Impact of race and ethnicity on the outcome of preterm infants below 32 weeks gestation.

OBJECTIVES: To determine the impact of race/ethnicity on mortality and morbidity such as intraventricular hemorrhage (IVH), periventricular … cant impact of race/ethnicity on the mean gestational age in any of the gestational age categories. Infant mortality and morbidity in each gestational age category by race/ethnicity were comparable. The multiple birth black infants were seen to have a lower gestational age and birth weight as compared to singleton black as well as to white, Hispanic and other race …

Disparities in tracheostomies and gastrostomy tubes in infants at the edge of viability.

OBJECTIVES: To assess trends of clinical practice of tracheostomy and gastrostomy tube (G-tube) placement over a 25-year period and to assess the association of both procedures with outcomes in extremely low birth weight (ELBW) infants (<1000 g). METHODS: We reviewed and analyzed data obtained from the National Inpatient Sample dataset from 1993 to 2018. ELBW infants who received a tracheostomy and or G-tube were included. Regression analysis was performed to assess the association of tracheostomy and/or G-tube placement with mortality after controlling for confounding variables. RESULTS: A

Current Reproductive Health Policy and Outcomes in the United States.

Regulation of abortion, contraception, and other facets of reproductive health care are historically intertwined into our nation's approach to controlling reproduction. Government interference into reproductive health care is associated with worse maternal and infant health outcomes. In the following review, we discuss the history of these restrictions and detail the ways in which federal and state-level policies restricting abortion, contraception, care for minors, and funding for reproductive health services impact reproductive health outcomes, including maternal and infant morbidity and mor

Maternal Smoking, Alcohol and Recreational Drug Use and the Risk of SIDS Among a US Urban Black Population.

BACKGROUND: Rates of sudden infant death syndrome (SIDS) are twice as high among Black infants compared to white infants in the US. While the contribution of sleep environment factors to this disparity is known, little is known about the risk of SIDS among Black infants in relation to maternal prenatal smoking, alcohol and drug use as well as infant smoke exposure. OBJECTIVE: To assess the contribution of maternal substance use during pregnancy and the potential interactions with infant bedsharing in a high-risk, urban Black population. METHODS: The Chicago Infant Mortality Study (CIMS) coll…

Volume-targeted versus pressure-limited ventilation for preterm infants.

BACKGROUND: To compare the effects of volume-targeted ventilation (VTV) with pressure-limited ventilation (PLV) in preterm infants. METHODS: A total of 100 preterm infants who required mechanical ventilation during the two study periods were investigated. PLV was used for 50 preterm infants during period 1 and VTV was used for 50 preterm infants during period 2. Clinical outcomes including mortality rate, duration of mechanical ventilation, air leak syndrome, hypocarbia, hypercarbia, hypoxemia, combined outcome of death or bronchopulmonary dysplasia (BPD), intraventricular hemorrhage, and ret

Group B streptococcal sepsis in adults and infants. Contrasts and comparisons.

Group B streptococcal infection may result in significant morbidity and mortality in both infants and adults. The experience with group B streptococcal disease was analyzed at one medical center over a ten-year period from 1975 to 1984. Streptococcus agalactiae bacteremia was observed in 29 adults and 26 infants, with an attack rate of 0.2 cases per 1000 adult admissions and 3.2 cases per 1000 live births, respectively. The majority of adult infections apparently occurred as a result of nosocomial acquisition and was associated with a high mortality rate of 38%. Risk factors for group B strept

Surfactant therapy guided by tests for lung maturity in preterm infants at risk of respiratory distress syndrome.

BACKGROUND: Administration of various exogenous surfactant preparations has been shown to decrease lung injury and pneumothorax and improve survival in very preterm infants with respiratory distress syndrome (RDS). There is no consensus on the threshold for surfactant administration, to allow timely intervention and avoid over-treatment, also considering the invasiveness of the procedure and its cost. Rapid tests for lung maturity, which include the click test, lamellar body counts and stable microbubble test, might guide the identification of those infants needing surfactant administration.

Trend in mortality from respiratory distress syndrome in the United States, 1970-1995.

OBJECTIVE: We examined the trend in mortality caused by respiratory distress syndrome (RDS) and its impact on changes in infant and neonatal mortality rates (IMR, NMR) in the United States. STUDY DESIGN: Data on infant deaths in the United States for the period 1970 through 1995 were used to compare RDS-specific IMR to other cause-specific IMR. Data from the U.S. birth cohorts of 1985 through 1991 were used to examine birth weight- and RDS-specific NMRs. RESULTS: IMR from RDS declined from 2.6 per 1000 live births in 1970 to 0.4 per 1000 in 1995. More than three quarters of this decline occu

A Synbiotic of Bifidobacterium animalis subsp. lactis BB-12 and 2'-FL Alleviate Infant Diarrhea and Anxiety-like Behaviors via Gut Microbiota Modulation in an EPEC O127 Infection Model.

Background/Objectives: Infant diarrhea is a major global cause of morbidity and mortality. While Bifidobacterium is linked to diarrhea, its preventive effects, underlying mechanisms, and potential synergistic benefits with prebiotics remain unclear. The objective of this study was to explore the efficacy of a synbiotic composed of Bifidobacterium animalis subsp. lactis BB-12 (BB-12) and 2'-fucosyllactose (2'-FL) in alleviating infant diarrhea. Methods: One-week-old C57BL/6J mice were used to construct a model of infant diarrhea via infection with enteropathogenic Escherichia coli (EPEC) O127.

Current and emerging treatments for neonatal sepsis.

Introduction: Mortality due to sepsis is still prevalent, peaking at extreme ages of life including infancy. Despite many efforts, the peculiarity of the infant immune system has limited further advances in its treatment. Indeed, neonates experience a dramatic physiological transition from immune tolerance to the maternal antigens to functional maturity. Such a transition is extremely dynamic, as is the pathophysiology of infant sepsis, which is dependent on many infant, maternal, and environmental factors.Areas covered: In this review, the authors critically update and summarize the current p

Impact of mode of conception on neonatal and neurodevelopmental outcomes in preterm infants.

STUDY QUESTION: Is assisted conception associated with neonatal morbidity and mortality and with neurodevelopmental impairment at 2 years of corrected age in preterm infants born before 34 weeks of gestational age? SUMMARY ANSWER: Assisted conception is not associated with an increase in neonatal morbidity and mortality and is even significantly associated with a better 2-year neurodevelopmental outcome in preterm infants. WHAT IS KNOWN ALREADY: Assisted conception appears to increase the rate of preterm births, though few studies have analysed outcomes for these preterm infants. STUDY DESI

[Prognosis in newborns after mother's preeclampsia].

Preeclampsia (PE) is associated with 3 main risks for the fetus: perinatal death, intrauterine growth restriction (IUGR), preterm birth. Perinatal mortality is increased in infants with IUGR or asphyxia. Conversely, mortality and morbidity associated with preterm birth are not altered by PE without IUGR or asphyxia. Very preterm infant with IUGR are exposed to high risk of prolonged respiratory insufficiency. Neurological complications of prematurity are not more frequent in infants born to mothers with PE. Nevertheless birth asphyxia, (i.e. placental abruption) is associated with impaired neu

Postdischarge Mortality Prediction in Sub-Saharan Africa.

BACKGROUND: Although the burden of postdischarge mortality (PDM) in low-income settings appears to be significant, no clear recommendations have been proposed in relation to follow-up care after hospitalization. We aimed to determine the burden of pediatric PDM and develop predictive models to identify children who are at risk for dying after discharge. METHODS: Deaths after hospital discharge among children aged <15 years in the last 17 years were reviewed in an area under demographic and morbidity surveillance in Southern Mozambique. We determined PDM over time (up to 90 days) and derived p
