Diagnosing peritoneal tuberculosis.

A 37-year-old male healthcare worker presented to the medical assessment unit complaining of a 3-month history of lethargy, weight loss, night sweats and intermittent abdominal discomfort. On examination there was some dullness to percussion at the right lung base and decreased breath sounds. He had mild generalised tenderness in his abdomen. Blood tests were normal. Chest x-ray and CT of the thorax showed small bilateral pleural effusions with no other abnormality. CT of the abdomen and pelvis however, showed ascites with extensive thickening of the peritoneum and marked induration of the mes

[Malignant peritoneal mesothelioma].

This work presents the case of a hemorrhagic, necrotized peritoneal malignant mesothelioma in a 46-year old man, discovered during the emergency surgery, performed for an acute surgical abdomen/hemoperitoneum. Mesotheliomas are rare neoplasms of the pleura, peritoneum or pericardium, which are frequently associated with exposure to asbestos. The paraclinical investigations (echography) were not able to specify the diagnosis before the operation. The surgical intervention--tumorectomy, electrocoagulation, hemostasis with Gelaspon sponges-- had favourable results.

[Peritoneal dialysis in the elderly].

BACKGROUND: Peritoneal dialysis is used more and more as treatment of substitution of the end stage renal disease at the old subjects of mor … morbidity. The pulmonary and urinary infectious complications are also frequent. The more frequent peritonitis compared to the literature, are comparable between the 2 age brackets < and = with 65 years and the mode of PD (APD or CAPD). The time separating the beginning from PD and which has occurred of the peritonitis is shorter in APD. The lesions of renal …

Peritoneal dialysis in microencephaly.

J.T. was able to remain home in her familiar environment and receive safe and adequate treatment for her renal disease. J.T. had no infectious episodes or hospitalizations while under this unit's care for 35 months. She was also able to participate in her regular activities of daily living, interact with her family members, and travel on occasion, thus maintaining a good quality of life. Therefore, unit goals for her care were met. J.T.'s experience demonstrates that with proper teaching, preparation, and support from the dialysis care team working with a dedicated family, peritoneal dialysis

Peritoneal mesothelioma: a review.

Malignant peritoneal mesothelioma (MPM) is a rare aggressive tumor of the peritoneum, regarded as a universally fatal disease. It is poorly described and the knowledge of its natural history is very limited. Occupational and environmental asbestos exposure still remains a public health problem around the world. The incidence has increased in the past 2 decades. Only 20% to 33% of all mesotheliomas arise from the peritoneum itself; the pleura is the most common site of origin.

The "heart" of peritoneal dialysis.

Cardiovascular disease accounts for more than half of all deaths in end-stage renal disease patients receiving chronic peritoneal dialysis (PD) treatment. The recent demonstration of the important association between residual renal clearance (but not PD clearance) and overall and cardiovascular survival in chronic PD patients has led us to further explore the mechanisms that can potentially explain the close link between residual renal function and cardiovascular disease in this population. This John Maher Award Lecture provides a review of my own work and that of other groups that provides su

[Peritoneal gelatinous ascites].

Pseudomyxoma peritonei or gelatinous ascites is a rare clinical entity, and its pathogenesis remains obscure. It most often follows a mucinous tumor of the appendix. An ovarian origin in woman has been suggested but remains controversial. Its onset is often insidious: an increase in the abdominal perimeter may be the first sign noted. Preoperative diagnosis is facilitated by modern imaging techniques. Ultrasonography and computed tomography provide complementary signs: septa and scalloping of the liver margins, respectively. Effusion in the lesser peritoneal cavity suggests this diagnosis. Mag

Pathophysiology of peritoneal transport.

The peritoneal membrane has the surface area similar to the body surface area. It consists of mesothelial cells, interstitium, connective tissue fibers, blood vessels, and lymphatics. Solutes of various sizes traverse the peritoneal membrane through at least three various pores: 'large' pores located in the venular interendothelial gaps, small 'paracellular' pores, and ultrasmall, 'transcellular' pores or aquaporins localized in peritoneal capillaries and mesothelial cells. High molecular weight solutes are mass-transfer limited; thus, their clearances do not increase significantly with high d

[Malignant peritoneal mesothelioma].

Mesothelioma is a neoplasm originating from the mesothelial surface lining cells of the serous human cavities. It may involve the pleura, less frequently the peritoneum rarely, the pericardium, the tunica vaginalis testis and ovarian epithelium. Asbestos has been widely used in industry. A causal relationship between asbestos exposure and pleural, peritoneal and pericardial malign mesothelioma was suggested, the risk of cancer being correlated to cumulate exposure. Studies from National Cancer Institute, USA, show that the malignant mesothelioma is a rare and aggressive asbestos related malign

[A false peritoneal carcinomatosis].

Abdominal actinomycosis is an unusual peritoneal disease due to actinomyces, which are Gram-positive bacillus bacteria. Those saprophytes pathogens of digestive tract and genital mucous, can appear accidentally in the peritoneum cavity, in tumor form, simulating a neoplasic disease. We report the case of a 43-year old woman, with abdominal actinomycosis, revealed by feverish bowel obstruction. During laparotomy, several nodes were removed. Their histologic study permitted the diagnosis. Long-term antibiotics treatment allowed patient recovery.

[Prevention of peritoneal adhesions].

Peritoneal adhesions are a major problem for health and economy. An adequate and atraumatic surgical technique is essential in the prevention of peritoneal adhesions. Laparoscopic microsurgery should be developed and promoted. The following conclusions can be proposed about the treatments available Adequately designed clinical studies are too rare. Future studies are necessary to obtain information on pregnancy and pain as primary outcomes. Cost implications of adhesion prevention treatments also have to be addressed.

Neoangiogenesis in the peritoneal membrane.

OBJECTIVE: This study reviews relevant publications on the peritoneal vasculature and tries to establish morphological-functional relationships. DESIGN: The design is a review article. RESULTS: Recent morphological studies in peritoneal dialysis (PD) patients have shown the presence of diabetiform neoangiogenesis in long-term peritoneal dialysis. The same abnormalities could be induced in rats administered a high glucose dialysis solution daily for 20 weeks. The animals showed functional abnormalities in peritoneal transport similar to those found in long-term PD patients. Evidence was obtai

[Peritoneal cystic mesothelioma].

Cystic mesothelioma of peritoneum is a rare neoplasia that was identified as a individual entity by ultrastructural and immunoistochemical studies. It is more frequent in young woman and often it tends to recur. Because of its rarity and difficulty in differential diagnosis, the Authors describe a case of cystic mesothelioma observed.

[Disseminated peritoneal leiomyomatosis].

Leiomyomatosis peritonealis disseminata is a rare disease. Approximately 50 cases have been reported in the world literature so far. Origination from the Müllerian epithelium, which is distributed throughout the subperitoneal mesenchyme seems possible in histogenesis. By specific hormonal stimulation and individual predisposition proliferation takes place along the lines of myofibroblastic differentiation. The case of a 41 year-old woman is presented. The patient's history revealed hormonal therapy for dysmenorrhoea. An incidental finding during diagnostic laparotomy for suspicious endometriot

[Imaging of peritoneal mesotheliomas].

Peritoneal mesothelioma is a rare, either benign or malignant tumor of the peritoneum. We retrospectively analyzed the imaging features of six malignant and three benign peritoneal mesothelioma. Asbestos exposure was proven in three of the six malignant cases and in none of the three benign cases. Ascites was found in seven patients (78%), peritoneal masses in eight patients (88%), small peritoneal nodules in six patients (66%), parietal peritoneal thickening in six patients (66%), thickened mesentery in five patients (55%) and pleural plaques as well as nodules in one patient. These imaging f

Is peritoneal closure necessary?

Closure of peritoneal defects after laparotomy has long been considered a standard surgical procedure. In 1895 Smith wrote that "Sinister results which we seek to avoid, arise when we leave raw surfaces to which intestines may adhere and cause obstruction. To cover such a surface by peritoneum would, according to published statistics, save nearly 2 percent of the deaths after abdominal operation." (1). The purpose of this review is to reexamine the necessity of peritoneal closure in current medical practice. The argument for peritoneal closure includes the following: 1) restoration of anatomy

Peritoneal dialysis in the neonate.

The etiology of renal failure in the newborn infant is multifactorial. The increasing survival of low-birthweight infants and the aggressive drug management of pathophysiological disease states such as pulmonary hypertension and patent ductus arteriosus has resulted in an increase in the number of infants presenting with acute renal failure. Experience in our intensive care unit has proved that early intervention with peritoneal dialysis is safe and may significantly reduce both morbidity and mortality in such patients.

[A case of peritoneal histoplasmosis].

The clinical history of 24 year old patient diagnosed as having peritoneal histoplasmosis is presented. This has been described as an infrequent clinical entity. In our patient, the only apparent clinical manifestation is the presence of an enlarged abdomen due to free ascites. Hepatic and peritoneal granulomas are found. The case is discussed and diagnostic difficulties and the prognosis analyzed. The prognosis depends on the condition of the patient and the speed with which treatment (Ketoconazole) is begun. Bibliography is revised.

[Disseminated peritoneal leiomyomatosis].

Peritoneal leiomyomatosis was discovered in a 33 year old woman. The tumor was sited at the uterine isthmus and another had developed in the broad ligament. No recurrence was noted at follow up after one year after limited surgical resection of these two tumors. A review of the literature revealed 45 cases of disseminated peritoneal leiomyomatosis. The tumor nodules varied considerably in size, number and topography. Their structure was composed of collagen fibres and smooth muscle cells. Their development might be the result of a metaplastic process favored by high levels of estrogen. No spec
