A novel manual therapy approach in chronic coccygodynia: a prospective case series.

BACKGROUND/PURPOSE: Coccygodynia, characterized by pain in the coccyx region, affects a significant portion of individuals with spinal discomfort. However, a universally accepted treatment remains undefined. This prospective case series aimed to evaluate the effectiveness of a combined approach using myofascial physical therapy (MPT) and Instrument Assisted Soft Tissue Mobilization (IASTM) in treating chronic coccygodynia. CASE DESCRIPTION: This study was a prospective case series. Patients referred for chronic coccygodynia to our physiotherapy clinic between September 2022 and December 2023

Endoscope-assisted Transoral resection of an elongated fractured styloid process in Eagle syndrome: A case report.

INTRODUCTION AND IMPORTANCE: Eagle's syndrome is a rare condition associated with the elongation of the styloid process or calcification of the stylohyoid ligament; it is clinically characterized by unexplained referred pain that radiates into the mandible, ear, and throat, often misdiagnosed as various cervicocraniofacial disorders. CASE PRESENTATION: A 38-year-old male presented with severe, unexplained neuralgic pain that persisted for about two months. The pain worsened with patient neck movements, and some relief was achieved only through lidocaine injections in the tonsillar bed. After …

Barodontalgia during hyperbaric oxygen therapy of an 8-year-old male: A case report.

Barodontalgia, barometric pressure-induced dental pain, may occur during hyperbaric oxygen(HBO2) therapy due to pressure changes. This case report presents an 8-year-old male patient with barodontalgia. The patient declared a severe toothache during HBO2 therapy. The diving medicine specialist referred the patient to the dental clinician immediately. On clinical examination, the pain was thought to be caused by caries lesions of the deciduous teeth in the left maxillary molar region. Tooth extraction was suggested. After extraction, the patient continued hyperbaric oxygen therapy sessions with

Effectiveness of Early Physical Therapy Rehabilitation in Patient With Juvenile Rheumatoid Arthritis.

A diverse group of idiopathic inflammatory arthritis that affects children under the age of 16 and lasts six weeks or longer is known as juvenile idiopathic arthritis (JIA) or juvenile rheumatoid arthritis (JRA). It causes joint pain and morning stiffness, and the affected joints swell and become difficult to move. During flares, patients may experience flu-like symptoms such as muscle aches and fatigue. Disease-modifying antirheumatic drugs (DMARDs) are the mainstay of rheumatoid arthritis (RA) treatment. If necessary, immunosuppressive medications can be administered in stages. Nonsteroidal

Successful management of segmental zoster paresis on the upper arm with temporally burst spinal cord stimulation.

Segmental zoster paresis (SZP) of the limbs is characterized by a focal, asymmetric neurogenic weakness that may occur in an extremity affected by herpes zoster (HZ). In this case report, we describe the case of a patient with SZP who presented with these problems and responded well to temporary spinal cord stimulation (SCS) and systematic rehabilitation. A 62-year-old female patient was referred for right upper limb pain, weakness, and insomnia due to pain. After completing the 14-day trial stimulation, the pain numerical rating scale of the patient in the right upper extremity decreased from

Skin Infections and Outpatient Burn Management: Outpatient Burn Management.

Burns, whether caused by thermal, chemical, or electrical exposure, are common and often preventable. Burn injuries are most common in children. All patients with burns should undergo primary and secondary assessment, including assessment of airway, breathing, and circulation. Evaluation of the location, size, and depth of burns can help to determine the optimal setting for management. Patients with full-thickness burns, circumferential burns, or burns on the face, hands, feet, genitals, or perineum should be referred to a burn subspecialist. Minor acute pain can be managed with irrigation of

[Management of adult abdominal pain in the Emergency Room].

Abdominal pain is a commonplace reason for surgical consultation in the emergency department and is the the most common symptom which the digestive surgeon on-call must evaluate. He must understand the pathophysiologic basis of visceral pain and referred pain in order to appreciate its diverse manifestations. Abdominal pain can stem from many causes intestinal and non-intestinal, medical and surgical. Evaluation and management in the emergency department must be rapid and pragmatic; clinical history and physical examination should define the gravity of the case, direct the first diagnostic pro…

Fibro-fatty nodules and low back pain. The back mouse masquerade.

BACKGROUND: Few useful interventions exist for patients with persistent low back pain. We suggest that a fibro-fatty nodule ("back mouse") m … relieved by multiple injection of the nodule. DISCUSSION: There is agreement that back mice exist. Referred pain from the nodules might explain the distant symptoms and signs in these cases. Multiple puncture may be an effective treatment because it lessens the tension of a fibro-fatty nodule. CONCLUSIONS: Randomized trials on this subject are needed. In the …

Back pain and collapse associated with receding subarachnoid blockade.

PURPOSE: Back pain and sudden loss of consciousness during recovery from spinal anesthesia are rare. Severe pain may raise fears of serious neurological damage and result in inappropriate management. Bladder distention may present in this way and clinicians should be alert to this possibility and communicate this to nursing staff. CLINICAL FEATURES: A lumbar spinal anesthetic was performed during a volunteer study, using 15 mg hyperbaric bupivacaine in a healthy 31 yr old man. During recovery, five hours later, as the block regressed to the L1-2 level, he complained of acute, severe back pain

[The rotator cuff syndrome--a frequent disease caused by loading].

The rotator cuff syndrome is defined as a cronical pain syndrome, originating from the rotator cuff with characteristic pain and referred pain. Disorders evolving from the rotator cuff and the rotator muscles comprise between 70 og 90% of the shoulder disorders. The rotator cuff syndrome may be of occupational origin, particularly in heavy work with tasks at or above shoulder level, e.g. shipyard welders and plate workers. Biomecanically surveys show that continuous blood flow in the rotator muscles ceases at about 30 degrees of flexion or abduction of the upper arm. Electromyographic examinat…

Our surgical experience in Arnold's neuralgia: Technical insights and clinical results.

BACKGROUND: Arnold's neuralgia, also referred to as greater occipital neuralgia, is a chronic neuropathic pain condition characterized by paroxysmal or continuous occipital pain. While most patients respond to conservative treatment, a subset develops medically refractory symptoms requiring surgical intervention. OBJECTIVE: To present our institutional experience with isolated surgical decompression of the greater occipital nerve, focusing on technical aspects, clinical outcomes, and patient satisfaction. METHODS: A retrospective review was conducted of 29 consecutive patients who underwent

Facet Joint Syndrome: Pathophysiology, Diagnosis, and Treatment.

Facet joint osteoarthritis (OA) is the most frequent form of facet joint syndrome. Medical history, referred pain patterns, physical examination, and diagnostic imaging studies (standard radiographs, magnetic resonance imaging, computed tomography and single-photon emission computed tomography) may suggest but not confirm lumbar facet joint (LFJ) syndrome as a source of low back pain (LBP). However, the diagnosis and treatment of facet joint syndrome is still controversial and needs further study. It is widely acknowledged that block with local anesthetic is perhaps the most effective method t…

Differential diagnosis of acute ocular pain: Teleophthalmology during COVID-19 pandemic - A perspective.

Ocular pain is a common complaint which forces the patient to seek immediate medical attention. It is the primeval first response of the body to any severe condition of the eye such as trauma, infections and inflammation. The pain can be due to conditions directly affecting the eye and ocular adnexa; or indirect which would manifest as referred pain from other organ structures such as the central nervous system. Paradoxically, there are several minor and non-sight threatening conditions, which also leads to ocular pain and does not merit urgent hospital visits. In this perspective, we intend t…

Painful legs and moving toes.

The syndrome of painful legs-moving toes (PLMT) is an adult-onset, rare disorder characterized by pain, typically of a neuropathic quality, in the feet or legs, associated with writhing movements of one or more toes. It is the pain which usually brings the patient to medical attention. The syndrome may be unilateral or bilateral. Identical toe movements may occur without pain, referred to as: "painless legs-moving toes," and a similar condition affects the upper limbs: "painful arms-moving fingers." The pathophysiology of PLMT and its variants is not known but most reports suggest an associati

[CT-guided celiac ganglion block. The authors' own experience].

Celiac plexus neurolysis is considered an effective technique for relieving abdominal cancer pain. CT yields accurate anatomical detailing, thus allowing the indirect location of the celiac plexus and the planning of needle trajectory and positioning. September 1992 to December 1993, twenty-eight celiac plexus neurolyses for pain relief were performed in 26 patients (13 men and 13 women), 17 through the anterior and 11 through the posterior access. These patients had been referred to our department for pain management. Forty-eight hours after alcoholization, 20 patients (71%) had complete pain

Hypercalciuria in children with urinary tract symptoms.

We performed this prospective study to determine the urinary calcium to creatinine ratio (Ca/Cr) in children with different urinary symptoms. We studied 523 children in our nephrology clinic with an age range of 3 to 14 years (mean= 8) and male to female ratio of 0.61. All the children had at least one of the urinary tract symptoms (dysuria, frequency, urgency, abdominal and/or flank pain, diurnal incontinence or enuresis), microscopic hematuria, urinary tract infection or urolithiasis. Fasting urine was collected for measuring calcium and creatinine and the results were compared to the values

Amylase normal, lipase elevated: is it pancreatitis? A case series and review of the literature.

OBJECTIVE: This study was undertaken to identify clinical scenarios in which the lipase is significantly elevated (three times above the upper limit of normal) but the amylase is normal, and to examine whether or not pancreatitis is the likely cause for this seemingly unusual constellation of laboratory results. METHODS: Twenty-five patients were retrospectively identified over a 2-yr period, which fulfilled the above criteria. A thorough review of their charts was conducted. In addition, a critical review of the literature was performed. RESULTS: It appears that isolated elevation of lipase
