A non-healing ulcerated fingertip following injury.

A man went to his primary care physician 3 months after slamming his right thumb in a car door. The nail had turned black and sloughed off several weeks later, leaving a red, draining wound on the tip of his thumb. The wound drained continuously for the next 2 months and showed little progress in healing. His physician started him on antibiotics, but the wound still showed no progress in healing over the next 6 weeks. Cultures were obtained that grew out Staphylococcus and Streptococcus spp. Another course of antibiotics was given, but the patient's condition failed to improve. At this point t

D2-40 immunohistochemical analysis of pediatric vascular tumors reveals positivity in kaposiform hemangioendothelioma.

Kaposiform hemangioendothelioma is a distinctive vascular neoplasm affecting predominantly children and neonates. In neonates it needs to be differentiated from common infantile hemangioma and other vascular lesions of infancy. Kaposiform hemangioendothelioma immunoreacts with vascular endothelial growth factor receptor 3, and partial lymphothelial differentiation of this lesion has been suggested. D2-40 has been recently proposed as a selective marker of lymphatic endothelium. We performed immunohistochemical analysis with the D2-40 antibody on 24 kaposiform hemangioendotheliomas and 48 other

Primary placement of a motility coupling post in porous polyethylene orbital implants.

The placement of a motility coupling post (MCP) to integrate the prosthesis with a porous orbital implant may enhance prosthetic motility following enucleation. Previously, MCP placement has required a second operation usually at least 6 months following enucleation. We developed a technique to place an MCP reliably and safely into a porous orbital implant at the time of enucleation. Eligibility criteria included high motivation to achieve maximal prosthetic motility, adequate conjunctiva to ensure desirable wound closure, and isolation of the 4 rectus muscles. Enucleation was performed in sta

Single tarsoconjunctival flap (lower eyelid) for upper eyelid reconstruction ("reverse" modified Hughes procedure).

We present a two-stage reconstruction of partial or total full-thickness upper eyelid defects. In the first stage, a single tarsoconjunctival flap from the donor lower eyelid reconstitutes the posterior lamella, and a full-thickness skin graft reconstructs the anterior lamella. In the second stage, 5 to 8 weeks later, the skin tarsoconjunctival flap is severed. The single tarsoconjunctival flap we describe is analogous to the modified Hughes reconstruction for full-thickness lower eyelid defects and thus may be termed a "reverse" modified Hughes procedure. In the lid-sharing Cutler-Beard proce

Cutaneous neuroblastoma. Peripheral neuroblastoma.

UNLABELLED: Cutaneous neuroblastoma (CN) is a rare tumor in adults. Histologically CN can be confused with other small, round cell tumors, especially with Merkel cell tumor. This is the second case that we have encountered in a 6-year period. A 46-year-old man with severe adult respiratory distress syndrome had a 2.5 x 2.0 x 2.0-cm, rapidly growing, fleshy nodule over his nose. The clinical impression was pyogenic granuloma (PG) or keratoacanthoma (KA). Histologically, the tumor showed undifferentiated cells with frequent Homer Wright rosettes and mitoses. Conventional special stains and a bro

Immunohistochemistry for FOSB and FOS is a Useful Ancillary Tool in the Diagnosis of Epithelioid Hemangioma but There are Pitfalls in Interpretation Including Expression in Other Vascular Lesions.

Introduction. Epithelioid hemangioma is a benign vascular neoplasm associated with FOS and/or FOSB protein overexpression detected by immunohistochemistry (IHC). Methods. The aim of our study was to determine the co-expression or independent IHC expression of FOS and FOSB in a cohort of epithelioid hemangiomas. We also included two cohorts of other vascular lesions: papillary endothelial hyperplasia and lobular capillary hemangioma / pyogenic granuloma. Results. We identified 50 cases of epithelioid hemangioma, 84% of which were cutaneous and the remaining involved other anatomic locations. Ov

Nail abnormalities associated with systemic pathologies.

Nail abnormalities can arise in conjunction with or as a result of systematic pathologies. These pathologies include single-organ diseases, multisystemic diseases, and drug-induced insults. Clinical signs associated with these conditions include dyschromias, vascular alterations, periungual tissue changes, textural dystrophies, contour alterations, and growth-rate alterations. The associated systemic pathologies may affect any part of the nail apparatus, including the nail matrix, the nail plate, the nail bed, the underlying vasculature, and the periungual tissues. The anatomical location and

Severe bleeding from a pregnancy tumor. A case report.

BACKGROUND: Hyperplastic gingivitis and gingival hyperplasia accompanying gestation have been termed "pregnancy gingivitis" and "pregnancy tumor." The condition is benign but, rarely, is complicated by severe bleeding that is difficult to manage. A single case of a pregnancy tumor is reported. CASE: A 28-year-old woman in the third trimester was evaluated for treatment of a pregnancy granuloma with recurrent episodes of severe bleeding for two weeks. Conservative management by firm pressure on gauze packs was applied to control the bleeding, but in vain. Induction of labor was conducted at 37

Polypoid Melanoma: A Rare and Aggressive Variant.

A 57-year-old man presented with a several-month history of a bleeding, lobulated mass on the left lower back and a palpable left axillary lymph node. Excisional biopsy revealed an 11-mm-thick, lobulated, sessile polypoid nodular melanoma (pT4b) with Clark level IV invasion, a mitotic rate of 4/mm², and associated ulceration. A sentinel lymph node biopsy was not performed, as the patient was subsequently lost to follow-up. Immunohistochemistry was positive for SOX10, MART-1, and HMB-45, confirming melanocytic differentiation. The lesion was initially excised with 2-cm clinical margins, and his

Kaposiform hemangioendothelioma in the mandibular gingiva: a rare case report with literature review.

Kaposiform hemangioendothelioma (KHE) is a locally aggressive vascular tumor in neonates and children. KHE can occur anywhere in the body, but occurrence in the oral cavity is exceptionally rare. The clinical features of KHE are similar to those of oral lesions, such as infantine hemangioma, pyogenic granuloma, and chronic periodontitis. However, unlike other oral lesions, KHE invades neighboring tissue planes and recurs frequently as malignancy. Moreover, some instances of KHE are accompanied by a life-threatening condition named Kasabach-Merritt phenomenon. Correct diagnosis of KHE is very d

Acquired Digital Fibrokeratoma: A Report of Five Cases and Review of the Literature.

Acquired digital fibrokeratoma (ADF) is rare in clinical practice and is easily misdiagnosed. Herein, 5 cases of patients (3 males and 2 females) with ADF are reported. The mean age at onset was 42.6 years, and the mean disease duration was 3 years. Four patients had ADF on the hands, and 1 patient had ADF on the foot. The clinical manifestations were all solitary, skin-colored papules, with a firm texture and smooth surface, protruding from the skin surface. In 3 patients, ADF manifested as columnar protrusions, and in 2 patients, ADF manifested as dome-shaped protrusions. For all 5 patients,

To Determine the Histomorphological Variants of Kaposi's Sarcoma in HIV-infected Patients in the University of Benin Teaching Hospital.

INTRODUCTION: There are different histopathological variants of Kaposi's sarcoma (KS) aside the typical patch, plaque, and nodular variants. The baseline data of the unfamiliar variants of this lesion have not been determined in our environment. Their determination will bring to the foreclose differential diagnoses that may lead to diagnostic pitfalls. AIM AND OBJECTIVES: To determine the histomorphological variants of KS in the University of Benin Teaching Hospital over a 3-year 2-month period from May 1, 2014, to July 31, 2017. MATERIALS AND METHODS: This was a prospective study. The patie

Cutaneous VCL::ALK fusion ovoid-spindle cell neoplasm.

Cutaneous VCL::ALK fusion spindle (ovoid) cell tumor is unique. Recently emerged RAS::MAP tyrosine kinase fusion sarcomas more commonly involve subcutis, skeletal muscle and even bone. We share our experience with a novel cutaneous VCL::ALK spindle cell tumor. An 11-year-old male presented with a back pedunculated pink-red papule thought to be a pyogenic granuloma. Biopsy histopathology revealed an epithelial collarette with pedunculated tumor extending to deep dermis/subcutis interface. The combination of spindled and epithelioid cells, an ovoid myopericytoid appearance within myxoid to colla

Pathergy of a Medial Heel Schwannoma.

Schwannomas are benign soft-tissue tumors derived from Schwann cells of the peripheral nervous system. They typically appear on the head, neck, or trunk, and are often asymptomatic or present with mild tenderness to palpation and numbness due to nerve compression. A 17-year-old male was referred to our dermatology clinic for evaluation and treatment of an asymptomatic, pink, flesh-colored subcutaneous nodule on the medial right heel. A biopsy was performed to rule out malignancy, with the pathology report consistent with the diagnosis of schwannoma. Following the biopsy, the patient developed

A case series of medication-related fibrovascular hyperplasia following hematopoietic stem cell transplantation for Fanconi anemia.

Systemic medications categorized as diphenylhydantoin, calcineurin inhibitor and calcium channel blocker may have effects on the oral cavity by modifying the inflammatory and immune response and causing undesired tissue proliferative reactions. Calcineurin inhibitors are medications commonly used for long periods in patients undergoing allogeneic hematopoietic stem cell transplant (HSCT) and solid organ transplantation. Medication-related fibrovascular hyperplasia (MRFH) is an extra gingival hyperplastic nodular growth associated with medications use. This study reports five cases of pediatric

Pseudomyogenic Hemangioendothelioma: A Vascular Tumor Previously Undescribed in the Oral Cavity.

The pseudomyogenic hemangioendothelioma (PMH) is a low-grade malignant vascular neoplasm of different tissue planes including skin and soft tissue. Primary tumors in the skeletal muscle and bone have also been diagnosed. The PMH was introduced into the WHO classification of tumors of soft tissue and bone in 2013. This is the first description of oral involvement. A 21-year-old female presented with a 2-month old swelling of her gingiva. The swelling appeared red in color and was soft in consistency. A clinical diagnosis of a pyogenic granuloma was made and an incisional biopsy was submitted fo

Benefits of an ungual prosthesis in traumatology and reconstructive surgery of the nail.

Lesions of the nail apparatus can leave esthetic and sometimes functional sequelae. Many sequelae are the result of incorrect growth of the nail plate. During the acute phase, in the absence of a nail plate, the nail bed may keratinize or form a pyogenic granuloma, compromising harmonious regrowth of the nail plate. The ideal is to reinsert the plate if it is available, but if it has been lost, an ungual prosthesis must be inserted to act as a temporary implantable splint. This implant must have the same shape and structure as the nail plate. Many ideas have been used in the past, including su

Caudal mucogingival lesions secondary to traumatic dental occlusion in 27 cats: macroscopic and microscopic description, treatment and follow-up.

The main aim of this retrospective study was to describe clinical and histopathological findings in cats with mucogingival lesions developed at the contact point of the premolar and molar teeth of the opposite quadrant. Cases were retrieved following manual review of the medical records, dental records and photographic documentation of all feline dental patients visited in the period between February 2001 and August 2011. Cats showing different lesions at different times were calculated as multiple cases. A total of 27 cats (31 cases) with 44 lesions (26 proliferations [59%], 11 clefts [25%] a
