embryonic studies have developed different models for the middle ear evolution in mammaliaforms. However, little is known about the evolutionary scenario of the middle ear in early therians. Here we report a detached middle ear preserved in a new eutherian mammal from the Early Cretaceous Jehol Biota. The well-preserved articulation of the malleus and incus suggest that the saddle-shaped incudomallear joint is a major apomorphy of Early Cretaceo…
