Delayed gyration with pontocerebellar hypoplasia type 1.

We report two sibling cases of pontocerebellar hypoplasia type 1 (PCH-1), which showed severe generalized hypotonia, psychomotor delay, and visual impairment. Magnetic resonance imaging in the neonatal period revealed delayed gyration compared to the postconceptional ages in both cases. The elder brother died with pneumonia at 10months of age, and the younger brother survived to over 6years of age. Repeated neuroimaging in the younger brother revealed improvement of the delayed gyration and progressive atrophy, not only in the pons and cerebellum, but also in the cerebrum. To the best of our k…

Imaging of the cerebellopontine angle.

Pathologic processes within the cerebellopontine angle (CPA) may present because of mass effect on local structures or on the pons or cerebellum. MR imaging is superior to CT for evaluation of the complete extent of a CPA mass and for characterization of its likely nature. MR is also superior to CT for the detection of leptomeningeal processes. Arriving at a short list of differential diagnoses requires a methodical approach, keeping in mind the location, signal, diffusion, and enhancement characteristics of the lesions. Advanced MR imaging techniques, such as magnetic resonance spectroscopy, …

Trigeminal neuralgia associated with achondroplasia. Case report with literature review.

A 59-year-old male with a history of 2 years of typical trigeminal neuralgia manifested the characteristics of achondroplasia. X-ray and magnetic resonance imaging demonstrated basilar impression, deformity of the posterior fossa with marked asymmetry of petrous bone and compression of pons and trigeminal nerve by the left vertebral artery and anterior inferior cerebellar artery. Microvascular decompression was performed through a suboccipital craniectomy. The neuralgia disappeared soon after surgery and remains completely resolved until today. This is the first reported case of trigeminal neu…

Epidural hematoma of the clivus. Case report.

This 8-year-old boy presented with a rare case of epidural hematoma of the clivus and atlantoaxial dislocation caused by a hyperflexion injury sustained in a traffic accident. Magnetic resonance (MR) imaging demonstrated an epidural hematoma in the posterior fossa that compressed the pons and medulla. On admission, the patient was confused and had bilateral abducens palsy. He was treated conservatively, and 6 months after admission, the epidural hematoma on the clivus had disappeared on MR imaging and the bilateral abducens palsy was cured. Only two such cases have been reported in the literat…

Rabies RNA synthesis, detected with cDNA probes, as a marker for virus transport in the rat nervous system.

The kinetics of viral RNA synthesis in different parts of the rat brain, infected with fixed or street rabies virus strains, is correlated with their anatomical neuronal connections with the masseter muscles, using hybridization with rabies cDNA probes. Viral RNA synthesis is first detected in the brain-stem and in the pons where the direct anatomical projection of the masseter muscle nervous arborization into the sensory and motor nuclei is located, through the trigeminus nerve. Rabies RNA detection is delayed in the other regions of the rat brain depending on the time course of virus transpo…

Peduncular hallucinosis following microvascular decompression for trigeminal neuralgia: case report.

A patient with peduncular hallucinosis following microvascular decompression is reported. A 63-year-old woman with trigeminal neuralgia had microvascular decompression through a lateral suboccipital approach. When a venous hemorrhage occurred on the surface of the pons during surgery, the two main tributaries of the superior petrosal vein were sacrificed to obtain a wider operative field. On the 1st postoperative day, the patient had no problems and the pain completely disappeared, but on the 2nd day, both visual and auditory hallucinations developed, which continued until the 5th postoperativ…

Subarachnoid haemorrhage in the rat: effect on the development of vasospasm of selective lesions of the catecholamine systems in the lower brain stem.

Intracisternal injection of blood in the rat produces an angiographically demonstrable biphasic vasospasm. Lesioning at the level of the mesencephalon of the ascending catecholamine pathways from locus coeruleus in the pons and the A1 and A2 nuclei in the medulla oblongata prior to cisternal blood injection prevents the development of both acute and late spasm. Selective lesioning in the medulla oblongata of ascending fibres from A1 and A2 also prevents development of spasm, indicating that these nuclei, which project to the hypothalamus-pituitary, are essential for the spasm syndrome. It is s…

[Pathohistologic and biochemical changes in the brain in atherosclerosis with hemorrhagic foci].

On the basis of a pathomorphological and biochemical study of 22 cases with focal hemorrhages in atherosclerosis the author detected a contraction of nerve cells, adipose infiltration, "shadow-cells", a deposit of ferrum in the cytoplasma. The swelling of myelinic fibres was detected more often than hypermyelinization, fragmentation and demyelinization. Glia changes consisted in ameboid transformation and clasmatodendrosis. The maximum increase of the cholesterin amount was observed in pons varoli. Medulla oblongata and dentate nucleus and the minimum--in the cerebellar cortex and cerebrum hem…

Spontaneous listeric encephalitis in sheep. Electron microscopic studies.

The brainstems of four sheep with spontaneous listeric encephalitis had scattered small foci of inflammatory cells (neutrophils or macrophages, or both) with scattered fragments of degenerating nerve fibers and glial cells. In extensive areas of malacia in the pons and medulla oblongata, there was loss of parenchyma with massive accumulation of macrophages, a few neutrophils, lymphocytes and plasma cells. In both types of lesions, phagocytes contained debris of myelin and axons, lipid vacuoles and occasionally bacteria. Neutrophils contained bacteria in phagocytic and digestive vacuoles. No ba…

Low molecular weight iron in cerebral ischemic acidosis in vivo.

BACKGROUND AND PURPOSE: Iron-catalyzed radical generation is a potentially significant mechanism by which extensive tissue acidosis exacerba … operated animals, with the highest amounts obtained from subcortical areas such as the hippocampus, pons, midbrain, and medulla. Total iron was higher in ISCH relative to sham-operated animals and higher in cortex and pons relative to GLU-ISCH. Regional LMW (as a percentage of total iron; LMW/total iron) was elevated in numerous brain areas in ISCH, in …

Gelastic dysarthria: Speech-triggered pathological laughter with evidence for a selective pontine gating mechanism.

We report a highly selective manifestation of pathological laughter in which speech initiation consistently triggers laughter, resulting in disrupted articulation. A 55-year-old male with bilateral pontine infarction exhibited laughter that was time-locked to speech, most prominent during speech production and absent during non-speech tasks. Functional MRI during speech demonstrated activation of language regions, medial prefrontal cortex, amygdala, and ventral pons, with concurrent subjective laughter. We propose that ventral pontine lesions disrupt inhibitory pathways, leading to disinhibiti…

Anatomic and MRI Bases for Pontine Infarctions with Patients Presentation.

OBJECTIVES: There are scarce data regarding pontine arteries anatomy, which is the basis for ischemic lesions following their occlusion. The … were most often isolated and unilateral, and rarely bilateral (10%). They were located in the lower pons (23.3%), middle (10%) or rostral (26.7%), or in two or three portions (40%). Each type of infarction usually produced characteristic neurologic signs. The clinical significance of the anatomic findings was discussed. CONCLUSIONS: There was a good c …

Dissecting the Tectal Output Channels for Orienting and Defense Responses.

Electrical stimulation and lesion experiments in 1980's suggested that the crossed descending pathway from the deeper layers of superior colliculus (SCd) controls orienting responses, while the uncrossed pathway mediates defense-like behavior. To overcome the limitation of these classical studies and explicitly dissect the structure and function of these two pathways, we performed selective optogenetic activation of each pathway in male mice with channelrhodopsin 2 (ChR2) expression by Cre driver using double viral vector techniques. Brief photostimulation of the crossed pathway evoked short l…

Locked-in, walked out.

Locked in syndrome is typically associated with significant morbidity and mortality. We report a patient who had an unusually good recovery from locked in syndrome due to pontine infarction. The good recovery exhibited by our patient may have resulted from resolution of oedema at the site of infarction and brainstem plasticity being augmented by initial supportive measures in the intensive care unit and early, intensive rehabilitation.
