Evaluation of fixation after plating of distal radius fractures - a validation study.

We used five fresh-frozen cadavers with 10 upper limbs to evaluate by finite element analysis (FEA) the plate fixation of distal radius fractures. The distal radius of the cadavers was fractured using a comminution fracture model. Plate fixation was performed using Synthes VATCP. Compression tests were performed on these specimens and force displacement curves were obtained. FEA was performed using Mechanical Finder. The Keyak, Keller vertebra, Carter, and Matsuyama conversion equations without contact analysis, and the Matsuyama equations with contact analysis, were used for the boundary cond

Scapho-capitate fracture syndrome. A case report.

A rare injury of the wrist, scapho-capitate fracture syndrome, in a young patient is reported. Despite early recognition of the injury and surgical intervention, the scaphoid fracture did not unite and another attempt to achieve union with bone grafting and internal fixation also failed. The wrist continued to be painful and stiff. Radiographs of the wrist, 18 months after the injury, showed nonunion of the scaphoid, avascular necrosis of the scaphoid and the lunate and carpal collapse with midcarpal joint arthritis. Due to persistent and disabling symptoms arthrodesis of the wrist had to be c

The radiological deformity of Colles' fractures.

The behaviour of the bony deformity in Colles' fractures and factors influencing this were prospectively investigated in 187 patients over the age of 55. Radiographic assessment was made throughout a 13-week period and during this time the deformity progressively recurred, even after the plaster cast had been removed. Radial length and radial deviation reverted virtually to their positions before reduction. Only those fractures with a volar tilt greater than -15 degrees when first seen showed any improvement. In all fractures the initial deformity clearly influenced the final radiological resu

Comparison of patient specific implant reconstruction vs conventional titanium mesh reconstruction of orbital fractures using a novel method.

To compare the reconstruction of orbital fractures using patient-specific implants (PSI) and conventional pre-formed titanium mesh; to develop a method of three-dimensional (3D) superimposition and analysis of the reconstructed orbits; and to present the pitfalls in 3D planning of orbital PSI and how to avoid them. This was a retrospective study of patients with orbital fractures who were treated in our institution between the years 2022 and 2023 using PSI or conservative prefabricated titanium mesh. Three different methods for virtual reconstruction of orbital fractures were used and are deta

Suture Anchor Versus Transosseous Tunnel Repair for Inferior Pole Patellar Fractures Treated With Partial Patellectomy and Tendon Advancement: A Biomechanical Study.

BACKGROUND: Comminuted inferior pole patellar fractures can be treated in numerous ways. To date, there have been no studies comparing the biomechanical properties of transosseous tunnels versus suture anchor fixation for partial patellectomy and tendon advancement of inferior pole patellar fractures. HYPOTHESIS: Suture anchor repair will result in less gapping at the repair site. We also hypothesize no difference in load to failure between the groups. STUDY DESIGN: Controlled laboratory study. METHODS: Ten cadaveric knee extensor mechanisms (5 matched pairs; patella and patellar tendon) we

Factors affecting functional outcome of displaced intra-articular distal radius fractures.

Open reduction and internal fixation is often required in comminuted, displaced intra-articular fractures of the distal radius when closed manipulation has failed to restore articular congruity. Results of surgical stabilization and articular reconstruction of these injuries are reviewed in this retrospective study of 49 patients with 52 displaced, intra-articular distal radius fractures. Forty-three patients with a mean age of 37 years (range, 17-79 years) were available for evaluation. The mean follow-up time was 38 months (range, 22-69 months). When rated by the system proposed by the Assoc

Treatment of radial head and neck fractures: in favor of anatomical reconstruction.

Radial head fractures represent the most common elbow fractures. Undisplaced fractures usually occur in isolation and can be treated nonsurgically. Displaced fractures should be treated surgically. Simple two-part fractures can easily be handled by osteosynthesis, but comminuted fractures pose a major problem for reconstruction. As the radial head is an important stabilizer of the elbow joint-especially in the context of concomitant ligamentous injuries-its resection may lead to pain, limited range of motion, and instability. Therefore, radial head resection is not recommended for the acute si

Postoperative Open Reduction Internal Fixation Discomfort and Rehabilitation Response: A Case Report.

Rehabilitation details after open, comminuted patellar fractures in older adults are seldom published; this case maps a full 20-week recovery using a structured, criteria-based program. A healthy 56-year-old woman was unable to bear weight after a fall, with anterior knee wounds and imaging that showed an open, comminuted left-patella fracture. Six weeks postoperatively, she reported resting pain of 4/5 on the Verbal Rating Scale (VRS) and demonstrated only 40° active range of motion (AROM). The injury was classified as a Type I/II open patellar fracture. It was stabilized with open reduction

The thoracolumbar crush fracture. An experimental study on instant axial dynamic loading: the resulting fracture type and its stability.

Seven vertebral preparations of L1, with surrounding discs, facet joints, and ligaments were exposed to an instant axial dynamic force in order to produce a burst or crush fracture. The resulting fractures were similar to fractures observed clinically and showed a comminuted vertebral body with fractured vertebral end-plates, dislocated disc nucleus, bone fragments severely encroaching upon the spinal canal, and facet joint laxity. The flexion-extension range was increased considerably. This implies that this fracture type should be regarded as unstable with a risk of progressive flexion defor

Multiple pathological fractures and delayed union associated with lead exposure in a German Shepherd Dog.

CASE REPORT: An 8-year-old 40.8-kg intact male German Shepherd Dog was evaluated for bilateral antebrachial fractures. Radiographs revealed osteopenia and comminuted proximal diaphyseal fractures of the left radius and ulna, and proximal articular fracture of the right ulna. A dual energy X-ray absorptiometry scan confirmed decreased bone mineral density. Bone mineral analysis collected at the time of definitive surgical repair demonstrated high lead concentration. Analysis further demonstrated normal bone calcium and phosphorus concentrations. Serum lead concentration was normal. The left rad

Fractures of the pediatric zygoma: a review of the clinical trends, management strategies, and outcomes associated with zygomatic fractures in children.

Fractures of the pediatric zygoma are uncommon and are often associated with high-impact trauma, as evidenced by the relatively increased prevalence of concomitant injuries observed in these patients. Despite advances in the prevention, diagnosis, and management of pediatric craniofacial injuries, data regarding zygomatic fractures in children remain poorly established. The diagnosis of zygomatic disruption is more difficult in children and requires the maintenance of a high index of suspicion on behalf of the surgeon. Early recognition and implementation of appropriate therapy are critical an

Indirect reduction and composite fixation of extraarticular proximal tibial fractures.

Forty-one extraarticular comminuted proximal tibial fractures were treated during a 7-year period. The fractures were proximal tibial metaphyseal injuries or metaphyseal-diaphyseal junction injuries with extension proximally and distally but not involving the knee joint. All fractures were treated surgically with open reduction and internal fixation using an indirect reduction technique with a lateral plate, and a medial substitution external fixator concomitantly. All fractures were seen for followup until they healed (average healing time, 12.1 weeks). The timing of internal fixation was bas

Osteoregenerative efficacy of a novel synthetic, resorbable Ca/P/S-based bone graft substitute in intra- and peri-articular fractures: a brief medical image-based report.

BACKGROUND: When a fracture goes into or around a joint, it usually damages the cartilage at the ends of bones and other joint tissue. As a result, the affected joints are prone to traumatic arthritis, leading to stiffness. Repairing bone damage, maintaining joint integrity, and avoiding subchondral and metaphyseal defects caused by comminuted fractures is often a great challenge for orthopedic surgeons. Tissue engineering of synthetic bone substitutes has proven beneficial to the attachment and proliferation of bone cells, promoting the formation of mature tissues with sufficient mechanical s

Dorsal transosseous reduction and locking plate fixation for articular depressed middle phalangeal base fracture.

PURPOSE: An articular depressed fragment at the base of the middle phalanx can be an obstacle to congruent reduction and stable fixation. This study assessed the outcomes of a transosseous reduction technique combined with locking plate fixation for the treatment of articular depressed middle phalangeal base fracture. METHODS: Between 2015 and 2017, seven patients (eight fingers) with intraarticular comminuted middle phalangeal base fracture were included in this study. Mean follow-up was 19 months (range 12-30 months). All patients showed depression of the articular fragment on sagittal comp

"Mother and baby plate": a strategy to improve stability in proximal fractures of the ulna.

INTRODUCTION: Proximal ulna fractures with a large zone of comminution, such as in the context of Monteggia injuries, require mechanically strong osteosyntheses as they occur in regions with high physiological joint load. Consequently, implant failure and pseudarthrosis are critical and devastating complications, especially with the background of mainly young patients being affected. An effective solution could be provided by adding a small second plate 90° angulated to the standard dorsal plate in the area of non-union. Thus, this study investigates whether, from a biomechanical point of view

Factors Affecting the Range of Motion of the Elbow after Open Reduction of Olecranon Fractures.

Background: Although postoperative outcomes of olecranon fractures are relatively favorable, postoperative stiffness of the elbow joint occurs occasionally. We aimed to identify negative prognostic factors for unfavorable outcomes following olecranon fractures. Methods: We retrospectively reviewed the records of 100 patients with olecranon fractures and collected data on the conducted procedures, age, fracture condition, postoperative reduced position, and postoperative active range of motion (AROM) of the elbow joint. The primary outcome was the regaining of an AROM of > 130° flexion and < -3

Operative treatment of distal femoral fractures above total knee arthroplasty with the indirect reduction technique: a long-term follow-up study.

The complication rate of conventional plate osteosynthesis (CPO) of periprosthetic femoral fractures above total knee arthroplasties (TKA) is high. Indirect reduction techniques were introduced to reduce surgical dissection at the fracture site. Twenty-one patients (4 men and 17 women) with femoral fractures above well-fixed total knee arthroplasties were consecutively treated with the indirect reduction technique. AO/ASIF (Arbeitsgemeinschaft für Osteosynthesefragen/Association for the Study of the Problems of Internal Fixation) Type 33A fractures were included. The mean age was 78 years (ran
