OBJECTIVES: To study the acute effects of radiotherapy after mastectomy and immediate transverse rectus abdominis myocutaneous (TRAM) flap reconstruction in breast cancer patients treated at Ramathibodi Hospital.

The subject of this work was to compare the effect of Conventional and Conformal techniques, used for anal canal cancer treatments, on the skin dose deposition. Skin dose was measured on a Rando phantom using XR-T GAFCHROMIC film. Local radiotherapy of a tumor is frequently applied in combination with chemotherapy or surgical measures. Possibly occurring toxicity must therefore be considered within the overall context. The radiosensitivity of the various organs is dependent on the sensitivity of the respective stem or mature end cells, and shows considerable differences within the individual cell systems. Apart from the application of prophylactic procedures, some radiation sequelae can be ameliorated by initiating specific measures (e.g. cosmetic products, drug treatment).

The temporary acute reactions during radiotherapy are a burden for the patient. These reactions are due to a depletion of stem cells and depend on the single dose, the total dose and the irradiated volume. The WHO describes the acute changes in a graded scale from 0 to IV. A knowledge of the course of these events helps the otolaryngologist to inform patients competently, calm their fears and give support during treatment. Efficacious measures to alleviate symptoms are presented.

Fractionated irradiation with X-rays or megavoltage is an important therapeutic option in the palliative treatment of Kaposi's sarcoma. We report on three HIV patients demonstrating bullous cutaneous eruptions in the irradiation field. Three homosexual men (age 45-60 y) stage of disease CDC 3C, were treated with fractionated soft X-rays with the scheme conventionally used by us (total dose 30 Gy, single doses 3 Gy). Immediately to three days after completion of treatment large blisters appeared within the irradiation field, resolving after 2-4 weeks, leaving post-inflammatory hyperpigmentation

OBJECTIVE: To determine the distribution and amount of elastic fibers in the dermis of clinically normal dogs and dogs with dermatoses, particularly solar dermatitis.
DESIGN: Skin specimens from 7 anatomic sites were obtained from 19 clinically normal dogs after euthanasia to evaluate the normal distribution of elastic fibers. Biopsy specimens also were obtained from 34 dogs with dermatoses, including 16 with solar dermatitis. Tissue sections were stained with H&E, Verhoeff-van Gieson, and periodic acid-Schiff.

Dermatologic therapy with ionising radiation has changed in the last 30 years. Adapted to the limitations of the area of indication, the improved radiotechnique, as well as the stricter working conditions, it is still today an important therapeutic method. As there are to be expected increased numbers of malignant diseases of the skin, radiotherapy will be an essential part of dermatologic tumor therapy in the near future.

Reported are the results of soft X-ray therapy of 237 patients with 245 basal cell carcinomas of the eye lids. The medial angle of the eye and the lower eye lid were mostly affected. Histologically in 62% solid basal cell carcinomas and in 14% sclerodermiform basal cell carcinomas could be diagnostized. By soft X-ray therapy 92% of all basal cell carcinomas could be healed. All recurrences were observed in the first 2 years following radiotherapy. They were relatively more frequent at the lateral angle of the eye. Recurrences occurred in 4.9% of female patients, and in 13.0% of male patients.

A case is reported of a 73-year old female patient in whom precancerous symptoms appeared in the mid-abdominal area of application which for a long time were mistaken for chronic eczema, until after 7 years the development of a skin carcinoma was clinically and histologically confirmed. The case provides a reason to draw attention to the "Radium Cushion" and to restate publicly that they should be delivered to the health authorities. Suspected cases must be thoroughly investigated medically.

Experiences with 30 latissimus dorsi flaps are described. Used either as a muscle flap, as a myocutaneous flap, or as an "island" of skin nourished by a subcutaneous pedicle of muscle and vessels, the flap has an excellent blood supply and is suitable for many repairs of defects of the chest wall.
