OBJECTIVES: The primary objective of this subanalysis was to examine the safety, tolerability, and efficacy of terbinafine in the treatment of toenail onychomycosis in the patients aged > or = 65 years in the Improving Results in Onychomycosis Concomitant Lamisil And Debridement (IRON-CLAD) trial. A 73-years old patient came to our outpatient clinic because of a blue discoloration of his hands. Furthermore there was a systemic inflammatory reaction, burning pain and numbness of his feet due to a polyneuropathy, and an indolent enlargement of the cervical lymph nodes. In 1996 and 2001 cervical lymph node resections were done because of localized angiofollicular lymphnode hyperplasia (Castleman's disease). The laboratory values confirmed a systemic inflammatory reaction, a hypothyreosis and a monoclonal gammopathy. A CT-scan showed enlarged cervical, intrathoracic and abdominal lymphnodes

Over a 10-year period, 230 patients with foot dermatitis were patch tested to a shoe series of allergens. About 54 (23%) had relevant allergic positive reactions to one or more allergens. Tunga penetrans is a burrowing flea that is prevalent in Central and South America, the Caribbean, tropical Africa, India, and Pakistan, and rarely is imported to the United States. Tungiasis results from the cutaneous infestation of humans by the gravid female flea, T. penetrans. We report a case of tungiasis in a female who had been on a missionary expedition along the Brazilian Amazon River. The clinical presentation, life cycle, differential diagnosis, military and historical aspects, and treatment are discussed.

A 71 year old man with newly diagnosed acute myelomonocytic leukaemia developed a soft tissue infection of his foot during his first course of chemotherapy. Scedosporium apiospermum was isolated from the lesion, which resolved rapidly on treatment with intravenous amphotericin B despite being resistant in vitro to this agent. This observation suggests that sensitivity testing of S apiospermum should be interpreted with caution and that clinical response may be a better indicator of outcome.

One hundred patients, workers from a meat packing plant that wear rubber boots while working, were studied. Besides the well known allergic manifestations to rubber or other plastic materials used in footwear other pathologic findings included mycotic and bacterial lesions formed by plantar hyperhidrosis with the development of bromhidrosis and plantar xanthokeratoderma. These occupational dermatoses are a medical problem, but also have social and economical implications.

During some hot summer-days in 1973 about 70 cases of acute feet dermatitis were reported among soldiers from one regiment. They had used newly laundered socks from the same batch. The socks were disinfected with a solution containing TBTO and a quarternary ammonium compound, but by mistake a roughly 7 times too high concentration of the solution had been used.
