Emphysematous cholecystitis after ERCP.

Emphysematous cholecystitis is a rare variant of acute cholecystitis, most frequently seen in elderly, debilitated, or diabetic patients. This report documents the development of fulminant sepsis due to acalculous cholecystitis after endoscopic retrograde cholangiopancreatogram (ERCP) in an otherwise healthy patient with suspected malignant obstructive jaundice. Three other cases of acute cholecystitis have been reported in the literature after ERCP. Although not proven to prevent infectious complications during ERCP, strong consideration should be given to prophylactic antibiotics in patients

[Liver hydatidosis causing obstructive cholangitis : a case report].

The authors report the case of a 41-year-old patient originating from Algeria who developed obstructive cholangitis caused by the membrane of a ruptured hydatid cyst leading to the diagnosis of cystic echinococcosis. Cystic echinococcosis can be asymptomatic for several years until a complication occurs, such as in this case an obstruction of the common bile duct, or cholangio-hydatidosis. This cause of jaundice is uncommon in Western Europe whereas it is more frequent in endemic areas. Identification and treatment of ruptured cysts are mandatory because of the mortality rate of these complica

[Diagnostic value of computed tomography for the detection of invasion of the caudate bile duct branch in carcinoma of the hepatic hilum].

The visualization of the caudate bile duct branch (B1) in computed tomography (CT-scan) with a high dose of contrast medium was evaluated in 71 patients with carcinoma of the liver, biliary tract and pancreas, preoperatively. The patients were classified into four groups: Group A, 22 patients (twelve hepatomas, six gall bladder cancers and four cancers of the pancreas body or tail) without abnormal findings in the biliary tract; Group B, two patients (cancer of the pancreas head and the common bile duct) with obstructive jaundice whose CT-scans were taken before percutaneous transhepatic chola

Acinar Cell Carcinoma of the Pancreas: A Three Case Series and Literature Review.

The pancreas is predominantly composed of acinar cells, yet malignant transformation of these cells is uncommon. Acinar cell carcinoma (ACC) is a rare pancreatic neoplasm, accounting for approximately 1-2% of pancreatic tumours in adults. It commonly arises in the pancreatic head and, unlike pancreatic ductal adenocarcinoma, often presents without jaundice. We report the clinicopathological features of three patients diagnosed with ACC at our institution. Case 1 was a 40-year-old man with recurrent vomiting, epigastric pain, weight loss, early satiety, and a 20-pack-year smoking history. Case

Chronic periaortitis presenting as common bile duct obstruction.

The case of a 67 year old woman is reported who presented with cholestatic jaundice and was found to have, in addition, an inflammatory abdominal aortic aneurysm. Only at necropsy did histopathology show chronic periaortitis as the aetiology of a pancreatic head mass which, during life, mimicked a pancreatic neoplasm obstructing the bile and pancreatic ducts.

Sporadic ampullary adenoma mimicking adenocarcinoma in the absence of genetic predisposition: a case report.

Ampullary tumors are uncommon and are frequently associated with hereditary cancer syndromes. Sporadic ampullary adenomas are infrequently reported and may mimic adenocarcinoma clinically and radiographically. We present a 71-year-old male with obstructive jaundice and imaging concerning for malignancy who was found to have a high-grade ampullary adenoma following attempted pancreaticoduodenectomy. Operative management required modification of the standard approach due to intraoperative anatomical distortions. This case highlights the diagnostic challenges in differentiating benign from malign

B-Cell Lymphoma Presenting as a Pancreatic Mass.

Non-Hodgkin lymphoma (NHL) has many subtypes, with diffuse large B-cell lymphoma being the most common subtype. Typical symptoms of NHL include fever, weight loss, night sweats, and lymphadenopathy. Treatment depends on the subtype of NHL. Our case presents the clinical course of a patient who presented with obstructive jaundice and pancreatitis rather than typical symptoms of NHL. Workup included abdominal imaging showing a pancreatic mass and biopsy ultimately revealed high-grade B-cell lymphoma. This case presents a rare diagnosis of likely primary pancreatic lymphoma.

Hemobilia Derived from Cystic Artery Pseudoaneurysm.

Cystic artery pseudoaneurysm (CAP) is a rare disease, with small number of previous reports related to CAP. Besides, it is frequently prone to critical condition due to arterial bleeding. Here, we presented a case of ruptured CAP with acute calculus cholecystitis and its subsequent successful management with temporary endoscopic biliary drainage for obstructive jaundice and embolization for the culprit artery without cholecystectomy. Since CAP is at high risk of bleeding, intravascular treatment, which is only one currently available therapeutic option, is urgently required in the clinical sit

Biliary stenting with puncturing technique using Baron aspiration needle in ulcerative papillary carcinoma.

A 60 year-old man with obstructive jaundice due to an ulcerative papillary carcinoma underwent endoscopic retrograde cholangiopancreatography. Owing to the totally destroyed papilla of Vater, access to the biliary tract was impossible. Instead of needle knife fistulotomy, puncturing with an 18-G aspiration needle on the supra-papillary bulge followed by biliary stenting was performed successfully. To our knowledge, this technique has not been reported before in the English literature and probably can have fewer complications of bleeding and perforations produced by needle knife fistulotomy.

A case of pancreatoduodenectomy as resolutive treatment for a solitary metastasis of breast cancer.

A 49-year-old woman was hospitalized for obstructive jaundice 4 years after extended mastectomy for lobular infiltrating carcinoma. After routine investigations that detected a malignant lesion of the pancreas head, the patient underwent pancreatoduodenectomy. The surgical specimen revealed unexpected breast carcinoma metastatic to the pancreas and duodenum wall. The woman is alive and free of disease at 10 years from the mastectomy and 6 years from the pancreatoduodenectomy. This experience confirms the validity of a surgical radical treatment for metastatic lesions in selected cases.

[Spontaneous migration of large stones from the bile duct (author's transl)].

Spontaneous migration of bile-duct stones through the papilla into the intestine was directly observed in two patients with choledocholithiasis. The mechanism of such spontaneous migration lies in the development of a pressure necrosis at the papilla causing a tear in its pore. Spontaneous migration of stone from the common bile duct can be assumed when endoscopic retrograde cholangiopancreatography reveals a balloon-shaped swollen papilla with a torn pore, and previously observed obstructive jaundice regresses in the presence of stone-free, though perhaps somewhat dilated, bile passages.

Unnoticed for 14 years: an asymptomatic forgotten common bile duct stent - a case report.

INTRODUCTION: Choledocholithiasis is the presence of gallstones within the common bile duct (CBD), affecting 10-20% of gallbladder stone patients. Endoscopic retrograde cholangiopancreatography (ERCP) with stent insertion is a common treatment of CBD obstruction and stenosis to maintain duct patency, with stents typically replaced within 3-6 months. However, prolonged stent presence can lead to complications such as pancreatitis and cholangitis. We report a rare case of a neglected CBD stent left in situ for 14 years without complications. CASE PRESENTATION: A 77-year-old female with controll

Intrabiliary pressure in the pathophysiology of extra hepatic biliary obstruction.

BACKGROUND AND AIM: The aim of this study was to evaluate the role of intrabiliary pressure (IBP) in the pathophysiology of extrahepatic biliary obstruction (EHBO) during percutaneous transhepatic biliary drainage (PTBD). MATERIALS AND METHODS: Adult patients with EHBO who underwent PTBD were prospectively enrolled. IBP was recorded during primary PTBD. The parameters of interest were age, gender, etiology of EHBO, baseline and post-PTBD liver function tests, duration for resolution of jaundice (decrease in total serum bilirubin ≥30% of baseline or <2 mg/dL), cholangitis, bile cultures, and s

The effect of preoperative biliary drainage on postoperative complications of pancreaticoduodenectomy: a triple center retrospective study.

BACKGROUND: Biliary obstruction which is a major complication of pancreas and periampullary tumors could result in cholangitis, coagulopathies, gastrointestinal symptoms, and impaired wound healing. Pancreaticoduodenectomy (PD) is still the standard approach for pancreas resection and imposes high risk of morbidity and mortality to patients. To reduce the high risk of PD and address the biliary obstruction, the use of preoperative biliary stenting was increased. However, available literature doubts its efficiency. METHODS: A total of 147 patients who underwent PD between September 2012, and F

Unusual manifestations of pancreatic pseudocysts and their surgical management.

Six cases representing selected complications of pancreatic pseudocyst are reported and their surgical management is discussed. Patients with mediastinal extension of a pseudocyst frequently present with symptoms in the chest rather than in the abdomen. Chronic recurrent pleural effusion, rich in protein and amylase, often accompanies mediastinal extension of a pancreatic pseudocyst. It is important to recognize that such an effusion almost certainly represents disruption of the pancreatic duct with formation of a pancreatic pseudocyst or a pancreaticopleural fistula. Internal drainage from be

[Pathogenesis of jaundice in acute cholecystitis].

An investigation of specific course of the disease in 911 patients operated upon for acute cholecystitis with bilirubinemia has shown that mechanical jaundice resulting from choledocholithiasis takes place in a third of the patients. Obstruction of the bile duct was confirmed in 27.1% of the patients during cholangiography. Prevalence of a number of factors was noted indicating of a toxic lesion of the liver (destructive forms of acute cholecystitis in 81.0% of the patients, higher level of bilirubinemia in long terms of the disease, the presence of coexistent pancreatitis in 30.5%, cholangiti

A case report of acute liver failure caused by biliary ascariasis in children.

Most patients with biliary ascariasis present with acute abdominal pain and biliary colic. Typically, conservative treatment involving deworming agents is employed. However, for those who do not respond to pharmacological therapy, endoscopic or surgical intervention becomes necessary. Generally, the mortality rate is low, and the prognosis is favorable. This case describes a pediatric patient exhibiting symptoms of intestinal disease, including upper abdominal pain and jaundice, resulting from extrahepatic biliary tract obstruction caused by roundworms. This condition subsequently led to compl

An uncommon cause of biliary obstruction (Mirizzi syndrome): report of five cases.

Mirizzi syndrome is a rare form of common hepatic duct obstruction resulting from an inflammatory response secondary to a gallstone impacted in the cystic duct or neck of the gallbladder. Herein, we report five patients with this syndrome. Clinically, all patients had prominent jaundice. Ultrasound examination showed a large stone in the neck of the gallbladder. Endoscopic retrograde cholangiopancreaticography demonstrated a filling defect in the biliary tract the cystic duct level. Four patients possessed cholecystobiliary fistulas. Two patients also had common bile duct stones. Operations in

Rapunzel Syndrome: A Rare Case of Small Bowel Intussusception in a Child.

Trichobezoars are indigestible masses of ingested hair commonly found in the stomach, often presenting with symptoms related to gastric outlet obstruction and severity related to the mass's size and location. Gastrointestinal complications include ulceration, perforation, peritonitis, pancreatitis, obstructive jaundice, pneumatosis intestinalis, and intussusception. Management of trichobezoars differs from that of other forms of bezoars, which can often be addressed with chemical dissolution. Trichobezoars are high-density structures that are also resistant to enzymatic and pharmacotherapy deg

MR cholangiopancreatography in malignant biliary obstruction.

Malignant lesions of the biliary tract are a frequent occurrence, typically presenting with clinical findings of obstructive jaundice. The authors discuss the role of MR cholangiopancreatography (MRCP) as a second level diagnostic technique, which can provide information regarding not only the location, but also the cause of the obstruction. This can be obtained if MRCP is considered as part of a complete study of the upper abdomen, with acquisition of T1- and T2-weighted images. The "all-in-one" approach may provide the identification, characterization, and staging of the lesion, giving the c
