Single-cell multi-omics analysis of human pancreatic islets reveals novel cellular states in type 1 diabetes.

Type 1 diabetes (T1D) is an autoimmune disease in which immune cells destroy insulin-producing beta cells. The aetiology of this complex disease is dependent on the interplay of multiple heterogeneous cell types in the pancreatic environment. Here, we provide a single-cell atlas of pancreatic islets of 24 T1D, autoantibody-positive and nondiabetic organ donors across multiple quantitative modalities including ~80,000 cells using single-cell transcriptomics, ~7,000,000 cells using cytometry by time of flight and ~1,000,000 cells using in situ imaging mass cytometry. We develop an advanced integ

Psychological Evaluation and Management of Chronic Pancreatitis.

Chronic pancreatitis is a chronic digestive disorder that greatly diminishes the quality of life and is associated with significant psychological distress. A best practice recommendation in treating chronic pancreatitis is offering care in a multidisciplinary model that includes access to a behavioral health provider among other medical professionals. Behavioral interventions for patients with chronic pancreatitis have promise to improve the management of pain, comorbid psychiatric symptoms, and quality of life. If surgical interventions such as a total pancreatectomy islet autotransplant are

Beta cell and immune cell interactions in autoimmune type 1 diabetes: How they meet and talk to each other.

BACKGROUND: The highly complex pathogenesis of Type 1 Diabetes is driven by several immune cell types with both effector and regulatory characteristics, which ultimately ends in the destruction of the insulin-producing beta cells. There are multiple layers of interaction between these immune cell populations and the pancreatic islets. SCOPE OF REVIEW: In this review article, we aim to discuss important recent insights into the multiple layers of interaction between immune cell populations and the pancreatic islets. Specifically, we discuss the environment where immune and beta cell interactio

Microencapsulation-based cell therapies.

Mapping a new therapeutic route can be fraught with challenges, but recent developments in the preparation and properties of small particles combined with significant improvements to tried and tested techniques offer refined cell targeting with tremendous translational potential. Regenerating new cells through the use of compounds that regulate epigenetic pathways represents an attractive approach that is gaining increased attention for the treatment of several diseases including Type 1 Diabetes and cardiomyopathy. However, cells that have been regenerated using epigenetic agents will still en

FasL microgels induce immune acceptance of islet allografts in nonhuman primates.

Islet transplantation to treat insulin-dependent diabetes is greatly limited by the need for maintenance immunosuppression. We report a strategy through which cotransplantation of allogeneic islets and streptavidin (SA)-FasL-presenting microgels to the omentum under transient rapamycin monotherapy resulted in robust glycemic control, sustained C-peptide levels, and graft survival in diabetic nonhuman primates for >6 months. Surgical extraction of the graft resulted in prompt hyperglycemia. In contrast, animals receiving microgels without SA-FasL under the same rapamycin regimen rejected islet

Dynamic Ins2 Gene Activity Defines β-Cell Maturity States.

Transcriptional and functional cellular specialization has been described for insulin-secreting β-cells of the endocrine pancreas. However, it is not clear whether β-cell heterogeneity is stable or reflects dynamic cellular states. We investigated the temporal kinetics of endogenous insulin gene activity using live cell imaging, with complementary experiments using FACS and single-cell RNA sequencing, in β-cells from Ins2GFP knockin mice. In vivo staining and FACS analysis of islets from Ins2GFP mice confirmed that at a given moment, ∼25% of β-cells exhibited significantly higher activity at t

Importance of Intestinal Environment and Cellular Plasticity of Islets in the Development of Postpancreatectomy Diabetes.

OBJECTIVE: To elucidate the pathogenesis of postpancreatectomy diabetes mellitus (PPDM). RESEARCH DESIGN AND METHODS: Forty-eight patients without diabetes undergoing either pancreatoduodenectomy (PD) (n = 20) or distal pancreatectomy (DP) (n = 28) were included. A 75-g oral glucose tolerance test was performed every 6 months. Microbiome composition and short-chain fatty acids (SCFAs) in feces were examined before and 6 months after surgery. The association of histological characteristics of the resected pancreas with PPDM was examined. RESULTS: During follow-up (median 3.19 years), 2 of 20

Effect of sulfur dioxide exposure on histopathology and morphometry of pancreatic islet cells and glycemic indices in Wistar rats.

Sulfur dioxide (SO2) is a ubiquitous air pollutant. Recent studies suggest that SO2 is a momentous risk factor for diabetes mellitus (DM). The present investigation aimed to evaluate the effects of SO2 exposure on histopathology and morphometry of pancreatic islet cells and serum glycemic indices in rats. Sixteen male Wistar rats were divided equally into SO2 and control groups. SO2 group was exposed to 10 parts per million (ppm) SO2 for 5 weeks (6 days a week, 3 h/day) and control group to filtered air for the same time as SO2 group. Blood serums were collected and pancreatic tissue isolated.

Solving the Puzzle of Immune Tolerance for β-Cell Replacement Therapy for Type 1 Diabetes.

Type 1 diabetes mellitus results from autoimmune destruction of pancreatic β cells. Insulin treatment is often inadequate in preventing devastating complications. Replacing β cells using stem cell-derived islets while fostering immune tolerance, exemplified in Yoshihara et al., holds the promise of a curative therapy for this disease.

Altered cannabinoid receptor expression in pancreatic islets in experimental model of uraemia.

BACKGROUND: Uraemia leads to a number of metabolic and hormonal disorders including defective carbohydrate metabolism. Endocannabinoids exert their effect on insulin and glucagon secretion via activation of specific receptors named CB1 and CB2. For this reason and the absence of reports on location and immunoreactivity of CB1, CB2 receptors compared to immunoreactivity of insulin- and glucagon-secreting cells in experimental uraemia, the author decided to investigate this issue. The aim of the present study was the immunohistochemical localisation and evaluation of cannabinoid receptors (CB1,

Current issues in allogeneic islet transplantation.

PURPOSE OF REVIEW: Transplantation of allogenic pancreatic islets is a minimally invasive treatment option to control severe hypoglycemia and dependence on exogenous insulin among type 1 diabetes (T1D) patients. This overview summarizes the current issues and progress in islet transplantation outcomes and research. RECENT FINDINGS: Several clinical trials from North America and other countries have documented the safety and efficacy of clinical islet transplantation for T1D patients with impaired hypoglycemia awareness. A recently completed phase 3 clinical trial allows centres in the United

The islet endothelial cell: a novel contributor to beta cell secretory dysfunction in diabetes.

The pancreatic islet is highly vascularised, with an extensive capillary network. In addition to providing nutrients and oxygen to islet endocrine cells and transporting hormones to the peripheral circulation, islet capillaries (comprised primarily of islet endothelial cells) are an important source of signals that enhance survival and function of the islet beta cell. In type 2 diabetes, and animal models thereof, evidence exists of morphological and functional abnormalities in these islet endothelial cells. In diabetes, islet capillaries are thickened, dilated and fragmented, and islet endoth

Porcine pancreas extracellular matrix as a platform for endocrine pancreas bioengineering.

Emergent technologies of regenerative medicine have the potential to overcome the limitations of organ transplantation by supplying tissues and organs bioengineered in the laboratory. Pancreas bioengineering requires a scaffold that approximates the biochemical, spatial and vascular relationships of the native extracellular matrix (ECM). We describe the generation of a whole organ, three-dimensional pancreas scaffold using acellular porcine pancreas. Imaging studies confirm that our protocol effectively removes cellular material while preserving ECM proteins and the native vascular tree. The s

Pancreatectomy and autologous islet transplantation for painful chronic pancreatitis: indications and outcomes.

Total pancreatectomy with intrahepatic autoislet transplantation (TP/IAT) is a definitive treatment for relentlessly painful chronic pancreatitis. Pain relief is reported to be achieved in approximately 80% of patients. Overall, 30% to 40% achieve insulin independence, and 70% of recipients remain insulin independent for > 2 years, sometimes longer if > 300 000 islets are successfully transplanted. Yet, this approach to chronic pancreatitis is underemphasized in the general medical and surgical literature and vastly underused in the United States. This review emphasizes the history and metabol

Laparoscopic lateral pancreaticojejunostomy for chronic pancreatitis: a case report and review of the literature.

BACKGROUND: Lateral pancreaticojejunostomy (LPJ) is a suitable procedure to treat unremitting abdominal pain of chronic pancreatitis (CP) in the presence of a markedly dilated pancreatic duct containing stones and when an inflammatory mass in the head of the gland is absent; this procedure is rarely performed laparoscopically. CASE REPORT: A 41-year-old woman with alcohol-related calcific CP and pancreatic endocrine and exocrine insufficiency suffered with opiate-dependent abdominal pain that was associated with a dilated pancreatic duct of 8.5 mm containing several calculi, but no mass in th

Effects of intra-abdominal hypertension on the endocrine functions of the pancreas in rats.

BACKGROUND: Intra-abdominal hypertension (IAH) has negative effects on the functions of intra- and extra-abdominal organs and systems. Intra … showed inflammatory cell infiltration in exocrine pancreatic tissue and vascular congestion in the islets of Langerhans. CONCLUSION: IAH causes an increase in glucagon levels and a decrease in insulin levels. These changes may be due to both the direct effects of IAH and the indirect effects of other organs that are affected by IAH.
