A controlled trial of synthetic surfactant in infants weighing 1250 g or more with respiratory distress syndrome. The American Exosurf Neonatal Study Group I, and the Canadian Exosurf Neonatal Study Group.

BACKGROUND: Surfactant-replacement therapy is now recognized as a life-saving and safe intervention in small premature infants, but there is … mechanical ventilation significantly sooner than those given placebo, and they less often required high-frequency ventilation or extracorporeal membrane oxygenation. The primary side effect observed more frequently among the infants who received surfactant treatment was pulmonary hemorrhage (six infants vs. one infant, P = 0.055). CONCLUSIONS: In infants weighing at least …

[Are there predictors for proliferative retinopathy of prematurity and is supplemental oxygen a useful conservative treatment option?].

BACKGROUND: Especially very immature preterm babies develop retinopathy of prematurity (ROP). This study aims at analysing risk factors for proliferative ROP and realizing the efficiency of supplemental oxygen therapy. PATIENTS: 180 preterm babies with birth weight < or = 1 500 grams were included retrospectively. METHODS: To determine potential predictors all preterm babies with ROP grade > or = 3 were matched to pairs with similar immature babies with ROP 1 or 2. Additionally we examined the influence of supplemental oxygen therapy on the coagulation rate of high grade retinopathy. RESULT

High-Frequency Chest Wall Oscillation in Critical Care: A Systematic Review.

INTRODUCTION: Airway secretion retention represents a major clinical challenge in critically ill patients, promoting gas exchange impairment and atelectasis, precipitating failure of noninvasive respiratory support, and prolonging invasive mechanical ventilation, with an associated increase in respiratory-related complications and mortality. High-frequency chest wall oscillation (HFCWO) is a noninvasive airway clearance technique widely used in chronic respiratory diseases, but its role in the intensive care unit (ICU) remains unclear. This systematic review aimed to evaluate the available evi

Influenza A (H1N1pdm09)-Related Critical Illness and Mortality in Mexico and Canada, 2014.

OBJECTIVES: The 2009-2010 influenza A (H1N1pdm09) pandemic caused substantial morbidity and mortality among young patients; however, mortality estimates have been confounded by regional differences in eligibility criteria and inclusion of selected populations. In 2013-2014, H1N1pdm09 became North America's dominant seasonal influenza strain. Our objective was to compare the baseline characteristics, resources, and treatments with outcomes among critically ill patients with influenza A (H1N1pdm09) in Mexican and Canadian hospitals in 2014 using consistent eligibility criteria. DESIGN: Observat

Multiple organ failure and severe bone marrow dysfunction in two 18 year-old Caucasian patients: Epstein-Barr virus and the haemophagocytic syndrome.

Haemophagocytic lymphohistiocytosis secondary to viral infection is an unusual but well recognised cause of bone marrow dysfunction and multiple organ failure in young patients. Two 18 year-old patients were admitted to a tertiary liver unit with features of acute liver failure, cardio-respiratory collapse and pancytopenia. Serological tests and bone marrow examination with in-situ hybridisation revealed severe acquired haemophagocytic lymphohistiocytosis secondary to acute Epstein-Barr virus infection. Both patients died despite full supportive therapy; the first due to pulmonary haemorrhage,

Pulmonary hyperinflation and respiratory distress following solvent aspiration in a patient with asthma: expectoration of bronchial casts and clinical improvement with high-frequency chest wall oscillation.

An 18-year-old student with a history of asthma accidentally inhaled organic solvent during a class, with immediate cough and dyspnea that worsened over several hours. He presented in severe respiratory distress, with hypoxemia and marked pulmonary hyperinflation. Administration of inhaled bronchodilator was ineffective because of agitation, and the patient could not be positioned for chest physiotherapy to treat presumed widespread mucus plugging. High-frequency chest wall oscillation (HFCWO) in the sitting position initially caused increased distress but was subsequently tolerated when nonin

Wolf-Hirschhorn syndrome: case report and review of the chromosomal aberrations associated with diaphragmatic defects.

A female fetus showing severe growth retardation was delivered at 31 weeks of gestation because of fetal distress. At birth, the infant showed bradycardia and no spontaneous breathing. Although high frequency oscillatory ventilation was started, severe asphyxia persisted and the infant died of respiratory insufficiency. At the autopsy, the propositus showed microcephaly, prominent glabella, broad bridge of the nose, ocular hypertelorism, poorly differentiated and low-set ears, bilateral palatoschisis, and micrognathia. Midline closure defects of the cervical spine bodies, lower jaw, and skull

An experimental study of pulsatile pipe flow in the transition range.

The study of pulsatile flows is relevant to many areas of applications. Typical applications include aerodynamics, biofluid mechanics, wind flows, and gas transport. Transition to turbulence during pulsatile flow is physiologically and clinically important. It has been suggested as a possible mechanism to enhance the transport of gases during high-frequency ventilation, may be related to valvular regurgitation and heart murmurs and to post stenotic dilatation and aneurysms. Measurements in a pulsatile pipe flow with a superimposed mean flow are reported. Data were taken in a water flow with me…

[Management in sequelae of pulmonary tuberculosis (chronic respiratory failure)].

1. The high risk groups for chronic respiratory failure due to pulmonary tuberculosis were identified based on the chest X-ray sketches on the TB Registration Cards in a health center. They showed 10.8-14.8% of mortality which was highly related to respiratory failure (ca 64%). To prevent developing acute exacerbation, they were educated collectively in daily care at the health center and were introduced to medical services immediately upon demand. 2. The 248 patients suffering from chronic respiratory failure or pre-respiratory failure due to pulmonary tuberculosis were divided into three gro

Thoracoscopic-assisted right thoracic approach for resection of proximal left main bronchial adenoid cystic carcinoma.

Adenoid cystic carcinoma (ACC) of the proximal left main bronchus is rare and presents significant surgical challenges due to its anatomical location. We report the case of a 31-year-old woman who presented with dyspnea caused by a proximal left main bronchial ACC. Initial bronchoscopic debulking provided histological diagnosis and symptomatic relief. For definitive staging, a post-debulking PET-CT confirmed the presence of residual metabolically active disease at the primary site and showed no evidence of distant metastasis. Definitive treatment involved thoracoscopic-assisted sleeve resectio

Abdominal Aortic Thrombosis in a Preterm Baby With Congenital Diaphragmatic Hernia.

Congenital diaphragmatic hernia (CDH) is a serious congenital anomaly often associated with pulmonary hypoplasia and persistent pulmonary hypertension of the newborn (PPHN). Central vascular access such as umbilical arterial catheters (UACs) is routinely used in neonatal intensive care but is associated with the risk of vascular complications, including thromboembolic events. We present a case of preterm dichorionic diamniotic (DCDA) twins born at 34 weeks of gestation with antenatally diagnosed CDH. The baby developed severe respiratory distress requiring high-frequency oscillatory ventilatio

Preclinical and clinical studies of smoke-inhalation-induced acute lung injury: update on both pathogenesis and innovative therapy.

Smoke-inhalation-induced acute lung injury (SI-ALI) is a leading cause of morbidity and mortality in victims of fire tragedies. SI-ALI contributes to an estimated 30% of burn-caused patient deaths, and recently, more attention has been paid to the specific interventions for this devastating respiratory illness. In the last decade, much progress has been made in the understanding of SI-ALI patho-mechanisms and in the development of new therapeutic strategies in both preclinical and clinical studies. This article reviews the recent progress in the treatment of SI-ALI, based on pathophysiology, t

Breathing detection from tracheal sounds in both temporal and frequency domains in the context of phrenic nerve stimulation.

Electrical stimulation of the phrenic nerves via implanted devices allows to counteract some disadvantages of mechanical ventilation in patients with high tetraplegia or Ondine's syndrome. Existing devices do not allow to monitor breathing or to adapt the electroventilation to patients' actual needs. A reliable breathing monitor with an inbuilt alarm function would improve patient safety. In our study, a real-time acoustic breathing detection method is proposed as a possible solution to improve implanted phrenic stimulation. A new algorithm to process tracheal sounds has been developed. It com

Incorrect Holter-ECG analysis caused by the pacemaker delivering small high-frequency currents for thoracic impedance measurement.

In an 86-year-old woman who had been treated for sick sinus syndrome, the small high-frequency current delivered by pacemaker in order to measure the minutes ventilation for utilizing the rate-response mode was transiently over-sensed on the Holter electrocardiogram. Although her pacing system was working appropriately, the numbers of the paced beats on the automatic Holter analysis were undercounted (from >60% to <5%) during the over-sensing periods because these currents were recognized as multiple pacing spikes. Physicians need to pay attention to such multiple pacing spike markers, because

Primary Combined Latissimus Dorsi and Serratus Anterior Flap Repair of Right-Sided Congenital Diaphragmatic Agenesis in a Neonate.

Large diaphragmatic defects can be repaired with latissimus dorsi and serratus anterior muscle flaps. We report the first successful primary repair of complete congenital diaphragmatic agenesis using a combination of autologous living bio-tissue and synthetic mesh in a neonate born in the NMC Specialty Hospital in Dubai, United Arab Emirates, in May 2014. Poor Apgar scores, a scaphoid abdomen and absent breath sounds over the right hemithorax were observed at birth. Chest and abdominal X-rays revealed a diaphragmatic hernia. The neonate was stabilised using high-frequency oscillatory ventilati

Management dilemmas in patients with hereditary renal adysplasia.

We report a neonatal case of right renal aplasia with left dysplastic kidney and mild pulmonary hypoplasia. The respiratory insufficiency gradually improved on high frequency oscillation and conventional ventilation. Severe hypotension necessitated the use of inotrops. Anuria and electrolyte imbalances were managed by peritoneal dialysis. At age 13 days the baby had a small bowel perforation, developed septic shock and, after discussion with the multi-disciplinary team and the family, inotropic support was withdrawn and the baby died. The family history revealed the father had a newborn from a

Transesophageal electroventilation. Optimization and improvement.

For electroventilation, short duration pulse trains (0.1 ms) were applied between two axillary pads (transchest) and compared with transesophageal electroventilation where pulses passed between the same pads to an esophageal electrode in apneic, pentobarbitone-anesthetized pigs. Significantly greater tidal volumes were produced by transesophageal electroventilation in comparison with transchest. As measured by maintained tidal volumes, duration of inspiratory air flow, peak flow, percentage hemoglobin oxygen saturation (%SaO2), and end tidal carbon dioxide concentration (ETCO2), the optimal re
