Transient Neurological Events After Surgery for Pediatric Moyamoya Disease: A Retrospective Study of Postoperative Sedation Practices.

BACKGROUND: Moyamoya disease is a cerebrovascular disease characterized by bilateral stenosis of the intracranial internal carotid arteries and an abnormal collateral vascular network at the base of the brain. Transient neurological events (TNEs), which are episodes of neurological dysfunction lasting <24 hours, are associated with stroke in pediatric patients with Moyamoya disease. Perioperative agitation often occurs in pediatric patients. We hypothesized that anesthetic technique and postoperative sedation would modify the association between TNE and superficial temporal artery-middle cereb

A Multidisciplinary Approach in the Management of Aggressive Retinopathy of Prematurity With Bleb-Like Posterior Combined Retinal Detachment.

We report two cases of bleb-like combined retinal detachment (BLRD) in infants with aggressive retinopathy of prematurity (A-ROP). Both cases involved premature infants born at 28 weeks of gestation with complex postnatal courses, who were referred to our clinic for ROP management. On presentation, both babies were diagnosed with stage 4B ROP in zone 1 with bilateral BLRD. Indirect ophthalmoscope-guided laser photocoagulation under topical anesthesia was performed on the same day. Pediatric clearance for surgery was obtained. Both infants underwent blood transfusions for anemia. Subsequently,

Efficacy and safety of spontaneous ventilation technique using dexmedetomidine for rigid bronchoscopic airway foreign body removal in children.

BACKGROUND: In children, removal of an airway foreign body is usually performed by rigid bronchoscopy under general anesthesia. Debate continues regarding the respiratory mode (spontaneous or controlled ventilation) and appropriate anesthetic drugs. Dexmedetomidine has several desirable pharmacologic properties and appears to be a useful agent for airway surgeries. OBJECTIVES: This study evaluates the efficacy of spontaneous ventilation (SV) technique using dexmedetomidine for bronchoscopic removal of foreign bodies in children. METHODS: Eighty pediatric patients undergoing rigid bronchoscop

Inferior alveolar nerve block anesthesia in children: The effect of ibuprofen and phentolamine mesylate on pain perception.

CONTEXT: For successfully managing pediatric dental patients, local anesthesia is essential to eliminate pain during or after the operative period. An early recovery from soft-tissue anesthesia after an inferior alveolar nerve block (IANB) should benefit a young child patient by avoiding the risk of inadvertently biting the soft tissues. AIMS: Hence, the purpose of the study was to (1) evaluate and compare the efficacy of pre- and postoperative ibuprofen on pain perception in children who undergo IANB anesthesia with or without the use of PM and (2) evaluate the average time required for reve

Anesthetic Considerations in Pediatric Corneal Neurotization: A Case Report.

Congenital corneal anesthesia (CCA) is a rare pediatric disorder characterized by absent or markedly reduced corneal sensation, predisposing to neurotrophic keratopathy and vision-threatening complications. Corneal neurotization has emerged as a surgical option to restore corneal innervation, but anesthetic considerations in pediatric cases remain scarcely reported. We present the anesthetic management of a six-year-old female child with CCA and recurrent herpetic keratitis following deep anterior lamellar keratoplasty, who underwent indirect corneal neurotization with a sural nerve graft. Ane

Evidence-Based Review by a Multidisciplinary Team of Pediatricians on the Use of Gastric Acid-Reducing Medications in Children: Indian Perspectives.

Gastric acid-reducing medications (ARMs) such as proton pump inhibitors (PPIs) and histamine type 2 receptor blockers (H2 blockers) are crucial in pediatric care for treating various gastrointestinal conditions. These medications are frequently used to treat erosive esophagitis, peptic ulcer disease, and gastroesophageal reflux disease (GERD). ARMs are essential to the administration of eosinophilic esophagitis and Helicobacter pylori infection. Additionally, literature also supports its use in alleviating drug-induced dyspepsia, preventing stress-related mucosal damage, and lowering the risk

Use of cryobiopsy for diagnosis and treatment of a rare laryngeal tumor in pediatrics.

This report highlights the importance of differential diagnosis of laryngeal obstructive airway pathologies, as well as the growing value of endoscopic techniques, such as cryobiopsy, in achieving a definitive, accurate diagnosis. This is an 8-month-old infant who presented with signs of airway obstruction and a history consistent with foreign body aspiration. During endoscopy under general anesthesia, a supraglottic tumor was observed, which was resected using a cryobiopsy probe, a method that is not widely used in pediatrics and in our setting. Postoperative pathological anatomy confirmed th

CNS toxicity after topical application of EMLA cream on a toddler with molluscum contagiosum.

EMLA (eutectic mixture of local anesthetics) cream is used topically to provide local anesthesia for a variety of painful superficial procedures. Although the side effects of EMLA are usually mild and transient local skin reactions, potential life threatening complications can be encountered. We report a case of central nervous toxicity after EMLA application for curettage of molluscum contagiosum lesions in a pediatric patient. This complication was the result of a therapeutic misadventure that led to an excessive application of EMLA cream over an extensive area causing an overdose of lidocai

BACKGROUND: Estimating the endotracheal tube size with the optimal internal diameter (ID) is of outstanding importance for airway management in pediatric patients. For many years different weight, height, and/or age-based formulas have been published. The aim of the present study was to identify and to compare published formulas to estimate optimal tube size in pediatric patients. MATERIALS AND METHODS: A PubMed search was performed to identify published formulas for tube diameter in pediatric patients. The keywords "pediatric" or "paediatric", "anesthesia" or "anaesthesia", "anaesthesiology"

Central Giant Cell Granuloma: Diagnosis and Treatment in a Multidisciplinary Setting.

A 7-year-old girl presented with painless swelling in the anterior region of the mandible as well as buccolingual expansion and spacing between the incisors. Imaging showed a well-defined, multilocular radiolucent lesion with cortical perforation, displacement of the canine tooth bud and root resorption of adjacent incisors. Diagnosis of central giant cell granuloma (CGCG) was confirmed by incisional biopsy and histopathology. Management involved a multidisciplinary team, including oral and maxillofacial surgery, pediatric dentistry, prosthodontics and orthodontics. Surgical enucleation with p

Case Report: Adjunctive hyperbaric oxygen therapy in pediatric glans reimplantation following penile trauma.

Pediatric penile trauma is a rare urological emergency, and evidence-based guidelines for complex tissue salvage are limited. We report the case of an eight-year-old boy who presented six hours after sustaining a partial ventral glans avulsion. The patient underwent evaluation under general anesthesia followed by delayed surgical reimplantation of the avulsed glans tissue. As an adjunct to surgery, he received ten consecutive sessions of hyperbaric oxygen therapy (HBOT). Post-treatment follow-up demonstrated complete graft integration, excellent cosmetic outcome, preserved urinary function, an

Enteral access and fundoplication in children with congenital heart disease.

Congenital heart disease (CHD) is the most frequently occurring congenital disorder and affects approximately 1% of live births.1,2 Advancements in supportive technology and surgical techniques have allowed many of these children to live into adulthood with reductions in morbidity and mortality.3,4 During infancy, many children with CHD are plagued with co-existing structural anomalies and/or feeding disorders that make adequate oral intake impossible.5 Pediatric surgeons are frequently consulted for enteral access and/or fundoplication to ensure proper growth and development while preventing

Successful topical treatment of anal giant condylomata acuminata in an infant.

Anogenital condylomata acuminata are induced by human papillomavirus (HPV) and they rarely manifest in immunocompetent children. Therapeutic options depend on patient's age and general conditions and extension of the lesions. However, management is still a challenge and recurrences are frequent. Cryotherapy, laser, and surgical treatments in children are painful and frequently require general anesthesia. Imiquimod is a topical immune response modifier and constitutes a noninvasive alternative for the treatment of anogenital condylomata acuminata. Here, we report an infant admitted to our hospi

Penile epidermoid inclusion cyst after circumcision: A case report.

Epidermoid inclusion cysts are benign lesions resulting from implantation and proliferation of epidermal elements within the dermis, often following trauma and surgery. They are rare in penile region, especially in pediatric patients. This report describes a four-year-old boy who experienced a painless swelling of the penile foreskin two years after undergoing traditional circumcision. Clinical examination revealed a mobile mass on the ventral aspect of the foreskin. Surgical excision, as well as circumcision, were performed under general anesthesia, and histopathology confirmed the diagnosis.
