Targeted Endodontic Microsurgery: A Guided Approach - A Report of Two Cases.

Targeted endodontic microsurgery combines a precisely designed three-dimensional (3D)-printed surgical guide in which the osteotomy site and angulation is defined preoperatively to avoid damaging anatomically important structures. The current endodontic microsurgical procedures have been progressing in pace with technological advances as a predictable alternative to nonsurgical treatment of persistent and recurrent apical periodontitis. The 3D-printed template has been used earlier in the guided endodontic procedure (access openings). The endodontic microsurgery utilizes the surgical microscop

The Relevance of Root's Convergence with Trauma from Occlusion.

BACKGROUND: The relevance of root's convergence with trauma from occlusion has not been widely studied. Anatomically, alveolar bone support can be divided into several levels. The root which is convergent gives a critical mass of alveolar bone support to the teeth so that it is inadequate to stabilize the tooth. AIMS: The purpose of the study is to observe the relevance of root's convergence with traumatic from occlusion. SETTINGS AND DESIGN: This was an observational retrospective design. SUBJECTS AND METHODS: Samples in this study consist of periapical radiograph of the maxillary and mand

[The role of scintigraphy in the diagnosis of mandibular bone changes].

Investigating the scintigraphic images of jaws may have a diagnostic value of bone alterations of dental origin. Anterior view of whole body bone scintigraphy revealed hot spot on jaws of 61% (279) of patients. Twenty-six patients (mean age 58.3 year) from all of those who had increased tracer uptake (ITU) in the maxillo-mandibular region were called back for dental examination. 279 out of 459 (61%) patients had ITU in the maxillo-mandibular region. Dental examination revealed the dental origin of ITU in all cases. In case of periapical pathosis tracer uptake showed 28.79% increase compared to

Oral disease burden in patients undergoing prosthetic heart valve implantation.

BACKGROUND: Valvular heart disease predisposing to endocarditis and requiring prosthetic valve implantation is common among the elderly. Spontaneous bacteremias associated with acute or chronic oral/odontogenic infections may represent a far greater cumulative risk for the development of endocarditis than do occasional health care procedures administered in a professional setting. METHODS: To determine the oral disease burden in patients undergoing mechanical or bioprosthetic heart valve implantation, we performed a comprehensive clinical and radiographic regional examination on 156 consecuti

Saving the despair: conservative management of an endo-perio lesion using hemisection.

Hemisection, a surgical procedure involving the removal of one root and its associated coronal portion from a multi-rooted tooth, is a complex treatment option that aims to salvage a severely compromised tooth while maintaining its functionality and overall oral health. A middle-aged female patient presented with pain and tooth mobility in the lower jaw for the past 2 months. Initially, root canal treatment was done to treat symptoms of symptomatic irreversible pulpitis and apical periodontitis. However, considering the poor prognosis and restorability of the tooth, surgical flap reflection wa

Uncertainty in Relation to Clinical Experience-A Qualitative Study on Dental Students' Reflections on Risk Assessment of Root Filled Teeth.

AIM: In uncertain clinical situations, such as assessing the risk of exacerbation of apical periodontitis in root-filled teeth, undergraduate students can benefit from being trained to reflect. This study aimed to explore how students specifically reflected on clinical experiences in relation to uncertainty when assessing a case involving a root-filled tooth with apical periodontitis. METHODOLOGY: Short reflections were written by 52 dental students at Malmö University in 2021. All students were asked to reflect on the risk for exacerbation of apical periodontitis in a case with a root-filled

Tet methylcytosine dioxygenase 1 modulates Porphyromonas gingivalis-triggered pyroptosis by regulating glycolysis in cementoblasts.

Porphyromonas gingivalis is involved in the pathogenesis of multiple polymicrobial biofilm-induced inflammatory diseases, including apical periodontitis, and it triggers pyroptosis accompanied by robust inflammatory responses. Tet methylcytosine dioxygenase 1 (TET1), an epigenetic modifier enzyme, has been is correlated with inflammation, though an association of TET1 and P. gingivalis-related pyroptosis in cementoblasts and the molecular mechanisms has not been shown. Our study here demonstrated that P. gingivalis downregulated Tet1 expression and elicited CASP11- and GSDMD-dependent pyroptos

A clinical report of Type III dens invaginatus: relevant aspects of a combined therapeutic approach.

Dens invaginatus is a developmental abnormality that alters dental morphology; as a result, treating this condition is a challenge for endodontic practices. This article describes how a combination of nonsurgical and surgical therapies was utilized to treat a maxillary central incisor with Type III dens invaginatus and vital pulp. The treatment plan included using computed tomography (CT) for a detailed analysis of the dental anatomy and periapical area, endodontic and surgical procedures, and a 4-year follow-up period that included periodic clinical and radiographic examinations. The follow-u

Traumatic bone cyst suggestive of large apical periodontitis.

This case report shows the importance of establishing the correct diagnosis to provide the appropriate treatment options The traumatic bone cyst is a pseudocyst, usually asymptomatic and found by a routine radiographic examination. Unicystic radiolucency is almost always observed, which can involve the periradicular area of teeth, simulating an inflammatory lesion of endodontic origin. Differential diagnosis should include other pathologies, such as odontogenic keratocyst, central giant cell granuloma, and unicystic ameloblastoma. Its etiology and pathogenesis are not yet definitely establishe
