Calculating the social impact of home automation for people with disability: A social return on investment study.

INTRODUCTION: Home automation can deliver important outcomes for people with disabilities, including enhanced independence. Despite the millions of dollars spent on home automation in Australia and other developed nations, to date, there has been no economic evaluation of this type of assistive technology. METHOD: A social return on investment analysis of home automation study was undertaken. Primary data were collected using qualitative interviews with home automation consumers and other key stakeholders, including occupational therapists, a spinal rehabilitation physician, peer support advo

Fidelity, acceptability, and feasibility of the revised functional autonomy measurement system for hospitalised people: An implementation study.

INTRODUCTION: The revised functional autonomy measurement system (SMAF-R) is an outcome measure of activity performance and resource needs. This study aimed to measure the implementability of the SMAF-R as a routine outcome measure in acute and subacute inpatients. METHODS: This was a mixed-methods implementation study. Occupational therapists were asked to complete the SMAF-R in hospitalised people with an anticipated length of stay greater than 48 hours over a 12-month period (n = 13,348). A baseline audit of SMAF-R completion rates was conducted, and behaviour change interventions were del

A Dose-Finding, Biomarker Validation, and Effectiveness Study of Transcranial Magnetic Stimulation for Adolescents With Depression.

OBJECTIVE: Research and clinical application of transcranial magnetic stimulation (TMS) for adolescents with major depressive disorder has advanced slowly. Significant gaps persist in the understanding of optimized, age-specific protocols and dosing strategies. This study aimed to compare the clinical effects of 1-Hz vs 10-Hz TMS regimens and examine a biomarker-informed treatment approach with glutamatergic intracortical facilitation (ICF). METHOD: Participants with moderate-to-severe symptoms of major depressive disorder were randomized to 30 sessions of left prefrontal 1-Hz or 10-Hz TMS, s

Serum Potassium and Risk of Death or Kidney Replacement Therapy in Older People With CKD Stages 4-5: Eight-Year Follow-up.

RATIONALE & OBJECTIVE: Hypokalemia may accelerate kidney function decline. Both hypo- and hyperkalemia can cause sudden cardiac death. However, little is known about the relationship between serum potassium and death or the occurrence of kidney failure requiring replacement therapy (KRT). We investigated this relationship in older people with chronic kidney disease (CKD) stage 4-5. STUDY DESIGN: Prospective observational cohort study. SETTING & PARTICIPANTS: We followed 1,714 patients (≥65 years old) from the European Quality (EQUAL) study for 8 years from their first estimated glomerular fi

Mapping the Cortical Representation of Paraspinal Muscles Using Transcranial Magnetic Stimulation Optimized in People With Chronic Back Pain.

INTRODUCTION: Chronic low back pain (CLBP) is a global burden with an unknown etiology. Reorganization of the cortical representation of paraspinal muscles in the primary motor cortex (M1) may be related to the pathology. Single-pulse transcranial magnetic stimulation (TMS), commonly used to map the functional organization of M1, is not potent enough to stimulate the cortical maps of paraspinal muscles in M1 in CLBP patients with reduced corticospinal excitability (CSE) with intensities even as high as maximum stimulator output (100% MSO). This makes TMS mapping impractical for these patients.

Updates in hairy cell leukemia (HCL) and variant-type HCL (HCL-V): rationale for targeted treatments with a focus on ibrutinib.

UNLABELLED: Hairy cell leukemia (HCL) and HCL-like disorders such as hairy cell leukemia variant (HCL-V) and splenic diffuse red pulp lymphoma (SDRPL) are rare indolent B-cell malignancies. Purine analogs (PNAs), alone or in association with rituximab (R), are the standard of care for HCL in the first-line setting. However, PNAs are toxic and patients may become resistant to these drugs. Therefore, new therapeutic strategies are needed. Several recent in vitro studies highlighted the importance of the interactions between HCL cells and their microenvironment, in particular with bone marrow str

Psychosocial Distress in Patients With Advanced CKD by Racial Group and Immigrant Status: A Canadian Cross-sectional Study.

RATIONALE & OBJECTIVE: Patients with advanced chronic kidney disease (CKD) have been reported to experience profound psychosocial distress. Other work has established that patients with CKD from marginalized populations (including individuals who on the basis of race often face racism and related discrimination, termed "racialization") experience health care inequities. Given limited information on the intersection of these 2 phenomena, we assessed the association of psychosocial distress with racialized status and immigrant status in Canadians with advanced CKD. STUDY DESIGN: Secondary analy

Rapid adaptation during the COVID crisis: Challenges experienced in delivering service to those with Opioid Use Disorders.

BACKGROUND: Adaptation is an accepted part of implementing evidence-based practices. COVID-19 presented a unique opportunity to examine adaptation in evolving contexts. Delivering service to people with opioid use disorder during the pandemic required significant adaptation due to revised regulations and limited service access. This report evaluated changes to addiction medication services caused by the pandemic, challenges encountered in rapidly adapting service delivery, and initial impressions of which changes might be sustainable over time. METHODS: Qualitatively-evaluated structured inte

Agomelatine for the treatment of generalized anxiety disorder: focus on its distinctive mechanism of action.

UNLABELLED: Generalized anxiety disorder (GAD), the most frequently diagnosed form of anxiety, is usually treated by cognitive-behavioural approaches or medication; in particular, benzodiazepines (acutely) and serotonin or serotonin/noradrenaline reuptake inhibitors (long term). Efficacy, compliance, and acceptability are, however, far from ideal, reinforcing interest in alternative options. Agomelatine, clinically employed in the treatment of major depression, expresses anxiolytic properties in rodents and was effective in the treatment of GAD (including severely ill patients) in several doub

Zanubrutinib in lymphoproliferative disorders: a comprehensive review.

UNLABELLED: The availability of Bruton tyrosine kinase (BTK) inhibitors has brought about a paradigm shift in the treatment of patients with B-cell lymphomas and chronic lymphocytic leukemia. BTK was clinically validated as a target by the efficacy of the first-in-class inhibitor ibrutinib. The extended survival conferred by BTK inhibitors has brought long-term tolerability to the foreground. To minimize toxicities thought to be attributable to off-target kinase inhibition, a next generation of BTK inhibitors with greater selectivity was developed. In the United States, zanubrutinib, a next-ge

Renin-angiotensin-aldosterone system blockers and region-specific variations in COVID-19 outcomes: findings from a systematic review and meta-analysis.

BACKGROUND AND AIMS: Coronavirus disease 2019 (COVID-19) has been observed to cause a high mortality in people with cardiometabolic diseases. Renin-angiotensin-aldosterone system (RAAS) blockers enhance the expression of ACE2, the binding receptor of SARS-CoV-2, and can enhance viral infectivity. We aim to provide a pooled estimate of the effect of RAAS blockers on COVID-19 outcomes. METHODS: A literature search was performed using MEDLINE/PubMed, Google Scholar and preprint servers. All clinical studies analyzing the effect of RAAS blockers on clinical outcomes in COVID-19 patients were incl

Wolfram syndrome: new pathophysiological insights and therapeutic strategies.

UNLABELLED: Wolfram Syndrome (WS) is an ultra-rare, progressive neurodegenerative disease characterized by early-onset diabetes mellitus and irreversible loss of vision, secondary to optic nerve degeneration. Visual loss in WS is an important cause of registrable blindness in children and young adults and the pathological hallmark is the preferential loss of retinal ganglion cells within the inner retina. In addition to optic atrophy, affected individuals frequently develop variable combinations of neurological, endocrinological, and psychiatric complications. The majority of patients carry re

Capturing the influx of living systematic reviews: a systematic methodological survey.

OBJECTIVES: Living systematic reviews (LSRs) are an emerging type of review that continuously updates as new evidence becomes available. A previous methodological survey conducted in 2021 identified and studied all health-based LSRs. Since then, the landscape has changed, including the on-going accumulation of COVID-19 research and availability of automation tools. Furthermore, various methods and guidance exist for conducting LSRs and review authors are often encouraged to explore opportunities to maximize dissemination. We conducted an LSR survey update to describe LSRs in a "post-COVID" era

Human-in-the-loop machine learning with community engagement reduces title, abstract and full-text screening workload in knowledge synthesis.

OBJECTIVE: Manual screening of titles, abstracts, and full texts for large-volume knowledge synthesis requires significant time investment. Machine-assisted tools offer solutions for facilitating screening. Most applications to date have focused on actively reordering citations or suggesting screening decisions for reviewers to prioritize relevant studies, or on automating exclusions based on narrowly defined eligibility criteria. However, few tools extend to full-text screening, and among those that report, a majority have largely been developed with little engagement of knowledge users. Know

Combining the tunneled coronally advanced flap (TCAF) with internal frenectomy-vestibular incision subperiosteal tunnel access (IF-VISTA) for the treatment of mandibular anterior gingival recessions: A technique report.

INTRODUCTION: Gingival recession (GR) defects in the mandibular anterior region represent a particularly challenging scenario in periodontal plastic surgery, owing to a thin soft tissue phenotype, minimal keratinized tissue, limited vestibular depth, and the frequent presence of a prominent labial frenum. While significant advances have been made in root coverage procedures for the anterior mandible, simultaneously addressing these coexisting anatomical factors in a single, minimally invasive procedure remains a clinical challenge. METHODS: This technique report describes the combined applica

Digitally customized bone block allograft with enamel matrix derivative for ridge reconstruction: Case series. Histological report in humans.

BACKGROUND: Alveolar ridge resorption following tooth extraction is a common clinical challenge that can compromise esthetics, implant placement, and prosthetic rehabilitation. Digital customization of bone block allografts offers precise adaptation to the recipient site, improving graft stability and reducing surgical time. Enamel matrix derivative (EMD) has demonstrated potential to enhance osteogenic differentiation and cell adhesion; however, its application in combination with digitally customized bone block allografts (DCBBAs) remains scarcely reported. This study aimed to evaluate, bone

Long-term stability after non-surgical furcation retreatment with adjunctive collagen hydrogel.

BACKGROUND: Management of inflammation at previously surgically treated periodontal sites represents a clinical challenge. Although surgical retreatment may be indicated in selected cases, a conservative non-surgical approach may restore periodontal stability while avoiding the additional morbidity associated with repeated surgery. This case report describes the long-term management of a previously surgically treated mandibular molar furcation using site-specific non-surgical retreatment combined with adjunctive application of a collagen-based hydrogel. METHODS: A 45-year-old non-smoking male

Reliability of accelerated confirmatory trials during the COVID-19 pandemic: the consequences of low pretrial plausibility.

OBJECTIVES: The reliability of confirmatory clinical trials (ie, the post-trial probability) depends on the pretrial probability. During the COVID-19 pandemic, many early COVID-19 confirmatory trials were launched despite limited phase II evidence and weak mechanistic or preclinical support. Whether this accelerated development strategy reduced the reliability of therapeutic evidence remains uncertain. STUDY DESIGN AND SETTING: Overall empirical pretrial plausibility was estimated at the trial level as the proportion of early randomized trials evaluating interventions that were ultimately con
