PURPOSE: To determine the management of torsional humeral shaft fractures in a group of expert shoulder and elbow surgeons and analyse the rate of return to sport of these throwing athletes.
METHODS: A survey was sent to all physician members of two prominent sports medicine professional associations: the American Shoulder and Elbow Surgeons and the Herodicus Society. Due to the rare nature of this injury, a historical survey of management and return to play was performed to allow analysis of trends in treatment and return to play after both non-operative and operative management.
RESULTS: T

BACKGROUND: Anterior shoulder dislocation combined with humeral surgical neck fracture is a severe injury that often requires emergency surgery. However, for some patients, such as those of advanced age or with surgical contraindications, closed reduction can be considered. This retrospective cohort study aimed to confirm the feasibility and clinical outcomes of the closed reduction technique with effective external fixation for the treatment of anterior shoulder dislocation combined with humeral surgical neck fracture in elderly patients.
METHOD: From January 2010 to March 2024, a total of 5

Managing chronic fracture pain presents a therapeutic challenge due to the development of resistance to conventional analgesics. We report the successful use of peripheral nerve stimulation (PNS) via an infraclavicular brachial plexus approach to treat refractory chronic pain after a humeral neck fracture in a 75-year-old woman. The patient achieved complete pain resolution, discontinued all adjuvant analgesics, and experienced no procedure-related complications. This case supports the emerging role of PNS as an effective opioid-sparing intervention for chronic upper extremity pain. .

Background and Clinical Significance: Neurological complications in extension-type-III supracondylar humeral fractures (SCHFs) in children represent 11% of cases. An extension-type-III SCHF with posterolateral displacement of the distal fragment is commonly associated with damage to the median nerve and the anterior interosseous nerve (AIN). Neurological complications are often unnoticed, and their immediate postoperative diagnosis is difficult, particularly in young children. Neurapraxia, the most common complication, usually undergoes spontaneous nerve recovery. Case Presentation: We report …

BACKGROUND: Vitality-threatening proximal humerus fractures often provide an indication for prosthetic treatment. We investigated the issue of how anatomic hemiprostheses perform in younger, functionally challenging patients with the use of a specific fracture stem and systematic tuberosity management in medium-term follow-up.
METHODS: Thirteen skeletally mature patients with a mean age of 64 ± 9 years and a minimum follow-up of 1 year after primary open-stem hemiarthroplasty for 3- and 4-part proximal humeral fractures were included. All patients were followed up regarding their clinical cou…

CASE: We present the case of a 36-year-old patient with a 4-part proximal humerus fracture with subcoracoid dislocation and devascularization of the humeral head after a fall onto his right shoulder.
CONCLUSION: The patient was successfully treated with open reduction and locking plate fixation to demonstrate that a successful postoperative functional outcome with humeral head survival can be achieved in these complex situations.

AIMS: The reasons for failure of a hemirthroplasty (HA) when used to treat a proximal humeral fracture include displaced or necrotic tuberosities, insufficient metaphyseal bone-stock, and rotator cuff tears. Reverse total shoulder arthroplasty (rTSA) is often the only remaining form of treatment in these patients. The aim of this study was to evaluate the clinical outcome after conversions from a failed HA to rTSA.
MATERIAL AND METHODS: A total of 35 patients, in whom a HA, as treatment for a fracture of the proximal humerus, had failed, underwent conversion to a rTSA. A total of 28 were avai

Open reduction and internal fixation (ORIF) via the deltopectoral approach is the gold standard for operatively treated proximal humeral fractures when joint preservation is desired. Indications include an unacceptable deformity, need for stability and early mobilization, and osteoporotic bone. (1) A 12 to 14-cm incision is made in the deltopectoral groove. The fracture is reduced. (2) Pins and tension sutures are placed for provisional fixation. (3) The locking plate is placed with unicortical screws in the metaphysis of the proximal part of the humerus and bicortical screws in the shaft. (4)…

INTRODUCTION: Lateral humeral condylar fractures are the second most common elbow fractures in children. We present the long-term clinical and radiographic results.
MATERIAL AND METHODS: All children (<16 years) in the geographic catchment area with lateral condylar fracture were asked to participate and 32 (76.2%) patients enrolled. Clinical and functional results are compared with randomly selected but age and sex matched normal controls at 12.4 years (range 10.6 to 16.0).
RESULTS: Unsatisfactory clinical outcomes were found in 40.6% of the fracture cases vs. 6.3% controls (P = 0.003), acc

PURPOSE: The role of reverse total shoulder arthroplasty (RTSA) for three and four-part proximal humerus fractures is evolving. However, there does not appear to be a clear consensus amongst surgeons. The purpose of this study is to further define the standard of care, assessing surgeon preference and treatment considerations for management of such fractures.
METHODS: Orthopaedic surgeons were surveyed on their training, practice setting, and experience regarding management of four-part proximal humerus fractures. The survey also presented five representative cases to assess treatment prefere

INTRODUCTION: The "terrible tetrad" of the elbow - simultaneous posterior ulno-humeral dislocation, comminuted radial head fracture with dislocation, coronoid process fracture, and proximal ulnar fracture - is an injury pattern that simultaneously fulfills criteria for both the terrible triad and a Monteggia-type fracture-dislocation. Fewer than fifteen cases have been documented worldwide. We present two cases managed by different surgeons at different institutions and propose the formal adoption of the term "terrible tetrad."
CASE REPORT: Case 1: A 50-year-old male painter sustained a right

INTRODUCTION: Plate osteosynthesis has traditionally been the preferred treatment for humeral shaft fractures requiring surgical intervention. However, plating involves an open approach, necessitating careful and meticulous handling of soft tissue. As a result, the anterior bridge plating (ABP) as minimally invasive plate osteosynthesis (MIPO) is emerging as a promising technique for managing humeral shaft fractures. We performed an analysis on both groups (40 patients each), and our primary objectives compared were radiological outcome, functional outcome using UCLA shoulder score Mayo elbow s

BACKGROUND: Fibular allograft (FA) augmentation is proposed to address the lack of medial column support in plate-fixed proximal humeral fractures; however, the role of FA augmentation in these fractures remains inadequately elucidated. Our previous clinical trial did not show additional benefit in inserting an FA when treating adults with medial column comminuted proximal humeral fractures. Nonetheless, it is possible that some subgroups may still derive benefits from FA. This study aimed to investigate whether there were any subgroups benefit from FA.
