Clinical biomechanics of the elbow in tennis: implications for evaluation and diagnosis.

Elbow injuries constitute a sizeable percentage of tennis injuries. Biomechanical analysis of the forces, loads, and motions on the elbow in tennis, and the constraint systems operating the control the forces, can lead to an understanding of the pathophysiology of these injuries. A biomechanically based evaluation framework can be used to document all of the clinical symptoms, anatomic alterations, and biomechanical alterations that are associated with the pathological problem.

Computed tomographic arthrography in the assessment of osteochondritis dissecans of the elbow.

Eleven cases of osteochondritis dissecans of the elbow are reviewed. Computed tomography (CT) accurately identified the abnormality, its extent and its precise location. Computed tomographic arthrography (CTA) allowed accurate delineation of the overlying cartilage in all cases, identifying cartilage defects in four patients, fissuring in two and cartilage thinning in a further two. In those patients who had plain CT, four were shown to have loose bodies, one of which was obscured by contrast medium at subsequent CTA. Two further patients with loose bodies were identified using CTA alone. The …

Arthroscopic fenestration of the olecranon fossa in the treatment of osteoarthritis of the elbow.

A new arthroscopic approach to the treatment of elbow osteoarthritis that permits debridement of the elbow joint together with the removal of intraarticular loose bodies is described. Twelve patients (11 male, one female) with an age range of 29-64 (mean 47.5 years) underwent the procedure. Post-surgery all patients had relief of elbow-locking and experienced a reduction in elbow pain (follow-up 3-30 months, mean 16 months). The technique is offered as a treatment option for patients with elbow osteoarthritis.

Surgical treatment of medial epicondylitis. Results in 35 elbows.

We reviewed 35 of 38 consecutive patients who had operative treatment for medial epicondylitis of the elbow after the failure of conservative management. Their mean age was 43 years and mean follow-up was 85 months. At operation residual tears with incomplete healing were consistently found in the flexor origin at the medial epicondyle and microscopy showed reactive fibrous connective tissue with varying degrees of inflammation. The mean subjective estimate of elbow function was improved from 38% to 98% of normal, while isokinetic and grip strength testing in 16 patients showed no significant …

[Fractures of the lateral condyle of the elbow in children. Radiologic aspects].

Fractures of the lateral condyle of the humerus classified as type IV in Salter schema are difficult to diagnose when the capitellum is not ossified. Therefore it is necessary to know exactly the radiological lines which are constructed from the humerus and the radius to the capitellum. Diagnosis is also difficult when the ossification center of the capitellum humeri is only slightly displaced. The epiphyseal fracture line which is always visible at surgery can be missed on X-Ray when ossification has not yet appeared. Laminograms are necessary in some cases to be sure that the fracture is dis

Floating dislocated elbow: case report and review of the literature.

A 59-year-old shrimper sustained ipsilateral fractures of the midshafts of the humerus, ulna, and radius, as well as an ipsilateral posterior dislocation of the elbow. The mechanism of this unusual injury involved the winch system of a shrimpboat. Closed reduction of the dislocation along with open reduction and internal fixation of the fractures allowed early elbow motion. The resulting final range of motion and function were good.

Massive allograft replacement of hemiarticular traumatic defects of the elbow.

Four elbow osteoarticular allografts were done for four patients as salvage procedures for unreconstructable elbow fracture malunions. With a mean follow-up of 60 months (range, 12 to 72 months) all elbows were stable, free of pain, and had mean motion of 130 degrees active flexion and 27 degrees of flexion deformity, 67 degrees pronation and 62 degrees supination (preoperative mean: 104 degrees flexion, 42 degrees flexion contracture, 20 degrees pronation, and 34 degrees supination). Complications occurred in two elbows. One had a deep infection necessitating graft removal and subsequent regr…

Lateral epicondylitis. A review of structures associated with tennis elbow.

139 limbs from embalmed specimens were dissected to reveal the attachments of extensor muscles in the vicinity of the lateral epicondyle. M. extensor carpi radialis brevis was found to consist of a keel-shaped tendon with attachments to m. extensor carpi radialis longus, m. extensor digitorum communis, m. supinator; and to the radial collateral ligament, the orbicular ligament, the capsule of the elbow joint and the deep fascia. On 29 limbs, a prolongation of the muscle was identified attaching proximal to the lateral epicondyle. On nine specimens a bursa was evident between the capsule over t…

Posttraumatic bacterial arthritis with luxation of the elbow. A case report.

Trauma predisposes an individual to bacterial arthritis. This report describes a case of posttraumatic bacterial arthritis with luxation of the elbow. It was necessary to perform synovectomy as a complement to systemic antibiotic treatment in order to control infection and reduce the dislocation. The use of external fixation is demonstrated.

A review of the Liverpool elbow prosthesis from 1974 to 1982.

Eighty elbows in 65 patients with an average age of 57 years have had two-part non-constrained Liverpool elbow arthroplasties performed since 1974. Fifty-five had rheumatoid arthritis, eight osteoarthritis or ankylosis secondary to injury, one osteochondritis dissecans and one pyknodysostosis. The average preoperative range of movement was 42 degrees to 112 degrees with 47 degrees of pronation and 42 degrees of supination. There was significant gain in the arc of movements at follow-up: 32 degrees in the extension-flexion range (average range 32 degrees to 134 degrees of flexion) and 42 degree…

[Osteochondritis dissecans of the elbow (presentation of 2 rare cases)].

The authors show two cases of dissecting osteochondritis of elbow, confirming the frequent absence of a clinical symptomatology in the initial stages of such disease and the benignancy of its course. From a careful study of the casuistries present in literature, what emerges is the rareness of this location, together with a prudent therapeutical trend with periodic clinical and radiographical checkings, reserving the surgical solution to the cases of articular free body with episodes of block.

Fractures and dislocations about the elbow in the head-injured adult.

Of 548 head-injured adults, 16 sustained injuries about the elbow (3%). Sixteen of 18 patients (89%) developed heterotopic ossification (HO). All elbows which had been dislocated developed new bone in the substance or adjacent to the medial or lateral collateral ligaments, or both. Some dislocations also developed significant HO in all planes. Final motion correlated with the amount of HO in the joint. Supracondylar fractures generally developed a moderate amount of anterior or posterior HO, or both. Motion was moderately compromised. Posteriorly located HO developed in olecranon fractures. Ra…

Biomechanics of elbow instability: the role of the medial collateral ligament.

The anterior oblique component of the medial collateral ligament of the elbow is the mainstay of joint stability. Fractures of the medial epicondyle must be anatomically reduced, open if necessary. A fibrous union of a minimally displaced fractured medial epicondyle may result in lengthening and functional compromise of the medial collateral ligament. Chronic elbow instability is an unusual lesion. Repair of chronic elbow instability is best performed by restitution of medial collateral ligament function.

Osteochondritis dissecans of elbow, ankle and hip: a comparison survey.

In 36 patients treated for osteochondritis dissecans (OCD) of the elbow, ankle and hip during a period of 20 years in the same hospital, trauma seems to have been the main etiologic factor in about half of the patients. The first symptoms of the lesions occurred when the patients were between 15 and 20 years of age. Osteochondritis dissecans of the elbow was seen in 19 men. Osteochondritis dissecans in the ankle occurred in 6 men and 5 women. Osteochondritis dissecans in the hip appeared in 5 men and one woman. The first symptoms were pain and restriction of joint movement. Conservative treatm…

[Immediate surgical treatment of supracondylar fractures of the elbow in childhood].

The results of surgical treatment, in children, of 10 supracondylar factures of the humerus, complicated by vascular and neurological disturbances were so satisfactory that open reduction is proposed for all those cases with severe lesions and oedema requiring a prolonged period of continuous trans-skeletal traction even without neurological and vascular problems.

Report on compound dislocation of the elbow with entrapment of the brachial artery.

Entrapment of the brachial artery within a compound dislocated elbow joint seems not to have been reported previously. Brachial artery rupture is the usual vascular injury associated with a compound elbow dislocation. A review of the surgical literature suggests that wide spectrum of disability may follow interruption of brachial artery flow at the elbow. While most patients experience no immediate or late ischemic symptoms because of the rich collateral network about the elbow, intermittent claudication or gangrene of the hand is possible and has been well-documented in the literature. To ass…
