[Flaps for soft tissue defect closure in the distal lower leg].

Soft tissue defects in the distal lower leg region are challenging to treat, especially in trauma cases. To achieve early closure of the defect, pediculated adipofascial or muscle flaps can be used as well as free flaps. The pediculated adipofascial suralis flap has a reliable blood supply and a broad radius so this flap can be used for almost every defect location on the distal lower leg except for defects larger than 10 × 10 cm. The donor site defect does not lead to major problems and is well tolerated. The soleus flap can cover defects in the middle third and proximal distal third of the l

Treatment of lower extremity injuries with orthotic shoe inserts. An overview.

Orthotic shoe inserts are very effective in providing symptomatic relief of lower extremity complaints in running athletes. Inserts adjust the biomechanical-variables associated with running injuries and reduce the effect of high stresses produced by running activities. Orthotic treatment is based on an understanding of complex coupling of rotation of the lower extremity with pronation and supination of the subtalar joint. Orthotic fabrication is initiated by determining the neutral position of the foot and obtaining an accurate cast of this position. Successful treatment with orthotic shoe in

[Preventive antibiotic treatment in cases of complicated injuries of the extremities. A questionnaire study].

A questionnaire about prophylactic antibiotic treatment of open bone, tendon or joint lesions in the extremities was sent to the 83 casualty departments in Denmark. The questionnaire was answered by 56 casualty departments which treated 72% of the orthopaedic surgical injuries in Denmark. In cases of lesions of the hands and feet, 23% of the patients were treated in casualty departments where prophylactic antibiotic therapy was rarely employed while 24% were treated in casualty departments where prophylactic antibiotic therapy was always employed. In cases of lesions proximal to the wrist or a

The pathology and prevention of Volkmann's ischaemic contracture.

Ischaemia is a rare complication of injury to a limb but must be excluded in every case. Two distinct types occur: Type I, where a proximal arterial injury gives rise to ischaemia distally; and Type II, where a direct injury gives rise to ischaemia at the site of the injury. Whatever the nature of the insult, an ischaemic contracture only develops as a result of swelling of the soft tissues where these soft tissues are contained in un unyielding osteofascial compartment. This secondary ischaemia can only be relieved by a timely fasciotomy. The diagnosis of ischaemia in an injured limb and the

Influence of posterior tibial slope on sagittal knee alignment with comparing contralateral knees of anterior cruciate ligament injured patients to healthy knees.

Posterior tibial slope (PTS) has been known to contribute to anterior-posterior knee stability and play an essential biomechanical role in knee kinematics. This study aimed to investigate the effect of PTS on single-leg standing sagittal knee alignment of the intact knee. This study included 100 patients with unilateral ACL injury knee (ACL injury group, 53 patients) or with the normal knee (control group, 47 patients). The single-leg standing sagittal alignment of the unaffected knees of the ACL injury group and normal knees of the control group were assessed radiographically with the followi

The Effect of Strength Training Targeting Medial Thigh Muscles on Neuromuscular and Biomechanical Risk Factors for Anterior Cruciate Ligament Injury: A Randomized Controlled Trial.

BACKGROUND: Knee valgus loading is thought to be an important contributor to noncontact anterior cruciate ligament (ACL) injuries, but the effects of training programs focusing on decreasing knee valgus loading on lower extremity biomechanics with respect to ACL injury risk remain unclear. Thus, this study aimed to examine the effect of strength training designed to strengthen the medial thigh muscles on lower extremity joint kinematics, kinetics and muscle activity during single-leg landing. METHODS: A total of 35 healthy participants randomly conducted either exercises targeting medial thig

Kinesiophobia Associates With Physical Performance in Patients With ACL Reconstruction: A Critically Appraised Topic.

CLINICAL SCENARIO: Anterior cruciate ligament (ACL) injuries remain one of the most common and debilitating injuries that physically active individuals experience. Pain-related fear of movement and/or reinjury, or kinesiophobia, is the most frequently cited reason for not returning to sport after ACL reconstruction. Understanding how kinesiophobia may impact recovery of physical performance is essential to guide targeted rehabilitation. CLINICAL QUESTION: Does kinesiophobia associate with physical performance in patients with a history of ACL reconstruction? SUMMARY OF KEY FINDINGS: Five cro
