Surgical Resection and Reconstruction of Ameloblastoma: A 13-Year Retrospective Review.

BACKGROUND: Ameloblastoma is a locally aggressive, benign tumor presenting in the maxilla and mandible prone to recurrence. Resection greatly limits recurrence; however, reconstruction becomes critical to preserve patients' functionality and esthetics. PURPOSE: The aim of this study was to describe surgical resection and reconstructive approaches in the treatment of ameloblastoma and compare clinical outcomes to conservative methods of treatment. STUDY DESIGN, SETTING, SAMPLE: A retrospective case series was completed through analysis of patient records. The study population was composed of

Peripheral primitive neuroectodermal tumor in masseter muscle.

Primitive neuroectodermal tumor is a member of malignant small round cell tumors. These tumors especially originate from the central and autonomous nervous system. However, these tumors may be originated from peripheral tissues and are called peripheral primitive neuroectodermal tumor. A 14-year-old girl attended to the Ear Nose Throat Clinic with the complaint of progressive painless swelling mass for 2 months on the right side of the face. Neck magnetic resonance imaging showed 3.5 × 2.5 × 2-cm isointense mass on T1 and hyperintense on T2 sequences. There was no pathological lymphadenopathy

Facial lymphoedema as an indicator of terminal disease in oral HIV-associated Kaposi sarcoma.

Rapidly progressive facial lymphoedema developing concurrently with, or immediately after rapid enlargment of oral Kaposi sarcoma (KS) in HIV-seropositive highly active antiretroviral treatment (HAART)-naïve subjects, foretokens death. We present here an unusual case of HIV-KS in an 11-year-old HIV-seropositive HAART-naïve boy. Our patient's KS disease had had a fulminant course characterised by rapidly progressing oral HIV-KS, resorption of the mandibular alveolar bone process beneath some of the HIV-KS lesions, and rapidly progressive facial lymphoedema. He died 3 weeks after the onset of fa

[Ghost cell odontogenic carcinoma of the mandible].

INTRODUCTION: Ghost cell odontogenic carcinoma is a rare malignant tumor that occurs both in the mandible and maxilla. It has a variable non-specific clinical and radiological appearance. The authors report a mandibular case. CASE REPORT: A 89-year-old man consulted for swelling of the left lower mandible and gums having developed over the previous 8 months. The lesion was 6cm long, ulcerative and budding, bleeding on contact. Radiological assessment revealed blurred contour osteolysis with extension to the mouth floor and sub-mandibular compartment. Histological examination of the surgical p

Oral squamous cell carcinoma associated with symphyseal dental implants: an unusual case report.

BACKGROUND: The development of squamous cell carcinoma (SCCa) around dental implants is an uncommon pathological manifestation. This case report describes a patient with history of oral lichen planus (OLP) and previous SCCa of the gingiva who developed SCCa adjacent to symphyseal implants. CASE DESCRIPTION: An 81-year-old edentulous woman with history of OLP developed an in situ SCCa on the left mandibular edentulous ridge. One of the authors, an oral and maxillofacial surgeon, performed a marginal mandibular resection of the lesion. Functional oral rehabilitation was achieved by means of two

Lymphoepithelioma-like carcinoma of oral cavity: report of three cases and literature review.

Lymphoepithelioma-like carcinoma is a rare tumour in the oral cavity and is characterized histologically by non-keratinizing, undifferentiated squamous cell carcinoma with lymphocytic infiltration. Three consecutive cases of intraoral lymphoepithelioma-like carcinoma are reported. A review of the literature reveals a similar biological behaviour to that of nasopharyngeal lymphoepithelioma: a high incidence of cervical nodal spread and remarkable radiosensitivity. Chemotherapy should be considered when nodal or distant metastases are present. The association of the Epstein-Barr virus with this

Intravascular disruption of central venous ports.

Intravasal catheter disruption is a rare complication of central venous ports. Main causes are chronic trauma due to compression of the catheter between clavicle and first rib, or primary damage by sharp instruments during insertion. Utilizing the subclavian route, a more lateral insertion can minimize the risk of catheter compression. Regular postoperative X-ray controls can help to recognize progressive catheter compression. In the case of significant compression, early replacement is advisable to avoid disruption. Percutaneous transvenous snare technique is the therapy of choice to retrieve

Oral complaints caused from metastases to the mandible and maxilla.

Jaw bone disorders causing oral complaints are common in primary care settings. Most of these conditions are of a chronic and benign nature. However they also may be the symptoms of a primary or secondary malignant process in the bone. The most common malignant bone tumor is metastatic carcinoma, and tumors arising in the breast, prostate, thyroid, lung and kidney have a special propensity to spread to bone. Yet metastases to the bones are rare; less than one per cent of all neoplasms metastases to the maxillofacial area. We describe four cases of metastatic tumours to the jaws. Two cases orig

[Clinical application of free microvascular flaps in reconstructive surgery of the oral cavity].

We present the results of reconstructive surgery after different surgical resections in the oral cavity. Reconstruction of the defects was made using free microvascularized flaps. We used a free radial fasciocutaneous or osteocutaneous flap in every case, with a 100% of viability and a low complication rate. This technique is widely versatile in the reconstruction of different oral defects, offering an adequate replacement of oral mucosa.

Unilateral skin lesions associated with multiple neoplasms.

A girl with unilateral partly depigmented, partly hyperpigmented atrophic skin lesions, hypertrichosis, postauricular nodules, cataracts, and mild mental retardation is presented. Additionally, she developed multiple neoplasms as optic glaucoma, a psammomatous meningioma and an ameloblastoma. It remains unclear whether our patient suffers from a variant of the epidermal nevus syndrome or a new neuro-ectodermal dysplasia.

Osteomyocutaneous fibular flap harvesting: Computer-assisted planning of perforator vessels using Computed Tomographic Angiography scan and cutting guide.

INTRODUCTION: Mandibular reconstruction performed after virtual planning has become more common during recent years. The gold standard for extensive mandibular reconstruction is of course a fibular free flap. In designing an osteomyocutaneous fibula flap, poor planning, aberrant anatomy and/or inadequate perforator vessels are the most frequent causes of complications and may force the surgeon to modify the flap design, explore the contralateral leg or harvest an additional microvascular flap. The goal of our study was to pre-operatively evaluate the vascular anatomy of the fibula and localize

Venous graft as temporary arteriovenous shunt in free tissue transfer.

A new method of interpositional venous graft in microvascular tissue transfer is described. The interpositional vein is transferred to the vessels of the recipient site primarily, forming an arteriovenous shunt. Later, when the microanastomosis to the flap is created, the shunt is divided into an arterial branch and a venous branch. This method reduces the ischemic time of the free flap and allows the use of interpositional grafts of optimal length.

Gardner's syndrome: a clinical and genetic study of a family.

OBJECTIVE: Gardner syndrome (GS) is an autosomal dominant genetic disorder with almost complete penetrance (80%) and variable expression. GS is a variant of familial adenomatous polyposis and characterized by extracolonic manifestations including osteomas and soft tissue tumors (desmoid tumors, epidermoid cysts). We describe clinical and surgical approaches in a family in which the genetic disorder was diagnosed in 3 generations. STUDY DESIGN: The studied family underwent clinical history and instrumental and genomic studies. Two members of this family, affected with GS, underwent surgery for
