Relational coordination and psychological safety among interprofessional healthcare team members on an acute inpatient stroke unit: a cross-sectional study.

Interprofessional teamwork is essential for delivering safe, high-quality stroke care, yet effective coordination across professions remains challenging. As part of a quality improvement project, we conducted a cross-sectional study to assess baseline interprofessional teamwork on an acute inpatient stroke unit, focusing on relational coordination (RC) and psychological safety. An electronic survey was completed by 53 healthcare team members (n = 137), including attending-physicians (senior physicians), trainee-physicians (fellows and residents), nurses, and other allied health professionals.

Key features of maltreatment of the infirm elderly in home settings.

OBJECTIVE: To identify contributory factors of elder abuse by caregivers in home settings. METHOD: Using a reliable instrument, visiting nurses rated observations symptomatic of abuse and neglect found in their current caseload of elderly patients. Their observations were also classified as related or unrelated to wilfull maltreatment. RESULTS: Regression analysis produced the following significant findings: 1. signs of poor physical care were found to be predictors of physical abuse; 2. signs of psychosocial distress and exploitation were identified as predictors of emotional abuse; 3. defe

Temporary nursing staff - cost and quality issues.

AIM: This paper is a report of a comparative study of temporary and permanent staff work activity, cost and quality of care. BACKGROUND: Temporary nurse staffing can consume significant proportions of the health service wages bill, and this is unlikely to fall owing to recruitment and retention problems, high sickness absence and the tendency for staff to work 'casually'. If temporary workers are here to stay, then what impact do they have on inpatient care? For example, do ward nursing quality and costs change when temporary staff are employed? METHOD: Ward patient dependency, nursing activ

Acceptability and feasibility of using non-specialist health workers to deliver mental health care: stakeholder perceptions from the PRIME district sites in Ethiopia, India, Nepal, South Africa, and Uganda.

Three-quarters of the global mental health burden exists in low- and middle-income countries (LMICs), yet the lack of mental health services in resource-poor settings is striking. Task-sharing (also, task-shifting), where mental health care is provided by non-specialists, has been proposed to improve access to mental health care in LMICs. This multi-site qualitative study investigates the acceptability and feasibility of task-sharing mental health care in LMICs by examining perceptions of primary care service providers (physicians, nurses, and community health workers), community members, and

Predictors of root caries in the elderly.

In an attempt to determine the association between overall health status, medication history, and oral hygiene status and root caries, 24 older persons residing independently in the community and 23 residents of a nursing home were interviewed and received oral examinations, using the Oral Hygiene Index and the Root Caries Index. The mean age of this population was 80 yr, with the nursing home subjects somewhat older than community-dwelling subjects. Only six persons had no root surface caries; three subjects had caries on all tooth surfaces with gingival recession. Mandibular teeth, particula

Caring for the infectious patient: risk factors during pregnancy.

Health care personnel are exposed to infectious diseases in the community as well as in the health care setting when they care for patients with transmissible conditions. Pregnant nurses, physicians and others face an additional risk--that of exposing their unborn children to some of these infections. Risk factors vary greatly between diseases and it is, therefore, important that each be evaluated and acted upon realistically. We will discuss and compare the risk of infections such as hepatitis, cytomegalovirus, tuberculosis, chickenpox, and others. With this knowledge, administrators and pers

"Imagine if I'm not here, what they're going to do?"-Health-care access and culturally and linguistically diverse women in prison.

BACKGROUND: Women in prison have complex medical needs and poorer health status than the general population. Culturally and linguistically diverse (CALD) women in prison, particularly those with limited English proficiency (LEP), have distinct needs and risk additional isolation, discrimination and marginalization when they are in prison. OBJECTIVE: We sought to examine how cultural and linguistic diversity, particularly LEP, affects the health-care experiences of women in prison. DESIGN, SETTING AND PARTICIPANTS: We conducted focus groups and semi-structured qualitative interviews with CALD

OnabotulinumtoxinA for Pediatric Migraine.

BACKGROUND: Migraine is a painful, prevalent, and problematic condition among children. Children need access to safe and effective treatment options to alleviate the impact of this chronic condition on their wellbeing. CLINICAL IMPLICATIONS: Nurses have a crucial role in supporting patient access to BTX-A. Given the results of this and other studies demonstrating the safety and efficacy of BTX-A in children, nurses can support policy change for health plans to fund this intervention for pediatric migraineurs. Allowing children to receive the safe and effective BTX-A injections will lessen the

Therapy of scabies: nursing homes, hospitals, and the homeless.

Scabies epidemics in nursing homes, acute and chronic care hospitals, residential facilities, and communities have become commonplace. Patients with keratotic (crusted, Norwegian) scabies are highly contagious and often initiate outbreaks in institutions. This is a review of problems associated with the management of scabies epidemics in health care facilities. A model treatment plan, readily customized to suit specific epidemic scenarios, is detailed.

Health care assistants in out of hours community palliative care: multiple qualitative organisational case studies.

BACKGROUND: Globally, the role, title and preparation of the unregulated worker is unstandardized, yet increasing evidence suggests they play an important role in the provision of palliative care. In the United Kingdom, a Health Care Assistant (HCA) is an unregulated healthcare worker who provides support to patients under the supervision of registered professionals across clinical settings. Whilst the provision of hospice out-of-hours (OOH) community palliative care is considered a key priority internationally; little is known about the role and contribution of the HCA in the provision of suc
