[Epidemiology of lip-maxilla-palate clefts in the canton of Vaud].

Epidemiological variables in oral clefts were examined in a cohort of patients with congenital malformations from the canton of Vaud in Switzerland during the years 1990-1999. Among 77'259 newborns or foetuses, 144 cases of oral clefts were identified in the Registre Vaudois des Anomalies Congénitales (RVAC), yielding a total prevalence of 18.6 per 10'000. These clefts consisted of 84 (58%) cases of cleft lip with or without cleft palate (CL/P) and 60 (42%) cases of cleft palate only (CP). Associated anomalies were found in 45% of cases. Our results are compared with previous reports in other …

An anthropometric analysis of indices of severity in the unilateral cleft lip.

The aim of this study was to analyze the relationships among three key anthropometric parameters in the unilateral cleft lip to determine the correlations, if any, among these indices of severity. Using a standardized anthropometric documentation protocol, preoperative measurements of 125 unilateral cleft lips (103 complete and 22 incomplete) were performed under general anesthesia by a single surgeon at the time of primary lip repair at the age of 3 months. The following key measurements were analyzed statistically: (1) the philtral height difference (PHD) between the cleft and noncleft sides…

[Corrective rhinoplasty in a patient with cleft lip nose deformity].

The author presents a case of secondary rhinoplasty in patient with cleft lip nose deformity. The preoperative computer planning and the stages of the surgery were observed. Good functional and cosmetic postoperative results were achieved by means of bone-cartilage autograft--a fragment taken from nasal dorsum and implanted in the nasal tip region. There is almost complete concurrence between the preoperative computer modeling and the good postoperative results.

An investigation of the dentition of parents of children with cleft lip and palate.

OBJECTIVE: The aim of this investigation was to identify whether any dental features in parents of children with cleft lip and palate can be useful as predictors of clefting. METHODS: The dentition of 28 pairs of parents of children with complete unilateral (17) and bilateral (11) cleft lip and palate and 21 pairs of parents of noncleft children were evaluated. Clinical and radiographic examinations were carried out to identify abnormalities of tooth number and morphology. Study cast assessment was undertaken to evaluate incisor relationship, overjet, overbite, intercanine widths, and mesiodi…

Use of the neisserial lipoprotein (Lip) for subtyping Neisseria gonorrhoeae.

The pathogenic Neisseria species N. meningitidis and N. gonorrhoeae possess an outer membrane lipoprotein, designated Lip, which is present in all strains tested. The predicted protein sequence of Lip consists of a consensus AAEAP amino acid repeat. The objective of this study was to determine the feasibility of using the Lip repeat number and sequence for subtyping of Neisseria gonorrhoeae. The lip genes of each isolate were amplified by PCR and sequenced to determine the repeat number and sequence. Among the 46 strains we examined, eight different Lip repeat numbers were identified, with len…

A review of common mucocutaneous disorders affecting the mouth and lips.

Most oral involvement in the mucocutaneous diseases is related mainly to disorders with a prominent immunological component, especially lichen planus and pemphigoid. However, less common conditions such as pemphigus, dermatitis herpetiformis, linear IgA disease, erythema multiforme, chronic ulcerative stomatitis and other conditions may need to be excluded. Diagnosis is initially clinical, based on a full history, general, and oral examination but precise diagnosis can be difficult without further data and therefore biopsy and immunological studies are invariably required to confirm the specif

[Improved treatment of cleft lip and palate is now available].

When cleft lip and palate treatment was introduced at Gothenburg in 1957, the procedure used was early bone grafting (EBG). By 1965, EBG had been omitted from the regimen, bone grafting being postponed until the appearance of mixed dentition. Analysis of the results of both techniques showed maxillary retrusion of different degrees. Accordingly, this routine was abandoned in 1975, being replaced by a procedure which is characterised by delayed closure of the hard palate (DCHP). Thus, the surgical procedure comprised the following steps: 1, lip closure at 1-2 months of age; 2, soft palate repai…

Lip and tongue function in multiple sclerosis: A physiological analysis.

A physiological analysis of the articulatory function of 16 adults with multiple sclerosis (MS) was performed using lip and tongue transduction systems. Sixteen nonneurologically impaired adults, matched for age, gender, and education, served as controls. The MS speakers demonstrated patterns of tongue function that were significantly different from those of the control speakers. Specifically, the MS speakers were found to have significantly reduced tongue strength, endurance, and rate of repetitive movements. In addition, preclinical signs of lingual dysfunction were evident in nondysarthric …

A peculiar malformation of the upper lip: report of six cases.

Six cases of a peculiar type of upper lip malformation have been observed by the author during the last 40 years. The malformations in this case series were remarkably similar, suggesting a single teratologic entity not been reported before. The six patients included 1-, 11-, 4-, and 6-month-old males, a 20 year-old male, and an 11-year-old female. Each was characterized by a crooked folded philtrum, a flat thin vermilion region of the upper lip, and high-positioned ala(e), that was unilateral or bilateral. The upper median incisors were separated occasionally by the thick lip frenulum without…

Results of a modified staircase technique for reconstruction of the lower lip.

Our experience with a modified staircase technique for closure of lower lip defects is reported. The procedure is based on the original technique of Johanson et al. (1974). However, the integrity of the orbicularis oris muscle is respected when advancing lower lip flaps. Twenty patients with squamous cell carcinoma of the lower lip were treated using this modified reconstruction technique. The size of the defects ranged from 30-60% of lower lip width. No recurrences were observed during a 3-year to 5-year follow-up. All patients showed symmetrical lip movement, an adequate buccal sulcus and in…

Dental findings in parents of children with cleft lip and palate.

The incidence of dental abnormalities in the cleft lip and palate population has been reported to be much higher than in the normal population. The role of genes in the production of a cleft lip and palate, and dental anomalies is thought to be complex, with autosomal dominant, recessive, and x-linked genes all playing a role. Noncleft parents can carry some of the cleft lip and palate genes, which produce clinically subtle manifestations in their facial skeleton. The purpose of this study was to look for evidence of increased dental anomalies in the non-cleft parents of cleft lip and palate c…

[Reconstructive surgery of the upper lip with esthetic-functional recovery].

Two cases are reported of reconstructive surgery of the upper lip in patients who had squamous cell carcinoma, using the Bürrow technique. This technique is more used in lower lips because it does not involve esthetic unity. We have opted for it in the upper lip also, using a variation in the lip commissure incision, getting a significant functional and esthetic result.

Abnormalities of the maxillary incisors in children with cleft lip and palate.

Dental anomalies of the maxillary anterior teeth were studied in seventy-seven children affected by unilateral and bilateral clefts of the lip and alveolar process, with or without involvement of the palate. As for the permanent lateral incisor in the cleft area, our results show that its congenital absence is the most frequent abnormality followed by anomalies in size and shape and supernumerary teeth. Enamel hypoplasia was found to affect the permanent central incisor on the cleft side more frequently. Early recognition of tooth abnormalities during the primary dentition phase for an interce…

[Artery supply of the lip and palate in normal and cleft patients].

50 normal and 10 cleft lip and palate head and neck specimens were studied by dissection and arteriography. The results are as follows: 1. In unilateral complete cleft lip, the superior labial artery runs along the labial margin and cleft edge, ending in base of nasal wing on the lateral side. In the medial side, upper labial artery ends in the base of the columella. 2. In the bilateral complete cleft lip, artery supply of superior lip is similar to the lateral side of unilateral complete cleft lip. The artery supplying the philtrum and premaxilla comes from the posterior septal artery, anteri…

Lifting of the upper lip using a single extensive incision.

In this report an operation to treat ageing of the upper lip is described, whereby a single strip of skin extending from one labial commissure, along the juxta-nasal part of the lip, to the other labial commissure is excised. Good results were obtained with this method in 12 patients operated on, and these results are presented.
