The weakest point of cardiac resynchronization therapy: new technologies facing old terminology.

Patients with symptomatic heart failure (HF) and left bundle branch block (LBBB) are currently treated with biventricular pacing (BiV) which has a Class IA recommendation. Given the possibility to re-establish the inter and intra-ventricular synchrony, BiV is commonly referred to as cardiac resynchronization therapy (CRT). This wording is widely utilized and over time the terms BiV and CRT have become interchangeable. Conduction system pacing (CSP) is emerging as a valid therapeutic opportunity to obtain CRT restoring the native conduction via the Purkinje network. Therefore the acronym CRT is

Misleading ECG appearance of AV block due to concealed AV nodal conduction caused by interpolated ventricular ectopic beats.

Concealed conduction commonly occurs when a retrogradely conducted interpolated ectopic impulse enters the atrioventricular (AV) node; thus, the next sinus beat is not conducted to the ventricle or conducted with a prolonged PR interval because of increased refractoriness of AV conduction system. A 67-year-old man had complaints of exertional fatigue and palpitations at rest. His blood pressure was 110/70 mmHg and heart rate was 78 beats/min. Auscultation revealed a mild systolic murmur at the apex and an irregular rhythm. His electrocardiogram was normal, except for the presence of frequent p

Congenital atrioventricular block: histological aspects.

It is known that maternal immunological factors such as systemic disease are involved in the genesis of cardiac conduction problems in the fetus but the histologic changes in the conduction system are less documented. We report the case of a 33-year-old woman with no significant medical history. Her first pregnancy was induced by Clomifene. At 17 weeks of gestation, the fetus presented sonographic abnormalities characteristic of a complete atrioventricular block. Biological investigations found anti-SSA and -SSB antibodies. Clinical history search for systemic disease was positive for photosen

Left Ventricular Lead Electrical Delay Is a Predictor of Mortality in Patients With Cardiac Resynchronization Therapy.

BACKGROUND: Electric left ventricular lead position, assessed by the electric delay from the beginning of the QRS complex to the local LV electrogram (QLV), was found in previous studies to be a strong predictor of short-term response to cardiac resynchronization therapy. We hypothesized that suboptimum electric position of the left ventricular lead is associated with an excess of heart failure events and mortality. METHODS AND RESULTS: We analyzed the clinical outcome of patients with left bundle branch block or intraventricular conduction delay treated with cardiac resynchronization therapy

Effects of sympathetic tone on vagally induced phasic changes in heart rate and atrioventricular node conduction in the anesthetized dog.

We examined the effects of stellate ganglia stimulation on the phase-dependent chronotropic and dromotropic responses to brief vagal bursts in open-chest anesthetized dogs. Stellate stimulation affected the phasic vagal effects on heart rate by shortening the latent period, shifting the phase at which maximum decrease in heart rate occurred to earlier phases, and reducing the maximum decrease in heart rate. These effects were due primarily to an increase in the basic heart rate. No significant sympathetic-parasympathetic interaction occurred for heart rate, indicating that accentuated antagoni

New observation of electrocardiogram during sinus rhythm on the atriofascicular and decremental atrioventricular pathways/clinical perspective: [corrected] terminal QRS [corrected] complex slurring or notching.

BACKGROUND: Atriofascicular and decremental atrioventricular pathways are variants of accessory pathways with anterograde decremental conduction properties. They result in typical wide Quantronic Resonance System (QRS) tachycardia of left bundle branch block morphology. Data on the sinus rhythm electrocardiographic characteristics are limited. METHODS AND RESULTS: Thirty patients with accessory pathways of anterograde decremental conduction properties were studied retrospectively (10 atriofascicular pathways and 20 decremental atrioventricular pathways). All patients had a pre-excited atriove

Dual atrioventricular nodal pathways in children without supraventricular tachycardia.

Atrioventricular conduction characteristics were examined by atrial extrastimulus technique in 45 children aged 50 days to 14 years following diagnostic cardiac catheterization. None of the patients had a history of supraventricular tachycardia and none had evidence of pre-excitation on routine electrocardiogram. By plotting the A2H2 and H1H2 intervals against corresponding S1S2 intervals 2 types of conduction curve were obtained. Eight of the 45 children (17.7%) showed discontinuous conduction curves, characteristic of dual atrioventricular nodal pathways, whereas 37 patients had continuous c

P-wave peak time: An emerging electrocardiographic marker of atrial stress and ischemic burden-review.

The 12‑lead electrocardiogram (ECG) remains a cornerstone of cardiovascular assessment, providing a noninvasive window into cardiac electrical and structural function beyond other markers of ischemia and arrhythmias. P-wave peak time (PWPT), defined as the interval from P-wave onset to its maximal amplitude, has emerged as a novel electrocardiographic marker of atrial conduction and hemodynamic stress. Increasing evidence suggests that PWPT prolongation accompanies elevated ventricular filling pressures, atrial stretch, and chronic atrial remodeling across a spectrum of ischemic conditions, in

Strain detects substrate: Pioneering non-invasive visualization of atrial electropathology.

BACKGROUND: Non-invasive assessment of arrhythmogenic substrates underlying persistent atrial fibrillation (AF) could potentially provide patient-tailored ablation therapy. Speckle Tracking Echocardiography (STE) is an imaging modality, which can be used to assess atrial biomechanical function. As biomechanical dysfunction is driven by electrical dysfunction, STE may serve as a diagnostic tool to detect AF-related arrhythmogenic substrates. PURPOSE: To investigate which non-invasive STE-derived measurements correlate best with invasively determined electrophysiological properties as a first s

Early assessment of ventricular synchronization and function after left bundle-branch-area pacing with right bundle-branch block.

AIM: To evaluate ventricular synchronization and function in patients with right bundle-branch block after left bundle-branch-area pacing (LBBAP) by echocardiography. METHODS: Forty patients who successfully received LBBAP were selected and divided into the right bundle-branch block group (RBBB group) and the non-RBBB group by pre-operation ECG. Echocardiography and follow-up were performed 1 month after operation. Interventricular synchronization was evaluated by tissue Doppler (TDI), tissue mitral annular displacement (TMAD), and interventricular mechanical delay. The tricuspid annular plan

Catheter Ablation of Atrial Arrhythmias in Patients Post-Fontan.

Atrial arrhythmias are highly prevalent in the aging Fontan population and contribute importantly to morbidity and mortality. Although the most common arrhythmia is scar-based intra-atrial re-entrant tachycardia, various other arrhythmias may occur, including focal atrial tachycardia, atrioventricular node-dependent tachycardias, and atrial fibrillation. The type and prevalence of atrial arrhythmia is determined, in part, by the underlying congenital defect and variant of Fontan surgery. Although the cumulative incidence of atrial tachyarrhythmias has decreased substantially from the atriopulm

Smartwatch-based detection of cardiac arrhythmias: Beyond the differentiation between sinus rhythm and atrial fibrillation.

Within the span of a few years, watches have functionally morphed from objects that tell time to wearable minicomputers that allow real-time recording of electrocardiograms (ECGs). Considerable information can be deduced from these single lead tracings, and it is now not uncommon to see patients in whom diagnostic tracings of clinically relevant but elusive arrhythmias are captured using a smartwatch. Empowering individuals to record their own ECG tracings in scenarios such as palpitations, syncope, and for risk stratification of sudden death intuitively has considerable potential, but its val

Differences in right-to-left vs left-to-right interventricular conduction times in patients indicated to cardiac resynchronization therapy.

INTRODUCTION: Differences in conduction times from right ventricle to left ventricle and from left ventricle to right ventricle respectively were observed during biventricular devices implantation when changing pacing vector direction. In this article the phenomenon of interventricular conduction time differences is described and assessed in relationship to various clinical and electrophysiological parameters. METHODS: In 62 consecutive patients (9 females) interventricular conduction times between right and left ventricle in both directions were measured during cardiac resynchronization ther

Combined epicardial and endocardial ablation for atrial fibrillation: Best practices and guide to hybrid convergent procedures.

The absence of strategies to consistently and effectively address nonparoxysmal atrial fibrillation by nonpharmacological interventions has represented a long-standing treatment gap. A combined epicardial/endocardial ablation strategy, the hybrid Convergent procedure, was developed in response to this clinical need. A subxiphoid incision is used to access the pericardial space facilitating an epicardial ablation directed at isolation of the posterior wall of the left atrium. This is followed by an endocardial ablation to complete isolation of the pulmonary veins and for additional ablation as

Normal ranges and potential modifiers of T-wave morphology parameters among healthy individuals: A meta-analysis.

BACKGROUND: T-wave morphology parameters, such as total cosine R-to-T (TCRT), T-wave loop dispersion (TWLD), T-wave morphology dispersion (TMD), and T-wave residuum (TWR), were suggested to be robust markers for adverse cardiovascular outcomes. Yet, the normal range of these parameters is unknown. This study aimed to evaluate the weighted normal values of T-wave morphology parameters of healthy individuals and study the effect of potential modifiers. METHODS: A systematic search of studies published in PubMed was conducted. Only those reporting on control groups of healthy individuals were in

Study of factors affecting the progression and termination of drug induced Torsade de pointes in two dimensional cardiac tissue.

INTRODUCTION: To study the conditions leading to the initiation and termination of drug induced Torsade de pointes (TdP) along with QT prolongation. METHODS: A 2D anisotropic transmural section of the ventricular myocardium is modeled using the TP06 equations and the cells are interconnected with gap junction conductances (GJC). The tissue is remodeled by reducing the repolarization reserve (by increasing calcium current (ICaL)) of all cells thus making them vulnerable to development of early after depolarizations (EADs). RESULTS: Clinical risk conditions like decreased potassium current (IK

Transient Electromechanical Dissociation Caused by a Transesophageal Echocardiography Probe.

Intraoperative transesophageal echocardiography is an established modality for confirming the preoperative diagnosis, assess adequacy of surgical repair, identify residual problems, and help improve the outcome after operation. Despite the multitude of advantages, intraoperative transesophageal echocardiography can affect the hemodynamic parameters in small neonates and children. A severe transient electromechanical dissociation caused by the anteflexion of a transesophageal echocardiography probe during a total anomalous pulmonary venous connection repair in an infant is reported.
