Assessment of Neuro-Musculo-Vascular Activity in Hemiplegic Shoulder Pain: A Multimodal Study With HD-sEMG, Ultrasonography, and Laser Speckle Imaging.

Hemiplegic shoulder pain (HSP) is a common complication following stroke, significantly affecting upper limb recovery and quality of life. However, the underlying pathophysiological mechanisms of HSP remain poorly understood, which poses a major obstacle to the development of effective therapeutic strategies. This study aims to investigate the underlying mechanism of HSP by evaluating neuro-musculo-vascular activities using high-density surface electromyography (HD-sEMG), musculoskeletal ultrasound, and laser speckle contrast imaging (LSCI). A total of 12 HSP patients and 12 hemiplegic control

Avascular Necrosis of the Hip after the COVID-19 Pandemic.

BACKGROUND AND OBJECTIVES: It is clear that COVID-19 can have a wide range of effects on the body, and the musculoskeletal system is no exception. Studies are ongoing to better understand how the virus may impact the hip bone and joint tissue. However, it is known that COVID-19 can cause inflammation and damage to various parts of the body, including the bones and joints, which could lead to pain, stiffness, and other symptoms. MATERIALS AND METHODS: A retrospective analysis is done with ten patients admitted to the hospital due to COVID-19 and later complained of hip pain. Magnetic resonance

Identification of Novel Co-Occurring ZNF711 and SRCAP Variants in a Child with Neurodevelopmental Disorders Through Exome Sequencing.

BACKGROUND: Global developmental delay (GDD)/intellectual disability (ID) represent a group of neurodevelopmental disorders (NDDs) with onset in childhood with a global prevalence of 1-3%. The etiology of GDD/ID is complex, with genetic factors accounting for about half of cases. Exome sequencing (ES) has proven to be an effective diagnostic tool for genetically heterogeneous disorders such as GDD/ID. This study aims to identify the genetic etiologies using ES in a Chinese patient with NDD. METHODS: Clinical data were collected through physical examination. Trio-ES was conducted to investigat

M2 macrophage-derived exosomal miR-26b-5p regulates macrophage polarization and chondrocyte hypertrophy by targeting TLR3 and COL10A1 to alleviate osteoarthritis.

Osteoarthritis (OA) is one of the most prevalent chronic musculoskeletal diseases among the elderly population. In this study, macrophage-derived exosomes were isolated and identified. Exosomes were subjected to microRNA (miRNA) sequencing and bioinformatic analysis, and differentially expressed miRNAs were verified. miR-26b-5p target genes were confirmed through target-site mutation combined with a dual-luciferase reporter assay. The effects of miR-26b-5p on macrophage polarization and chondrocyte hypertrophy were assessed in vitro. miR-26b-5p agomir was applied to mice with OA induced by ant

Foot, Toe, and Nail Changes: Are They Interdependent?

INTRODUCTION: The toes are the distal extension of the foot increasing its weight-bearing area and the security of the stance. The nails are the acral part of the toes and are so intimately linked with the distal interphalangeal joint, its ligaments, and tendons that they were also called musculoskeletal appendages. The big toe is of particular importance for gait and stance. Anatomic alterations of the foot and toe lead to pathological changes of the nails, both directly as well as indirectly. METHODS: The author evaluated the clinical photographs of 1,663 patients examined for toenail condi

Diabetic microvascular disease in non-classical beds: the hidden impact beyond the retina, the kidney, and the peripheral nerves.

Diabetes microangiopathy, a hallmark complication of diabetes, is characterised by structural and functional abnormalities within the intricate network of microvessels beyond well-known and documented target organs, i.e., the retina, kidney, and peripheral nerves. Indeed, an intact microvascular bed is crucial for preserving each organ's specific functions and achieving physiological balance to meet their respective metabolic demands. Therefore, diabetes-related microvascular dysfunction leads to widespread multiorgan consequences in still-overlooked non-traditional target organs such as the b

Effect of Diabetes on Tendon Structure and Function: Not Limited to Collagen Crosslinking.

Diabetes mellitus (DM) is associated with musculoskeletal complications-including tendon dysfunction and injury. Patients with DM show altered foot and ankle mechanics that have been attributed to tendon dysfunction as well as impaired recovery post-tendon injury. Despite the problem of DM-related tendon complications, treatment guidelines specific to this population of individuals are lacking. DM impairs tendon structure, function, and healing capacity in tendons throughout the body, but the Achilles tendon is of particular concern and most studied in the diabetic foot. At macroscopic levels,

Hypomelanosis of Ito: neurological and psychiatric pictures in developmental age.

Hypomelanosis of Ito (HOI) is a multisystem neurocutaneous disorder. In the described cases, cutaneous manifestations (unilateral or bilateral streaks and swirls of hypomelanosis with regular and confluent borders) and extracutaneous abnormalities are often associated. Extracutaneous abnormalities involve the musculoskeletal system (scoliosis, vertebral anomalies, cranial-facial malformations) and other organs, as well as the central nervous system (CNS). The most significant anomalies of the CNS are psychomotor retardation and cognitive deficit. Autism, epilepsy, language disorders, cerebral

Ball heading and subclinical concussion in soccer as a risk factor for anterior cruciate ligament injury.

Soccer players have a high risk of anterior cruciate ligament (ACL) injury, a potentially career-ending event. ACL rupture has been linked with abnormal neuromuscular control in the lower limb. Additionally, heading the ball with the unprotected head during game play is increasingly recognized as a major source of exposure to concussive and sub-concussive repetitive head impacts. This article provides a hypothesis of potential connection of ACL injury with ball heading in soccer players. The study reviews literature sources regarding the impact of neurocognitive alterations after ball headings

Onset of developmental dysplasia of the hip during clubfoot treatment: report of two cases and review of patients with both deformities followed at a single institution.

Developmental dysplasia of the hip and idiopathic clubfoot are common musculoskeletal conditions in the pediatric orthopedic population. Although ultrasound or radiographic screening is generally recommended to rule out occult hip dysplasia, it has been suggested that children with clubfoot should no longer be screened. We report two cases of newborns with unilateral clubfoot and bilaterally stable hips that developed unilateral hip dysplasia on the same side as the foot deformity during clubfoot treatment. All cases previously reported in the literature are reviewed. We recommend systematic h

Physical examination of the aged patient.

The aged patient presents challenges to the examining physician. Some of these relate to inefficient historical information transfer, reduced mobility, increased disease burden, cognitive and sensory alterations, and confusing background changes in the prevalence and interpretation of physical signs that would be unequivocally abnormal in the younger adult. This review suggests techniques to overcome these difficulties. Descriptions of methods of improving evaluation and interpretation are grouped into the following categories: general appearance and vital signs; skin; head and neck; respirato

The distinct musculoskeletal phenotypes of Type 1 and Type 2 diabetes: a sonographic characterization of enthesopathy burden.

OBJECTIVE: Musculoskeletal complications represent a significant burden in diabetes, yet the distinct presentation between Type 1 (T1DM) and Type 2 Diabetes Mellitus (T2DM) remains under-characterized. This study aimed to evaluate and quantify the burden of rheumatic manifestations and subclinical enthesopathy in these two distinct clinical phenotypes. METHODS: We conducted a cross-sectional analysis of 65 patients (21 T1DM, 44 T2DM). A comprehensive rheumatological evaluation assessed adhesive capsulitis (defined as painful restriction of active and passive motion in ≥ 2 planes), knee osteoa

The role of ultrasound and magnetic resonance imaging in the assessment of musculoskeletal involvement in systemic lupus erythematosus: A review of recent advances and insights.

Systemic lupus erythematosus (SLE) is an autoimmune disease characterized by diverse manifestations, with musculoskeletal (MSK) involvement being one of the most prevalent and debilitating. Traditional perceptions of SLE arthritis as mild and primarily non-erosive are increasingly challenged by advanced imaging techniques, such as ultrasound and magnetic resonance imaging (MRI). These modalities have revealed subclinical inflammatory changes, structural damage, and peri-articular soft tissue involvement in both symptomatic and asymptomatic patients. Ultrasound is highly sensitive in detecting
