Developing effective delivery systems for Chlamydia vaccines.

Members of the genus Chlamydia cause a plethora of ocular, genital and respiratory diseases, with severe complications, such as blinding trachoma, pelvic inflammatory disease, ectopic pregnancy and tubal factor infertility, interstitial pneumonia, and chronic diseases that may include atherosclerosis, multiple sclerosis, adult-onset asthma and Alzheimer's disease. The current medical opinion is that an effective prophylactic vaccine would constitute the best approach to protect the human population from the most severe consequences of these infections. There are three essential and mutually in…

Machine learning based model for the detection of multiple sclerosis from OCT-derived macular and optic disc retinal biomarkers.

BACKGROUND: Multiple sclerosis (MS) is a chronic neurological disease characterized by inflammatory demyelination and progressive neuroaxonal damage. Retinal layer thickness in the macular and optic disc regions, measurable using optical coherence tomography (OCT), might be considered as a promising non-invasive biomarker for detecting MS-related neurodegeneration. METHODS: In this study, we present a novel AI framework leveraging macular ganglion cell-inner plexiform layer (GCL-IPL) and Retinal Nerve Fiber Layer (RNFL) thickness across sectors (covering [Formula: see text]) and peripapillary…

10-Weeks of resistance training improves sleep quality and cardiac autonomic control in persons with multiple sclerosis.

PURPOSE: To examine the acute and chronic effects of 10-weeks of progressive resistance training on sleep quality and sleeping heart rate variability in persons with Multiple Sclerosis (pwMS). METHODS: Eighteen pwMS (age = 44.8 ± 10.6 years; EDSS = 3.1 ± 1.7) completed a 10-week of resistance training, with three training sessions per week. Each session consisted of 4 lower body exercises, performing 2-4 sets of each exercise, with 8-15 repetitions each set, at an intensity ranging from 60 to 75% of 1-repetition maximum. Subjective and actigraphic sleep quality and sleeping heart rate variabi…

Mitochondrial bioenergetic remodeling underlies fingolimod-induced immunometabolic adaptation in multiple sclerosis.

Mitochondrial dysfunction and immune cell metabolic reprogramming are central mechanisms driving the pathogenesis and progression of multiple sclerosis (MS). Fingolimod (FTY720), a sphingosine-1-phosphate analog widely used in MS, limits lymphocyte egress from lymphoid organs; however, its effects on mitochondrial function and immunometabolic pathways remain incompletely understood. We evaluated mitochondrial bioenergetics and metabolic reprogramming in lymphocytes from healthy controls incubated with FTY720 and from MS patients treated with fingolimod, compared with healthy controls and patie…

Cuprizone-mediated demyelination reversibly degrades voiding behavior in mice while sparing brainstem reflex.

Multiple sclerosis (MS) is a chronic, progressively debilitating demyelinating disease of the central nervous system (CNS). Nearly 80% of MS patients experience lower urinary tract dysfunction early in their diagnosis. This significantly affects the quality of life, and in latter stages of disease is a leading cause of hospitalization. Previously, animal models have shown that inflammatory demyelination in the CNS causes profound bladder dysfunction, but the confounding influence of systemic inflammation limits the potential interpretation of the contribution of CNS demyelination to bladder dy…

Low dose dextromethorphan attenuates moderate experimental autoimmune encephalomyelitis by inhibiting NOX2 and reducing peripheral immune cells infiltration in the spinal cord.

Dextromethorphan (DM) is a dextrorotary morphinan and a widely used component of cough medicine. Relatively high doses of DM in combination with quinidine are used for the treatment of mood disorders for patients with multiple sclerosis (MS). However, at lower doses, morphinans exert anti-inflammatory activities through the inhibition of NOX2-dependent superoxide production in activated microglia. Here we investigated the effects of high (10 mg/kg, i.p., "DM-10") and low (0.1 mg/kg, i.p., "DM-0.1") doses of DM on the development and progression of mouse experimental autoimmune encephalomyeliti…

T Helper Cells and Cytokine Networks in the Immunopathogenesis of Multiple Sclerosis.

Multiple sclerosis (MS) is a chronic autoimmune disorder of CNS with demyelination, neurodegeneration and compartmentalized inflammatory disorder. Excessive T-helper cell (CD4+) activation and unregulated cytokine signaling play a key role in its onset and progression. These changes impair communication between peripheral immune cells and CNS resident microglia, astrocytes and oligodendrocytes. This review provides an overview on the contribution of specific subsets of T-helper cells to MS pathology/immunity. Th1 cells release interferon-γ and lymphotoxin, that stimulate activation of myeloid …

Under the microscope: focus on Chlamydia pneumoniae infection and multiple sclerosis.

Multiple Sclerosis (MS) is a chronic inflammatory demyelinating disease of the Central Nervous System (CNS) of supposed autoimmune origin whose etiology still remains unknown. Epidemiological studies suggest that MS pathogenesis could be related to an infection superimposed on a predisposing genetic background. However, at present, no direct evidence for an infectious implication in MS autoimmunity exists. Recently, the potential role of Chlamydia pneumoniae as a causative agent of MS has received increasing attention. After the initial high rates reported for molecular and culture demonstrati…

Sphingosine-1-phosphate (S1P) signaling as a novel therapeutic target for alcohol abuse.

Sphingosine-1-phosphate (S1P) is a lipid mediator signaling through broadly expressed G protein-coupled receptors. We found that S1P is regulated by alcohol and that S1P receptor agonists reduce alcohol drinking in rodent models. Specifically, we observed that two S1P receptor agonists FDA-approved for multiple sclerosis, fingolimod and ozanimod, and the more brain penetrant S1P1 receptor agonist CYM5442, reduced binge alcohol drinking in the drinking in the dark (DID) paradigm in mice. CYM5442 also reduced drinking in dependent mice in the chronic intermittent ethanol vapor paradigm of depend…

Neuroprotective Role of Cannabinoid CB1 and GPR55 Receptors in a Cell Model of Multiple Sclerosis.

Multiple sclerosis (MS) is a chronic neuroinflammatory disorder characterized by demyelination, oligodendrocyte (OLG) loss, and progressive neurological decline. While current disease-modifying therapies primarily target immune responses, they offer limited neuroprotective or remyelinating effects. Through its receptors, including cannabinoid receptor type 1 (CB1R) and the G protein-coupled receptor GPR55, the endocannabinoid system (ECS) has emerged as a promising therapeutic target due to its roles in neuronal survival and glial function. To better study the functional role of these receptor…

Predictors of Response to Multiple Sclerosis Therapeutics in Individual Patients.

Multiple sclerosis (MS) is a chronic inflammatory disorder of the central nervous system. Several disease-modifying therapies have been shown to ameliorate the disease course; however, the individual treatment response and the occurrence of adverse events remain highly unpredictable. In the last 2 decades, a multitude of studies have aimed to identify biomarkers that enable treatment allocation in the individual patient or subgroup of patients with regard to treatment efficacy and safety profile. Following a PubMed database search, we provide an overview on what is presently known about body f…

Natalizumab exerts direct signaling capacity and supports a pro-inflammatory phenotype in some patients with multiple sclerosis.

Natalizumab is a recombinant monoclonal antibody raised against integrin alpha-4 (CD49d). It is approved for the treatment of patients with multiple sclerosis (MS), a chronic inflammatory autoimmune disease of the CNS. While having shown high therapeutic efficacy, treatment by natalizumab has been linked to progressive multifocal leukoencephalopathy (PML) as a serious adverse effect. Furthermore, drug cessation sometimes induces rebound disease activity of unknown etiology. Here we investigated whether binding of this adhesion-blocking antibody to T lymphocytes could modulate their phenotype b…

Antibody reactivity to individual structural proteins of measles virus in the CSF of SSPE and MS patients.

BACKGROUND: Chronic progressive disorders of the central nervous system (CNS) impose diagnostic problems, particularly in younger patients. The demonstration of antibodies against measles virus (MV) in the cerebrospinal fluid (CSF) plays a major role in the laboratory diagnosis of subacute sclerosing panencephalitis (SSPE) as well as multiple sclerosis (MS). OBJECTIVES: Because intrathecally synthesized antibodies against MV can be found in both diseases, it is necessary to establish easy and reliable methods to improve the differential diagnosis. STUDY DESIGN: Seventy-one paired serum/CSF s…
