Red blood cell transfusion in sickle cell disease: From clinical necessity to immunological complexity.

Sickle cell disease (SCD) is one of the most common inherited haemoglobin disorders worldwide and is associated with substantial morbidity, premature mortality and lifelong healthcare utilisation. Red blood cell (RBC) transfusion remains a cornerstone of SCD management and is indispensable for the treatment and prevention of life-threatening complications, including acute chest syndrome, ischaemic stroke, severe anaemia and perioperative complications. Despite its undeniable therapeutic benefits, repeated transfusions are accompanied by important adverse consequences, including alloimmunisatio

The role of platelets in blood-aqueous barrier breakdown induced by anterior chamber paracentesis in the rabbit.

Anterior chamber paracentesis of the rabbit eye causes disruption of the blood-aqueous barrier, which is characterized by a rapid increase in the albumin and total protein content of the aqueous humor. Prostaglandins appear to be implicated as major mediators in this reaction, since a cyclooxygenase inhibitor, indomethacin, very efficiently prevents protein leakage. When paracentesis was performed in platelet-depleted rabbits (either by transfusion or by treatment with an antiplatelet plasma), the protein content in the aqueous humor did not rise to values observed in normal animals. These dat

Perioperative management of a patient with glanzmann's thrombasthenia for mitral valve repair under cardiopulmonary bypass.

A 30-year-old male patient presented with Glanzmann's thrombasthenia and mitral valve prolapse. He was in acute decompensated congestive heart failure due to severe mitral and tricuspid regurgitation. After his cardiac failure had been stabilized, the patient was subjected to mitral and tricuspid valve repair. His transfusion requirements were guided by thrombelastography and his bleeding disorder was managed by infusing single donor plasmapheresed platelet transfusions in the perioperative period. The patient underwent surgery uneventfully.

[Thrombotic thrombocytopenic purpura. Reduced activity of von Willebrand factor cleaving protease].

Thrombotic thrombocytopenic purpura (TTP) is a rare thrombotic microangiopathy. Besides anemia and thrombocytopenia, neurological impairment is common in TTP. A 42-year-old woman was admitted to a department of obstetrics/gynecology because of severe vaginal bleeding due to thrombocytopenia. After platelet transfusion, the patient developed a reduced level of consciousness, confusion, headache, and fever. CT scan did not show pathological changes. Transcranial Doppler sonography revealed increased blood flow velocities of all basal cerebral arteries. Because encephalitis was suspected the pati…

Investigation of coronary artery bypass grafting for a patient with myelodysplastic syndrome.

A 79-year-old male with unstable angina, who had myelodysplastic syndrome (MDS), was treated with coronary artery bypass grafting (CABG). MDS causes refractory anemia accompanied by various degrees of granulocytopenia and thrombocytopenia. Pancytopenia caused by MDS may complicate patients with major infections and bleeding during cardiac surgery. There were very few patients with MDS who had undergone open-heart surgery. Three case studies, including this study, had reported successful cases of CABG in patients with MDS and the analogous diseases of MDS. We used granulocyte colony-stimulating

Bernard-Soulier syndrome in pregnancy with retinal detachment: a rare phenomenon.

Bernard-Soulier syndrome (BSS) is a rare congenital bleeding disorder of the platelet, and it is mainly inherited as an autosomal recessive trait. It is caused by both qualitative and quantitative deficiency of the platelet membrane glycoprotein (GP) Ib-IX-V receptor complex, thereby causing abnormal platelets adhesion. We report a case of a primigravida in her 20s with history of BSS diagnosed in childhood due to family history. Her preconception period was challenging as she suffered from severe menorrhagia often requiring hospital admission, blood and platelet transfusions. At 35 weeks gestat…

A Case Report of Siblings with Dystonia: A Potential Link Between DYT11 Mutation and Platelet Dysfunction.

Myoclonus-dystonia syndrome (MDS) is an autosomal dominant disorder due to a mutated epsilon-sarcoglycan gene (SGCE) at the dystonia 11 (DYT11) locus on chromosome 7q21-31. ε-sarcoglycan has been identified in vascular smooth muscle and has been suggested to stabilize the capillary system. This report describes two siblings with MDS treated with bilateral globus pallidus interna deep brain stimulation. One patient had a history of bleeding following dental procedures, menorrhagia, and DBS placement complicated by intraoperative bleeding during cannula insertion. The other sibling endorsed freq

Generation of Functioning Platelets from Mature Megakaryocytes Derived from CD34+ Umbilical Cord Blood Cells.

Clinically patients with thrombocytopenia are in urgent need of platelet transfusion, thus it is necessary to produce the platelets in large scale in vitro to meet the clinical needs. In this study, we developed efficient protocol to generate functioning platelets by differentiating umbilical cord blood (CB)-derived CD34+ cells into mature megakaryocytes. Under our condition, up to 85% of mature megakaryocytes were generated from CB-derived CD34+ cells, and over 75% CD42b+CD62p+ platelets were produced. The megakaryocytes at day 12 after the differentiation had the similar gene expression patt…

Immune thrombocytopenia. Use of a Coombs antiglobulin test to detect IgG and C3 on platelets.

We applied a radiolabeled Coombs antiglobulin test to the diagnosis and management of immune thrombocytopenia in adults and children. This assay substantiated that the majority of patients with idiopathic thrombocytopenic purpura have increased levels of IgG on their platelets. Platelet-associated C3 was elevated in a subset of these patients, some of whom had normal levels oet-associated IgG, thus suggesting a role for C3 in the pathogenesis of the thrombocytopenia. The assay enabled us to anticipate a change in clinical status, establish the presence of immunologic remission after splenectom

High-Throughput Metabolomics in Transfusion Medicine.

Over the past two decades, the introduction of omics technologies to the field of transfusion medicine has contributed to a significant expansion of our understanding of the storage lesion and its impact on transfusion recipients. Hundreds of application papers have leveraged the implementation of novel high-throughput omics techniques, such as the high-throughput metabolomics approach described herein. We will briefly describe how these approaches have been used to learn more about erythrocyte storage biology, chronic and massive transfusion recipient biology, and the development or testing o

[Indications and clinical aspects of granulocyte transfusions (author's transl)].

Granulocyte transfusions (n = 222) were administered to 41 patients with granulocytopenia (less than 500 X 10(6)/1) and antibiotics-resistant infections. The average granulocyte content per transfusion was 2.1 X 10(10), 212 transfusions had been obtained by filter leucapheresis. Infections improved in 25 episodes, and 18 patients survived the 30th day. There was no influence in 18 episodes and only 2 patients survived beyond the 30th day. Extent and duration of bone marrow insufficiency, and type and grade of infection, clearly influenced prognosis. Further prognostically relevant risk factors
