Predicting and enhancing American Board of Surgery In-Training Examination performance: does writing questions really help?

BACKGROUND: The generative learning model posits that individuals remember content they have generated better than materials created by others. The goals of this study were to evaluate question generation as a study method for the American Board of Surgery In-Training Examination (ABSITE) and determine whether practice test scores and other data predict ABSITE performance. METHODS: Residents (n = 206) from 6 general surgery programs were randomly assigned to one of the two study conditions. One group wrote questions for practice examinations. All residents took 2 practice examinations. RESUL

Non-randomized comparative study on the efficacy of a trauma protocol in the emergency department.

PURPOSE: Trauma is a major health problem in developing countries and worldwide which requires many resources and much time in an emergency department (ED). Although Advanced Trauma Life Support (ATLS) is the gold standard, operator dependence can affect the quality of care. The objective was to identify differences in numbers and time to lifesaving interventions, investigation, ED length of stay, and mortality between an in-house protocol and conventional practice. METHODS: This was a single-center prospective non-randomized study for adult trauma patients in the ED. The trauma protocol was

Adhering to Radiographic Clinical Practice Guidelines for Distal Radial Fracture Management Is Associated with Improved Outcomes and Lower Costs.

BACKGROUND: The American Academy of Orthopaedic Surgeons Clinical Practice Guideline on the Treatment of Distal Radius Fractures has not been evaluated in clinical practice. We hypothesized that adhering to the distal radial fracture radiographic clinical practice guideline (CPG) improves outcomes and reduces costs. METHODS: We reviewed 266 patients with distal radial fractures treated at 1 institution. Based on CPG radiographic parameters (Recommendation 3), care was rated as "appropriate" or "inappropriate." Stress management at the worksite for hypertension: compliance, cost-benefit, health care and hypertension-related variables.

The blood pressure-lowering effects of a group stress management program conducted with hypertensive employees at the worksite were assessed and replicated. Both systolic (SBP) and diastolic (DBP) blood pressure were significantly reduced from a baseline period to the end of a 10-week stress management training period (Group T1, N = 22). A control Group, T2 (N = 18), showed no significant SBP and DBP reduction over a comparable time period. When Group T2 was then given stress management training, the resulting SBP and DBP reductions were significant. When BP was recorded 3 years later, there w

A letter intervention to GPs practices to promoting prescription uptake in school-age children with asthma during summer holidays (TRAINS study): a pragmatic cluster randomised controlled trial.

In school-aged children, asthma exacerbation rates peak following the return to school after the summer break. A cluster randomised controlled trial (PLEASANT) found that sending a reminder letter from a family doctor to parents of children with asthma during summer holiday led to a 30% increase in prescription collection in August and a decrease in unscheduled care visits after school return in the period September to December. This intervention also resulted in an estimated cost saving of £36.07 per patient per year. We aimed to assess whether informing general practitioner (GP) practices ab

Awareness and agreement with neurofibromatosis care guidelines among U.S. neurofibromatosis specialists.

INTRODUCTION: The neurofibromatoses (NF) are a group of rare, genetic diseases sharing a predisposition to develop multiple benign nervous system tumors. Given the wide range of NF symptoms and medical specialties involved in NF care, we sought to evaluate the level of awareness of, and agreement with, published NF clinical guidelines among NF specialists in the United States. METHODS: An anonymous, cross-sectional, online survey was distributed to U.S.-based NF clinicians. Respondents self-reported demographics, practice characteristics, awareness of seven NF guideline publications, and leve

The effects of five sessions of continuous theta burst stimulation over contralesional sensorimotor cortex paired with paretic skilled motor practice in people with chronic stroke.

BACKGROUND: In individuals with chronic stroke, impairment of the paretic arm may be exacerbated by increased contralesional transcallosal inhibition (TCI). Continuous theta burst stimulation (cTBS) can decrease primary motor cortex (M1) excitability and TCI. However, contralesional cTBS shows inconsistent effects after stroke. Variable effects of cTBS could stem from failure to pair stimulation with skilled motor practice or a focus of applying cTBS over M1. OBJECTIVE: Here, we investigated the effects of pairing cTBS with skilled practice on motor learning and arm function. We considered th

Improving consolidation by applying anodal transcranial direct current stimulation at primary motor cortex during repetitive practice.

Engagement of primary motor cortex (M1) is important for successful consolidation of motor skills. Recruitment of M1 has been reported to be more extensive during interleaved compared to repetitive practice and this differential recruitment has been proposed to contribute to the long-term retention benefit associated with interleaved practice. The present study administered anodal direct current stimulation (tDCS) during repetitive practice in an attempt to increase M1 activity throughout repetitive practice with the goal to improve the retention performance of individuals exposed to this trai

Respiratory-Swallow Training and Variable Practice in Parkinson's Disease: A Clinical Trial Pilot Study.

INTRODUCTION: Dysphagia in people with Parkinson's disease (PwPD) is closely linked to impairments in respiratory-swallow coordination. Respiratory-swallow training (RST) is a promising, mechanism-based intervention for dysphagia, but, to date, only a single-subject study has examined RST in PwPD. As an early-stage next step in refining RST for PwPD, this clinical trial pilot study aimed to: (1) explore the preliminary associations between exhale-swallow-exhale (EX-EX) RST and swallowing safety, swallowing efficiency, and respiratory phase patterning; and (2) explore differences in practice st

Evaluating the effect of digital game-based nutrition education on anemia indicators in adolescent girls: A randomized clinical trial.

Iron deficiency anemia is the most common type of micronutrient deficiency in the world. Adolescence represents a period of increased risk of iron deficiency. Therefore, we aimed to determine the impact of nutrition education by a digital game on markers of iron-deficient anemia in adolescent girls. In this study, 176 adolescent girls were randomly dichotomized into the intervention and control groups. At the beginning and the end of the intervention, knowledge, attitude, and practice of both groups were assessed by a questionnaire. Girls in the intervention group received the necessary educat
