Exploring the role of transcranial direct current stimulation (tDCS) for the treatment of fibromyalgia: a narrative review.

INTRODUCTION: Fibromyalgia is a chronic pain syndrome characterized by widespread nociplastic pain, fatigue, sleep disturbances, and mood symptoms. Conventional treatments often yield limited benefits, prompting interest in noninvasive brain stimulation techniques such as transcranial direct current stimulation (tDCS). AREAS COVERED: This review synthesizes evidence from 31 randomized controlled trials (RCTs) evaluating the efficacy, safety, and tolerability of tDCS in fibromyalgia. A systematic search of Embase and Medline were conducted without date or language restrictions. Most studies as

Suction Ureteral Access Sheaths for Renal Stone Treatment Are Essential for Every Case: Con.

A flexible suction ureteral access sheath (FS-UAS) is not essential for every endoscopic renal stone treatment. To date, endoscopic combined intrarenal surgery (ECIRS) is the most efficient approach for treating any renal stone. Rigorous randomized controlled trials comparing retrograde intrarenal surgery with an FS-UAS versus ECIRS should be performed before superiority or even equivalence can be claimed.

It is still a secret disease - clinical images in randomized controlled trials on hidradenitis suppurativa.

PURPOSE: Hidradenitis suppurativa (HS) is a recalcitrant, chronic, inflammatory skin disease characterized by painful nodules, abscesses, and tunnels, primarily affecting intertriginous areas. Despite its substantial negative impact on patients' quality of life, HS has been historically understudied. Recently, an increasing number of randomized controlled trials (RCTs) have expanded treatment options and informed clinical guidelines. While quantitative data using clinical scoring systems is standard to disseminate RCT results, this study aimed to assess the extent of clinical image disseminati

Personalized Feedback Interventions for Indicated Prevention of Gambling Disorder: A Systematic Review and Meta-Analysis.

While personalized feedback interventions (PFIs) are a promising indicated prevention strategy for reducing gambling behavior and gambling harm immediately following intervention, no meta-analysis has examined the durability of these reductions. The current systematic review and meta-analysis aimed to examine the effect of PFIs immediately following the intervention and in the months following the intervention. The Cochrane Central Register of Controlled Trials, Embase, PsycINFO, PubMed were searched in January 2024 to identify randomized controlled trials comparing PFIs to standard care or in

Medical Management Versus PCI in Patients with Stable Angina: An Updated Systematic Review and Meta-analysis.

BACKGROUND: Despite the clinical guidelines favoring medical therapy for stable angina, the optimal management of these patients remains unclear. Here, we compare medical management (MM) versus percutaneous coronary intervention (PCI) in patients with stable angina. METHODS: An extensive literature search was conducted using PubMed and Embase to identify randomized controlled trials of interest. Data were extracted and analyzed using a random-effects model to calculate pooled odds ratios (OR). RESULTS: Our meta-analysis of 28 randomized controlled trials included 9346 PCI patients and 9503 m

Exercise as a transdiagnostic intervention for improving mental health: An umbrella review.

Exercise is beneficial for mental health in general, but no review has systematically assessed its potential transdiagnostic nature, i.e. whether it is beneficial across specific disorders. We performed a systematic umbrella review of meta-analyses of randomized controlled trials (RCTs) of exercise in participants with mental disorders defined according to the International Classification of Diseases (ICD) or the Diagnostic and Statistical Manual of Mental Disorders (DSM), assessing exercise's transdiagnostic nature with TRANSD criteria, including eight meta-analyses (six included in the TRANS

Time to Change the Drug Trial Paradigm for Dravet Syndrome: Designing Trials for the Disease Rather than Squeezing the Disease into the Trial.

Drug trial design is at an inflection point in epilepsy and neurological disorders. We are moving from conventional parallel-group randomized controlled trials (RCTs) to other designs, such as n-of-1 trials. Historically, inclusion criteria for antiseizure medicine (ASM) RCTs have stipulated a minimum number of seizures in the baseline period to judge ASM efficacy. For Dravet syndrome, RCTs have demanded 4 or more seizures/month in the baseline period. However, only 25% of children have sufficiently frequent seizures to qualify for RCTs. Trial outcomes should be tailored to the disease and lon

CONSORT statement adherence and risk of bias in randomized controlled trials on deep caries management: a meta-research.

BACKGROUND: Recently, trials have supported changes in deep caries management. However, reporting might lack details, affecting interpretation and implementation. Thus, we aimed to evaluate the adherence to the CONSORT statement and the risk of bias of randomized controlled trials (RCTs) on deep caries management published in pediatric dental journals. METHODS: We searched PubMed for RCTs in six pediatric dental journals between 2010 and 2022, focusing on deep caries lesion management. Adherence to the CONSORT guideline and the risk of bias were assessed using a modified tool with 19 items; e

A systematic review of sample size estimation accuracy on power in malaria cluster randomised trials measuring epidemiological outcomes.

INTRODUCTION: Cluster randomised trials (CRTs) are the gold standard for measuring the community-wide impacts of malaria control tools. CRTs rely on well-defined sample size estimations to detect statistically significant effects of trialled interventions, however these are often predicted poorly by triallists. Here, we review the accuracy of predicted parameters used in sample size calculations for malaria CRTs with epidemiological outcomes. METHODS: We searched for published malaria CRTs using four online databases in March 2022. Eligible trials included those with malaria-specific epidemio
