A case of Erdheim Chester disease with central nervous system involvement.

Erdheim Chester disease (ECD) is a rare non-Langerhans cell histiocytosis, commonly involving the musculoskeletal system. Other tissue can also be involved, including the central nervous system with wide spectrum of clinical features, at times being nonspecific. This can cause diagnostic dilemmas with delay in diagnosis and initiation of therapy. Here we describe a 63-year-old man who had presented with ataxia and behavioral changes, bony pains, weight loss, and fatigue. His computed tomography (CT), 99Tc scintigraphy and histopathological features on bone biopsy were consistent with ECD. Thus…

Asymmetric crying facies and congenital hypothyroidism: report of two patients.

A congenital asymmetric crying facies (ACT) is caused by congenital hypoplasia or agenesis of the depressor anguli oris muscle. Associations of this facial defect with major congenital anomalies have been reported, most commonly in the cardiovascular system and less frequently involving the genitourinary, musculoskeletal, cervicofacial, respiratory, and rarely, the endocrine system. It is also well known that congenital hypothyroidism may be associated with other congenital malformations, especially with cardiac defects. Two newborn infants who had ACF associated with congenital heart defect a

Assessing barriers to musculoskeletal trauma care in Ethiopia: a national providers' survey.

BACKGROUND: Musculoskeletal trauma represents a major burden in Ethiopia, yet system level barriers to effective care remain poorly studied. This study aimed to identify evidence-based, expert-informed priorities for national orthopaedic capacity building in Ethiopia. METHODS: A scoping literature review guided the development of essential goals and challenges to musculoskeletal trauma care delivery in Ethiopia. These domains were refined through in-depth interviews with orthopaedic surgeons at tertiary hospitals. A structured survey was then distributed to providers and public health workers

Data-driven classification of primary Sjögren's syndrome: From cluster analysis to clinical immune phenotypes and predictive biomarkers.

BACKGROUND: Primary Sjögren's Syndrome (pSS) exhibits significant clinical heterogeneity, and traditional organ-based classification systems fail to capture the underlying disease mechanisms. This study aims to identify distinct clinical immune phenotypes of pSS through a data-driven approach and explore their predictive biomarkers. METHOD: This cross-sectional study included 1087 patients who met the 2016 ACR/EULAR classification criteria for primary Sjögren's syndrome between 2014 and 2024. Unsupervised K-means clustering analysis was applied to 10 organ involvement variables to identify na

Influence of musculoskeletal injury history on countermovement jump performance in rugby players.

OBJECTIVE: To examine whether musculoskeletal injury history is a factor that influences countermovement jump (CMJ) performance in rugby players and to compare CMJ performance between player's position (forwards or backs). METHODS: Thirty rugby players (15 with an injury history and 15 without an injury history) performed the CMJ using an inertial sensor (Baiobit®) to evaluate kinematic (Jump Height, Velocity and Phase duration) and kinetic data (rate of force development [RFD], impact peak [IP], take-off force and Stiffness]. The group comparison was performed with an independent t-test, and

Primary Soft-Tissue Lymphomas: MRI Features Help Discriminate From Other Soft-Tissue Tumors.

OBJECTIVES: MRI presentation of extra-nodal soft-tissue lymphomas (STLs) is scarcely reported and lacks of comparison with other soft-tissue tumors (STTs) including sarcomas (STS). Yet, suggesting this diagnosis on MRI would considerably reduce diagnostic intervals. Our aim was to investigate if conventional MRI could discriminate STLs from other STTs. METHODS: MRIs of STL patients were compared with those of patients addressed to a sarcoma reference center for the diagnosis of a STT. MRI characteristics depicting the tumor (size, signal, habitats, shape, surrounding tissues) were reported. U

The effects of sitting Tai Chi on physical and psychosocial health outcomes among individuals with impaired physical mobility.

BACKGROUND: Impaired physical mobility, most often seen in people with neurological disorders (i.e., stroke and spinal cord injury survivors), musculoskeletal diseases or frailty, is a limitation in independent and purposeful physical movement of the body or one or more extremities. The physical restrictions result in negative consequences on an individual's physical and psychosocial functions. This proposal describes a systematic review protocol to determine the effectiveness and approaches of sitting Tai Chi intervention for individuals with impaired physical mobility. Our review would infor

Congenital Incidental Findings in Children that Can Be Mistaken as True Pathologies in Adults: Pearls and Pittfalls of Imaging Diagnosis.

Congenital entities sharing imaging characteristics with true pathologies occasionally are discovered incidentally in adults. These may occur in the neck, chest, abdomen/pelvis, or musculoskeletal systems. Although these incidental findings share imaging features with true pathologic processes, up-to-date knowledge and assessment with the most appropriate imaging modalities generally allow a distinction between congenital entities that may be safely dismissed and pathologic processes requiring further assessment and treatment. This article reviews several of the most common congenital processe…

Effectiveness of manual therapy versus surgery in pain processing due to carpal tunnel syndrome: A randomized clinical trial.

BACKGROUND: People with carpal tunnel syndrome (CTS) exhibit widespread pressure pain and thermal pain hypersensitivity as a manifestation of central sensitization. The aim of our study was to compare the effectiveness of manual therapy versus surgery for improving pain and nociceptive gain processing in people with CTS. METHODS: The trial was conducted at a local regional Hospital in Madrid, Spain from August 2014 to February 2015. In this randomized parallel-group, blinded, clinical trial, 100 women with CTS were randomly allocated to either manual therapy (n = 50), who received three sessi

Power and color Doppler ultrasound settings for inflammatory flow: impact on scoring of disease activity in patients with rheumatoid arthritis.

OBJECTIVE: To determine how settings for power and color Doppler ultrasound sensitivity vary on different high- and intermediate-range ultrasound machines and to evaluate the impact of these changes on Doppler scoring of inflamed joints. METHODS: Six different types of ultrasound machines were used. On each machine, the factory setting for superficial musculoskeletal scanning was used unchanged for both color and power Doppler modalities. The settings were then adjusted for increased Doppler sensitivity, and these settings were designated study settings. Eleven patients with rheumatoid arthri

Dual-Energy CT: Basic Principles, Technical Approaches, and Applications in Musculoskeletal Imaging (Part 2).

In recent years, technological advances have allowed manufacturers to implement dual-energy computed tomography (DECT) on clinical scanners. With its unique ability to differentiate basis materials by their atomic number, DECT has opened new perspectives in imaging. DECT has been successfully used in musculoskeletal imaging with applications ranging from detection, characterization, and quantification of crystal and iron deposits, to simulation of noncalcium (improving the visualization of bone marrow lesions) or noniodine images. Furthermore, the data acquired with DECT can be postprocessed t

Return-to-play: a primary care physician's guide to management.

BACKGROUND: In the primary care clinic, allowing young athletes to return to play following injuries and illnesses can be a challenging balancing act between promoting speed of recovery while still allowing for full recovery and prevention of future injuries or complications. METHODS: A literature review of relevant return-to-play articles was performed for musculoskeletal injuries, concussions, and infectious mononucleosis. RESULTS: We identified several evidence-based approaches to allowing athletes to return to activity following injuries and illnesses. The majority of return-to-play reco

The study of entheses: proposal of a standardised scoring method for twenty-three entheses of the postcranial skeleton.

Entheses are always visible on the bone, although with variable forms and degrees of expression. The term "robusticity" indicates the "normal" osseous markings at entheses, while "enthesopathy", indicates the presence of enthesophytes or osteolytic areas. A scoring method for enthesopathy development has already been proposed. In this paper, a standardised descriptive and photographic scoring method is proposed for the robusticity of 23 postcranial skeleton entheses. For each enthesis, 3 levels of development are described, corresponding, in general, to a weak-moderate expression (degree 1), s

The young skater.

Figure skating is one of the best lifetime sports. Participants may skate alone or with a group, recreationally or competitively, at all ages and at all skill levels. As a sport that improves physical fitness, skating builds strength, flexibility, and cardiopulmonary endurance. As a weight-bearing or high-impact activity, it is one of the best sports for increasing bone mineral density and perhaps for preventing osteoporosis. At least half of all competitive figure skating injuries appear preventable. Boots should be as flexible as the skater can control and should be carefully fitted. Normal
