[Obstructive sleep apnea-hypopnea syndrome].

We analyzed a cohort of 400 patients referred from the otorhinolaryngology department (40.05 %), primary care (PC) (36.52 %), and the pediatric pulmonary unit (17.63 %). The children were referred for clinical suspicion of apneas in 191 (47.87 %), snoring and apneas in 101 (25 %), and snoring in 87 (21.80 %). Adenotonsillar hypertrophy was found in 211 patients (52.75 %), tonsillar hypertrophy in 87 (21.75 %), and adenoid hypertrophy in 73 (18.25 %). All patients underwent respiratory polygraphy (RP) during sleep. Obstructive sleep apnea-hypopnea syndrome (OSAHS) was diagnosed in 298 patients …

Ethnicity and obstructive sleep apnoea.

There is a scarcity of adult prevalence studies of OSA outside the Caucasian populations of North America, Europe and Australia, and comparisons have been complicated by methodological differences in sleep study settings, respiratory events definition, measured risk factors and clinical outcomes, and the lack of objective parameters for the measurement of ethnicity itself. Comparing studies with the same methodological design and respiratory events definition, recent large-scale prevalence studies from Hong Kong, Korea and India show similar OSA rates to populations of mainly Caucasian composi…

Sleep apnoea syndrome in Nepal.

Sleep apnoea syndrome is a well recognized entity. Sleep studies of diagnosed patients with sleep apnoea were analyzed to understand the clinical profiles of Nepalese patients with this disorder. Eighty nine patients were diagnosed to have sleep apnoea syndrome during the period of 15 months starting from November 2003. All these patients were subjected to a detailed clinical evaluation including symptoms, presence or absence of other risk factors and co-morbidities, general physical and systemic examination and relevant investigations before performing polysomnography. Clinical profiles of pa…

Sleep disturbance in anxiety disorders.

Many patients suffering from the majority of anxiety disorders complain about their sleep by reporting difficulties in initiating and maintaining it. Polysomnographic studies have shown that, in comparison to normal subjects, the sleep of patients with panic disorder is characterized by longer sleep latency, increased time awake and reduced sleep efficiency. Sleep architecture is normal and there are no significant changes in REM sleep measures. Nocturnal panic attacks are non-REM-related events and occur without an obvious trigger in 18-45% of panic disorder patients. Regarding generalized an…

Sleep disorders and neuromuscular disease.

Breathing is the most important executive function of sleep. Compromise of the ventilatory mechanism is the principal consequence of a neuromuscular dysfunction in the individual who is asleep. This includes alterations of the lower motor neuron, the neuromuscular junction, and muscle. The primordial phenomenon in rapid eye movement (REM) sleep respiratory physiology is that the diaphragm assumes all functions of respiration. Any dysfunction of the diaphragm, whether neurogenic or neuromuscular, will interfere with breathing during REM sleep. So prevalent are sleep respiratory difficulties in …

Shiftwork: safety, sleepiness and sleep.

This brief paper reviews the available published literature on shiftwork and safety that allows the relative risk of "accidents" or injuries … not entirely consistent with predictions derived from considerations of the circadian variations in sleep propensity or rated sleepiness, and consider factors relating to sleep that may underlie the observed trends in risk. Finally, the practical implications of the trends in risk for the design of safer shift systems are discussed.

Sleep disorders in multiple sclerosis.

Sleep disorders are pervasive in patients with multiple sclerosis (MS) although clinically underrecognized by most physicians. The most common sleep disorders seen in patients with MS include insomnia, nocturnal movement disorders, sleep-disordered breathing, narcolepsy, and rapid eye movement sleep behavior disorder. Factors that influence the quality of sleep in this patient population include pain, nocturia, depression, medication effect, location of lesions, and disease severity. Disrupted sleep has the potential to cause daytime somnolence, increased fatigue, and nonrefreshing sleep, and …

[Methadone and sleep apnea syndrome].

BACKGROUND: Sleep apnea syndrome occurs when, during sleep, breathing stops for 10 seconds or longer, with an index of 5 times or more an hour. It is clinically characterized by loud snoring at night, continuous or interrupted by pauses followed by loud breathing. Sleep is fitful, broken by arousals, and yields little rest. There is daytime excessive sleepiness with repeated involuntary falling asleep, often unknown by the subject. CLINICAL DESCRIPTION: In this article, we describe an observation of central sleep apnea syndrome in a female patient receiving an opiate replacement therapy. MET…

Sleep investigations in erectile dysfunction.

Regarding the aetiology of erectile dysfunction, beside numerous organic causes in many cases psychological factors play an important role. The pathophysiological mechanisms underlying the so-called psychogenic erectile dysfunction are not yet understood. Based on a neurobiological approach to psychogenic erectile dysfunction, polysomnographic investigations were carried out with the aim of identifying possible alterations of the sleep EEG. 24 male patients with psychogenic erectile dysfunction without detectable organic factors or other mental disorders were studied in the sleep laboratory, a…

Sleep disordered breathing following stroke.

In recent years there has been increasing interest in the relationship between obstructive sleep apnoea and stroke. It is clear that many patients who have had a stroke have marked obstructive sleep apnoea. This is seen during recovery but also during the acute phase when transient hypoxaemia and the blood pressure swings associated with upper airway obstruction, may worsen the ischaemic penumbra of the area of the brain which is compromised, leading to a worse outcome. There is some evidence to support this hypothesis. This article explores these issues.

Sleep homeostasis in infant rats.

Homeostatic regulation is a defining characteristic of sleep but has rarely been examined in infants. This study presents an automated method of sleep deprivation in which 5-day-old rats were shocked whenever the nuchal muscle became atonic. The intensity of shock was always set at the minimal level required to maintain arousal. Deprived pups exhibited rapid increases in sleep pressure, as evidenced by increased attempts to enter sleep and subsequent increases in sensory threshold; this increased sensory threshold was not due to sensory adaptation of peripheral receptors. In addition, myocloni…

Neuroendocrine changes in sleep apnea.

PURPOSE OF REVIEW: To review recent investigations examining the effects of neuroendocrine changes in obstructive sleep apnea. RECENT FINDINGS: Gonadal hormones have long been implicated in the pathogenesis of obstructive sleep apnea. Recently, exogenous testosterone has been shown to exacerbate obstructive sleep apnea, whereas hormone replacement therapy in menopausal women may be protective in obstructive sleep apnea. Effective treatment of obstructive sleep apnea with nasal continuous positive airway pressure has been associated with improved insulin sensitivity and testicular function in …

Sleep deprivation in honey bees.

Rest at night in forager honey bees (Apis mellifera) meets essential criteria of sleep. This paper reports the effect of a 12-h total sleep deprivation (SD) by forced activity on the behaviour of these animals. The behaviour of sleep-deprived animals is compared with that of control animals under LD [periodic alternation between light (L) and darkness (D)] 12 : 12 hours. SD for 12 h during the first D period resulted in a significant difference with respect to the parameter 'hourly amount of antennal immobility' between sleep-deprived and control animals during the remaining L and D periods. T…

Age, performance and sleep deprivation.

Young subjects are frequently involved in sleep-related accidents. They could be more affected than older drivers by sleep loss and therefore worsen their driving skills quicker, or have a different perception of their level of impairment. To test these hypotheses we studied variations of reaction time (RT), a fundamental prerequisite for safe performing, as measured by lapses, i.e. responses > or = 500 ms and self-assessment of performance and sleepiness after a night awake and after a night asleep in a balanced crossover design in young versus older healthy subjects. Ten young (20-25 years o…

[Sleep apnea syndrome in children].

Obstructive sleep apnea is characterized by prolonged, generally partial, upper airways obstruction associated with hypoxemia and/or hypercapnia. Main etiological factors include hypertrophy of the tonsils and adenoids. Polysomnography is the gold standard for diagnosis. Diagnosis of obstructive sleep apnea must be established early in order to prevent complications. Adenotonsillectomy is an effective therapy.
