BACKGROUND: While soft tissue sarcomas affect younger patients, few studies have assessed the distribution of underlying pathogenic germline variants.
PATIENTS AND METHODS: We retrospectively identified all pediatric and young adult patients (0-22 years) at Haukeland University Hospital, Norway (1981-2019), through clinical and pathological records. Most of tumours of the foot are tumour-like (synovial cyst, foreign body reactions and epidermal inclusion cyst) or benign conditions (tenosynovial giant cells tumours, planta fibromatosis). Primary bone tumors in children are almost exclusively represented by osteosarcomas and Ewing sarcomas. They mainly affect the limbs, particularly the lower limbs. A 55-year-old white man with an extensive retrorectal tumor died after transrectal needle biopsy. The indications and methods for biopsy of tumors in this location are controversial and are not well delineated in the literature. When resection seems unlikely or risky, or if a neoplasm is suspected which could be treated by nonsurgical means, biopsy is indicated. Antibiotics with aerobic and anaerobic coverage should be given prophylactically. When CT scan or sigmoidoscopic findings suggest a necrotic tumor, the transrectal approach should be avoided.

The clinical results of roentgenographic examination in an eight-year-old girl with a large tumor in the first metatarsus suggested malignancy. The definitive pathologic diagnosis was atypical aneurysmal bone cyst. Massive bone grafting and preservation of the surrounding joints were successful, and a functional foot was obtained two years after operation.

Fifteen patients with proximal femoral tumors had resection and limb salvage with an uncemented Kotz (HMRS) megaprosthesis. There were five osteosarcomas, four chondrosarcomas, one hemangioendothelioma, three fibrosarcomas, and two Ewing's sarcomas. The mean follow-up was 6.7 (range 3-10) years. From 1987 to 1991, 26 patients with malignant bone tumors (osteosarcoma, Ewing's sarcoma, chondrosarcoma and malignant fibrous histiocytoma) in the diaphysis or metaphysis of long bones have been treated by chemotherapy, radiotherapy and intercalary bone allograft replacement after resection. Complete or partial trisomy of the long arm of chromosome #1 was observed in six patients with malignant disorders. Distal femur and proximal tibia are the main localization of primary malignant bone tumor. Osteosarcoma and Ewing sarcoma are the most frequent. Ifosfamide/mesna was given to 97 patients who had malignant solid tumors diagnosed before they were 21 years of age.
