Prevalence, Etiology, Risk Factors, and Complications of Facial Nerve Palsy at King Abdulaziz Medical City: A Multicenter Study.

BACKGROUND: Facial nerve palsy is a condition of nerve damage that results in impaired facial movement on one or both sides of the face. OBJECTIVES: This multicenter study aimed to determine the prevalence of facial nerve palsy and evaluate the association between its risk factors and complications to identify its etiology among patients admitted to King Abdulaziz Medical City in Jeddah and Riyadh, Saudi Arabia, between 2016 and 2023. METHODS: A retrospective cross-sectional study was conducted to obtain data from medical records using the best care system for patients with facial nerve pals

Mitochondrial Cyclophilin D as a Potential Therapeutic Target for Ischemia-Induced Facial Palsy in Rats.

Many studies have demonstrated that ischemia could induce facial nerve (FN) injury. However, there is a lack of a suitable animal model for FN injury study and thus little knowledge is available about the precise mechanism for FN injury. The aims of this study were to establish a reliable FN injury model induced by blocking the petrosal artery and to investigate whether dysfunctional interaction between cyclophilin D (CypD) and mitochondrial permeability transition pore (MPTP) can mediate cell dysfunction in ischemic FN injury. The outcomes of ischemia-induced FN injury rat model were evaluate

Intraoperative monitoring of the facial nerve.

Anatomic preservation of the facial nerve, with maximal facial function, is one of the goals of acoustic neuroma surgery. Application of electrophysiologic monitoring techniques is useful in achieving this goal. Preoperative electromyography and nerve conduction studies provide important prognostic information for preservation of the nerve and postoperative function. Intraoperative electromyography alerts the surgeon to facial nerve proximity and potential injury. Direct nerve stimulation is utilized to confirm the location and integrity of the nerve. Matched-pair analysis of two groups of pat

Animal bite injuries to the face : A Case Report.

Traumatic lacerations to the skin are problems frequently seen and treated by emergency centers around the world. Among all wounds, dog and cat bites are commonly seen. As in many mammals, different species of microorganisms are found in dog and cat mouths with a potential pathological effect to humans, as represented by rabies. The injuries have disfiguration effect with possible psychological repercussion to the patient. This article aimed presenting up to date considerations regarding the management of animal bite injuries to the face, exemplified by a case report that should be the interes

Gunshot wounds of the temporal bone: a rational approach to evaluation and management.

Gunshot injuries of the temporal bone are uncommon but are being seen more frequently with the increasing incidence of violent crimes and use of firearms. A retrospective analysis of 43 consecutive patients treated at our institution with this injury was undertaken to determine the clinical symptoms, associated injuries, and treatment outcome. This is the largest series from a single institution treated by a single group during a brief period of time. Most of these patients had injury to various structures within and around the temporal bone, including hearing loss (29), intracranial injury (2

Caring for the Patient with Otolaryngologic Trauma Is a Complex Matter.

In an Austrian study of 14,654 patients with 35,129 injuries treated at a single Department of Cranio-Maxillofacial and Oral Surgery over a 15-year period, the following was seen.1 Older people were more prone to soft tissue lesions with a rising risk of 2.1% per year older, equal in men and women. Younger patients were at higher risk of dentoalveolar trauma with an increase of 4.4% per year younger, and this was 19.6% higher for women. The risk of sustaining facial bone fractures increased each year by 4.6% and was 66.4% times higher in men. A total of 2550 patients (17.4%) suffered 3834 conc

Oral motor dysfunction after carotid endarterectomy: Challenges in diagnosing marginal mandibular nerve injury and effectiveness of rehabilitation.

Carotid endarterectomy (CEA) is a frequently used surgical treatment for carotid artery stenosis. Cranial and peripheral nerve injuries are among the intraoperative complications. Diagnosing isolated injuries of the marginal mandibular branch of the facial nerve after CEA is challenging and leads to oral motor dysfunction that affects the patient's quality of life. Early diagnosis and treatment are valuable, since delayed diagnosis requires a surgical restoration of the affected nerve or muscle. In this article, we present a case of marginal mandibular nerve injury after CEA to increase the aw

Posttraumatic Nasal Deformities: Correcting the Crooked and Saddle Nose.

The nose is frequently traumatized in facial injuries and this often results from motor vehicle accidents, sports-related injuries, and altercations. Subsequently, posttraumatic nasal deformity is one of the most common reasons that patients seek consultation in the doctor's office. Depending on the type of nasal deformities, this can result in functional impairment and aesthetic problems. Two challenging problems to be addressed in the posttraumatic nose include the crooked nose deformity and the saddle nose deformity. The numerous publications on these two topics attest to the exacting surgi…

Immediate socket restoration after orbital trauma with globe loss: principles, timing, and our experience.

The literature-reported incidence of ophthalmic injuries occurring with facial fracture ranges widely from 0.8% to 30%. Ocular trauma necessitating enucleation or evisceration is less common, but it is not rare. The trauma and physical disability related to removal of the eye are extreme. Moreover, the loss of an eye causes severe changes to the anatomy and physiology of the orbit, resulting in deformities that affect the relationship between the socket and the prosthesis. Here, the authors present their own experience of 8 consecutive cases of trauma injuries with globe loss and emphasize the

[Surgical tactics in severe combined injuries of the face and other body regions].

The results of treatment in 67 patients with associated injuries of the face and other regions of the body are analysed. Sixteen of them died. Their poor state was conditioned not by facial injuries but by traumas of other organs and system of the organism. It is concluded that such patients should be treated not in stomatological but in special traumatological departments for polytraumas. Surgery is first to be performed in the region where the damage is most hazardous. But the first step is to prevent aspiration of blood from the oral cavity into the respiratory tract followed by the fixatio…

Topography of the frontal branch of the facial nerve and its clinical implication for temporal direct browplasty.

Due to anatomic proximity to the surgical site, iatrogenic trauma to the frontal branch of the facial nerve (FbFN) with resultant brow paralysis is a recognized major complication of temporal direct browplasty. This study was aimed to elucidate the course of the FbFN in the area superolateral to the brow in order to facilitate safer temporal direct browplasty by preventing facial nerve injury. Forty-five hemifaces from 32 embalmed Korean cadavers were dissected. A horizontal line connecting the tragion to lateral canthus was established. Then, an oblique line passing through the lateral canthu

[Repair of a facial nerve substance loss by interposition of a collagen neurotube].

We are exposing the case of a 22 year-old patient presenting a wound of the right cheek, with a palsy of the right corner of the mouth. He has been sent to us 6 days after the trauma for secondary exploration. A section of the buccal branch of the right facial nerve with a 1cm gap has been brought out. We have bypassed the loss of substance with a collagen absorbable biological conduit. The 6-months clinical and electromyographic follow-up has shown a clear improvement of the function of the orbicularis oris, as well as its reinnervation by the buccal branch of the right facial nerve.

[Regeneration of the severed facial nerve].

Twenty-four specimens from distal end of the human facial nerve obtained at reinnervation surgery (nerve grafting) were studied by light and electron microscopies. In 21 specimens, with complete severance of the facial nerve but continuous neurilemma confirmed during surgery, regeneration phenomenon was observed. Histological examination of the auricular branches of vagus nerve (2 cases) and auriculotemporal nerve (1 case) with their communication portion showed regeneration of axons into the facial nerve below the level of sectioning. This phenomenon points to the possibility that surgically

[Traumatic laceration of facial nerve and Stenone duct. A clinical case].

After a brief review of the main operating techniques employed in the reconstruction of the facial nerve and Stenone's duct lacerated by trauma, a clinical case in which the two lesions are associated is reported. Suture of the lacerated nervous branches and attachment of the salivary fistula to the oral mucosa using a silicon drainage applied for a few days permitted functional recovery of neuronal activity and salivation.

Craniofacial trauma.

The treatment of combined major dislocations of the face and skull requires a craniofacial team consisting of a plastic maxillofacial surgeon, a neurosurgeon, an anesthesiologist, and radiologists. The best results can only be achieved at the primary intervention both from a functional and an esthetic standpoint. Careful preoperative clinical and radiologic examinations are mandatory to reach these high goals. In primary repair, the cranial approach aims at constructing "a monobloc bone flap" to suture meningeal tears and to reinforce them with a pericranial graft, to close the cranial base co
