[Cryptococcal whitlow in a HIV positive patient].

INTRODUCTION: Cutaneous cryptococcosis is a systemic fungal disease; it is commonly observed in immunocompromised patients. OBSERVATION: We report the case of a cryptococcal whitlow in an HIV positive patient. The mycologic culture of the cutaneous lesion was positive for Cryptococcus neoformans serotype D. The detection of the blood antigen was positive but there was no pulmonary nor central nervous system involvement. The lesions cured with fluconazole (400 mg/day during 2 months and 200 mg/day after). DISCUSSION: This unusual clinical presentation of cutaneous cryptococcosis has never bee

The cutaneous signs of eating disorders.

Anorexia nervosa and bulimia nervosa are common yet occult eating disorders which each affect between 1 and 2% of young adult women in the UK (1). Cutaneous signs may provide valuable clues allowing earlier diagnosis. Eating disorders may both imitate (Table I) and complicate other medical illnesses, resulting in unnecessary and expensive investigations. Organic diseases that may be complicated by eating disorders are Crohn's disease (2), diabetes mellitus (3, 4) and human immune deficiency virus disease (5). The concurrence of an additional eating disorder will destabilize the primary medical

A double blind study of itraconazole vs griseofulvin in patients with tinea pedis and tinea manus.

AIM: To compare the efficacy and safety of itraconazole and griseofulvin in a double-blind parallel study of 20 patients with microscopically and/or culturally proven tinea pedis and/or tinea manus. METHODS: A 4 week study was undertaken with patients randomised to receive either itraconazole 100 mg or griseofulvin 500 mg once daily for 4 weeks. Microscopy and cultures were performed prior to and at the end of the treatment and 2 weeks posttreatment. RESULTS: After 4 weeks therapy, 50% of itraconazole-treated patients and 30% of griseofulvin-treated patients had negative microscopy; 70% and

Plantar Trichophyton rubrum infections may cause dermatophytids on the hands.

Over a 2-year period, we saw 37 patients with a diagnosis of dermatophytid on the hands based on 1) culture-proven dermatophytosis on one or both feet; 2) symmetrical, secondary vesicular eruptions on the fingers and/or palmar aspects of the hands; and 3) a resolution in both areas of involvement after treatment of the dermatophytosis on the foot. During the study period, 128 patients had culture-proven dermatophytosis of the feet caused by Trichophyton rubrum. Nine of these (7%) developed dermatophytid. Seventy-eight patients had dermatophytosis of the feet caused by Trichophyton mentagrophyt

Contact dermatitis from telefax paper.

A nonatopic female secretary developed hand dermatitis after 1 year of full-time work with telefax paper. Her dermatitis improved on sick leave and vacation and relapsed at work. Patch testing showed allergic reactions to several fragrances, balsam of Peru, colophony and neomycin. In 3 patch test sessions, her own telefax papers gave a doubtful reaction which persisted for 4 days, but it could not be determined whether the reactions were allergic or irritant. The colophony content of the telefax paper was about 1%, and it was possible that colophony in the telefax paper was responsible for the

Hand dermatitis in the pottery industry.

Irritant hand dermatitis has long been recognized in the pottery industry. In our series, among workers handling glaze, sensitization to chromate was common and allergy to other metals and to biocides also occurred. Allergy to oil additives was found in mould makers. Whilst irritant hand dermatitis does occur, allergy to metals and biocides should be looked for in workers handling glazes, and allergy to oil additives in mould makers.

Occupational contact dermatitis from rhodium and cobalt.

We report a case of occupational allergic contact dermatitis from rhodium sulphate and cobalt chloride in a 29-year-old woman working in a goldsmith's workshop. The relevance and source of sensitization to these metals in our patient is described, and the literature on the subject reviewed.

[Allergic contact eczema caused by rubber-containing substances simulating pustulosis palmaris].

The allergic contact dermatitis usually shows eczematous skin lesions. In few cases however pustular reactions can be observed. They are often caused by sensitization to metals and halogens. Herein we report a case occurring as pustulosis palmaris, that has been identified as an occupational allergic contact dermatitis to black rubber.

Contact allergy to furazolidone.

A case of occupational contact allergy to furazolidone, used as an animal feed additive and as an antimicrobial drug in veterinary medicine, is described. The patient did not react to furazolidone 2% pet. Using PEG-400 and alcohol as patch test vehicles resulted in positive patch test reactions. No cross-reactions were observed to other nitrofuran derivatives (nitrofurazone, nitrofurantoin) or to furfural. The literature on contact allergy to nitrofurans is reviewed.

Occupational allergic contact dermatitis in beauticians.

We patch tested 13 beauticians with hand dermatitis between 1982 and 1986. They were all young female novice beauticians or those in training. The onset of their allergic dermatitis was noticed within 1 month to 1 year of their starting this occupation. Definite positive reactions to products were seen from hair dyes (as is, open test) (6/12), cold permanent wave primary solutions (as is, open test) (7/13) and a shampoo (1% aq., closed test) (1/13). Positive reactions to allergens were seen with para-phenylenediamine (1% pet) (12/13), ammonium thioglycolate (5% aq., open test) (3/7), para-tolu

Knuckle pads in live-chicken hangers.

A study of live-chicken hangers in a poultry processing plant demonstrated a high prevalence of callosities over the knuckles (knuckle pads) of both hands. Knuckle pads were observed in 56% (23/41) of live-chicken hangers, but in no (0/41) workers from other departments (p less than 0.001). The probable cause was the repeated striking and sliding of the knuckles against metal shackles in which live birds were being placed. Additional medical and ergonomic evaluation would be worthwhile to confirm the probable cause, to determine whether associated tissue disorders are present in the digits of

Hyperkeratotic (Norwegian) scabies and onychomycosis in an immunosuppressed patient.

Hyperkeratotic scabies and onychomycosis were associated in an immunosuppressed patient. Onychomycosis healed spontaneously after treatment of scabies. A possible interrelationship between Sarcoptes scabiei infestation of the hyponychium and dermatophytes infestation of the nail plate could be responsible for this peculiar evolution.

[Recurrent pyoderma as an occupational disease in a butcher].

Butchers seldom file for workman's compensation for the occupational disease pyoderma. In the case presented here there is a clear temporal correspondence between the practice of the trade and the onset of the disease. It is suggested that the disease, contracted by humans from animals, be recognized under number 3107 of the BKVO (list of occupational diseases recognized by West German law). No factors were found outside the butcher's trade which could have contributed to the pyodermia.
