[Dynamic computed tomography of the neck as an alternative to carotid arteriography].

Spontaneous dissection of the internal carotid artery is a frequent cause of acute ischemic cerebrovascular accidents in young patients. The diagnosis is usually based on arteriographic findings. We report a 31-year-old man with spontaneous dissection of the internal carotid artery in the neck, and compare the findings on arteriography with those on contrast enhanced, dynamic computed tomography (DCT) of the cervical and intracranial portion of the carotid system DCT may play an important role in the diagnosis of dissection of the carotid artery, and in its follow-up, as an alternative to arte

FDG-PET/CT imaging for preradiotherapy staging of head-and-neck squamous cell carcinoma.

PURPOSE: Image localization of head-and-neck squamous cell carcinoma lags behind current techniques to deliver a precise radiation dose with … T imaging protocol between September 2000 and June 2003. Of these patients, 20 went on to immediate neck dissection surgery and were studied further. Of these 20, 17 (85%) had American Joint Committee on Cancer Stage III or IV disease. All patients underwent preoperative FDG-PET and contrast-enhanced CT of the head and neck. FDG-PET/CT images were created using a …

Sentinel lymph node biopsy in a young child with thick cutaneous melanoma.

Head and neck melanoma is a rare and aggressive childhood malignancy. Surgery remains the primary treatment, with lymphatic involvement determined by neck dissection. In the adult population, sentinel lymph node biopsy has emerged as a less morbid yet accurate method of staging regional lymph nodes. This innovative technique can also be used in the pediatric population.

Unilateral fenestration of the internal jugular vein: a case report.

The internal jugular vein (IJV) is a significant landmark that is encountered during dissection of the cervical lymph nodes in oncological surgery, central venous catheter insertion and interventional radiological procedures. The variations in the patterns of its course, and knowledge of these variations, are important. During the neck dissection of an approximately 75-year-old male cadaver, unilateral fenestration of the IJV, the medial branch of which received the submental and linguofacial veins, was found--a case that hitherto has not been reported. An embryological evaluation and the clin…

Medullary thyroid carcinoma presenting as a supraglottic mass.

We report a rare case of medullary thyroid carcinoma that presented as a metastasis to the supraglottic larynx. A 92-year-old man with a 3-month history of voice change and airway obstruction was diagnosed with medullary thyroid carcinoma metastatic to the supraglottis. Excision of the mass, total thyroidectomy, and elective neck dissection were recommended, but the patient declined because of his advanced age. Medullary carcinoma of the thyroid gland is a rare neuroendocrine tumor with a poor prognosis when associated with a distant metastasis. To the best of our knowledge, this is the first …

[External ear melanoma].

Cutaneous melanomas are the tumours that have increased more their incidence in the last fifty years. Melanomas arising from the external auditory canal are extraordinariously unfrequent. These tumours show an aggressive and silent behaviour, and due to this the diagnosis is frequently made in an advanced stage. A male with a malignant melanoma arising from his left external auditory canal was attended in our department, suspecting an epidermoid carcinoma. The clinical findings and the extension of the lesion required a lateral temporal bone resection, parotidectomy and neck dissection to achi…

Superficial parotidectomy in the treatment of cutaneous melanoma of the head and neck.

The preauricular lymph nodes are frequently the first site of metastatic disease from primary malignant melanoma of the upper two-thirds of the face or the anterior scalp. For these patients, or those with adjacent metastatic nodal disease, the prognosis is poor. The median survival of 13 such patients presented was 2 years, with two long-term survivors at 4 and 6 years. Palliative surgery can, however, prevent uncontrolled locoregional disease. Patients with palpable preauricular and cervical node disease should be treated by facial nerve-preserving parotidectomy and radical neck dissection. …

A Rare Anastomosis Between the Internal and the External Jugular Veins: A Case Report.

Usually, the external jugular vein (EJV) is located superficially over the sternocleidomastoid muscle and joins the subclavian vein or the venous angle. The internal jugular vein (IJV) lies deeply in close relation with the common carotid artery and vagus nerve, enveloped by the carotid sheath. Normally, there is no direct connection between those vessels. During a routine neck dissection, we found a rare anastomosis between IJV and EJV. The anastomosis was localized on the level of the cricoid cartilage. It was approximately 1 cm long, with the diameter of the lumen being 0.3 cm. There was no…

Comprehensive anatomical dissection procedure with special reference to the layer-structured facial muscles and fasciae and mouth floor.

OBJECTIVES: Conventional head and neck practice procedures fail to address several clinical issues. Anatomical structures were removed from the surface layers in that order, and the dissection was shifted to the deeper layer. This approach makes it difficult for medical and dental students to understand the relationships among various anatomical structures. Furthermore, it is difficult to determine the risk on the mandibular nerves and blood vessels during conventional mandible removal procedures. This study aims to develop an anatomical technique that maintains the skin, superficial facial mu

Surgical anatomy of the spinal accessory nerve in the posterior triangle of the neck.

OBJECTIVE: The major complication of neck dissection and surgery at the posterior triangle of the neck is severe disability of the shoulder or "shoulder syndrome", which results from spinal accessory nerve injury. Surgical landmarks of the nerve in this area were studied. METHODS: Fifty-six fresh Thai cadavers (112 necks) were dissected to identify the anatomical relationship of the spinal accessory nerve and its commonly used landmarks. RESULTS: The spinal accessory nerve was found within 3.6 cm (mean, 1.43 cm) above Erb's point. The distance between the spinal accessory nerve entering the …

Number of excised lymph nodes as a quality assurance measure for lymphadenectomy in melanoma.

IMPORTANCE: Although the number of excised lymph nodes (LNs) represents a quality assurance measure in lymphadenectomy for many solid tumors, the minimum number of LNs to be dissected has not been established for melanoma. OBJECTIVE: To investigate the distribution of the number of excised LNs in a large patient series (N = 2526) to identify values that may serve as benchmarks for monitoring the quality of lymphadenectomy in patients with melanoma. DESIGN, SETTING, AND PARTICIPANTS: A retrospective multicenter study was conducted (1992-2010) in tertiary referral centers for treatment of cuta

Dissection of the epiaortic and intracranial arteries.

Dissection of epiaortic vessels is a rare event but can have serious clinical consequences such as ischaemic injury to the brain, cerebellum or, more rarely to the retina and is an important cause of stroke in young adults. The main clinical presentation is headache or neck pain, usually but not always associated with Horner syndrome or other local symptoms, followed by an ischemic event in the carotid or vertebral district. Very rarely, the dissection can extend to the intracranial vessels leading to subarachnoid hemorrhage. The time between the headache and the stroke is variable, ranging fr

Controversies in relation to neck management in N0 early oral tongue cancer.

The standard local treatment for early-stage tongue cancer with no clinical lymph node metastases is partial glossectomy. The frequency of occult lymph node metastasis is ~20-30%. Thus, whether prophylactic neck dissection with glossectomy or glossectomy alone should be performed has been a controversial issue since the 1980s. Both treatments have advantages and disadvantages; however, especially in cases involving prophylactic neck dissection, surgical invasion and complications including the cosmetic disadvantage caused by neck skin incision, accessory nerve paralysis or facial nerve (mandib…
