Narcolepsy and hypersomnia: review and classification of 642 personally observed cases.

In this paper the author gives a survey and a classification of 642 cases of narcolepsy and hypersomnia which he himself studied in the course of 26 years. 368 cases were classified as narcolepsy, 274 as hypersomnia. The author further classifies narcolepsies according to their etiology, clinical form and pathophysiological mechanisms of origin. Hypersomnias are divided by the author into the symptomatic and the functional groups. According to the author it is useful to distinguish "short cycle hypersomnia", i.e. those with short duration of sleep attacks (hours) and intervals, from "long cycl

Microsleep during a simplified maintenance of wakefulness test. A validation study of the OSLER test.

Daytime somnolence is both a symptom in many patients and a prevalent complaint in the general population, but its objective assessment remains elusive. The current available tests are technically complex and thus inadequate for routine clinical use or epidemiological studies. A simplified behavioral maintenance of wakefulness test (OSLER test) has been recently described that could allow for widespread availability of objective measurements of this symptom. We verified the occurrence of (micro)sleep (episodes of sleep of > or = 3 s duration) during the performance of the OSLER test in 10 norm

Sleep and temporal lobe epilepsy: a case study with depth electrodes.

We studied a patient with hippocampal epilepsy who had frequent nocturnal and diurnal seizures. Depth electrode recording showed that focal seizure discharges in the right hippocampus were of shorter duration in REM and non-REM sleep. However, awakening, especially from REM sleep or shortly after a REM period, facilitated the occurrence of a generalized seizure. There was no ultradian fluctuation in frequency or duration of seizure during diurnal recording. Night terrors (but not nightmares) disappeared after right temporal lobectomy, suggesting that pavor nocturnus was an ictal manifestation

Non-pharmacological interventions for sleep disruptions and fatigue after traumatic brain injury: a scoping review.

OBJECTIVE: The aim of this study was to conduct a scoping review to determine the nature, variety, and volume of empirical evidence on nonpharmacological interventions for sleep disturbances with potential implications for fatigue in adults sustaining a traumatic brain injury (TBI). METHODS: A systematic literature search was conducted across four databases to identify primary studies testing a single non-pharmacological intervention or a combination of non-pharmacological interventions for sleep disturbances and fatigue in community-dwelling adults with TBI. RESULTS: Sixteen studies were re

Application of Noninvasive Brain Stimulation for Sleep Quality Enhancement and Cognitive Improvement.

Sleep is a fundamental process for maintaining our physical and mental health by adjusting brain homeostasis and repairing axons to refresh memories. Due to its essentiality, sleep disorders and insufficiency can cause both physiological and behavioral risks. This report diverges from traditional medical treatments and focuses mainly on physiotherapy-based neuroregulatory techniques for sleep treatment. Noninvasive brain stimulation (NiBS) techniques have been developed to enhance patients' sleep and memory, including transcranial electrical stimulation (tES), transcranial magnetic stimulation

Sleep quality and architecture in Idiopathic generalized epilepsy: A systematic review and meta-analysis.

Idiopathic generalized epilepsies are a group of sleep related epilepsy syndromes with sleep deprivation as a strong trigger for seizures and increased spike-wave activity during sleep and transition to sleep. Neuropsychological deficits are common in Idiopathic generalized epilepsy patients. Learning and memory processes are closely linked to sleep. Therefore, this systematic review and meta-analysis investigates the evidence of sleep disturbances in Idiopathic generalized epilepsy patients. A search of the databases EMBASE, Medline and Scopus identified 22 studies comparing polysomnographic

Sleep in children with cystic fibrosis: More under the covers.

Cystic fibrosis (CF) is a chronic multisystem disease with manifestations from birth. It involves the entire respiratory system, with increased cough, and recurrent pulmonary infections, and it also leads to intestinal malabsorption, all of which can have an impact on sleep. In this review, we summarize the available literature on the various sleep disturbances in children with CF. Sleep quality and sleep efficiency are often impaired in children with CF. They may be accompanied by symptoms associated with sleep-disordered breathing (SDB), and objective findings, such as nocturnal hypoxemia. I

Nocturnal Gastroesophageal Reflux Disease (GERD) and Sleep: An Important Relationship That Is Commonly Overlooked.

Gastroesophageal reflux disease (GERD) is a prevalent, chronic medical condition that affects 13% of the adult population globally at least once a week. Sleep disturbances are frequently encountered in up to 25% of the GERD patients, likely due to nocturnal gastroesophageal reflux (GER). With advance in diagnostic techniques allowing for an improved understanding of involved physiological mechanisms of nocturnal reflux, there is growing evidence of a bidirectional relationship between GERD and sleep disturbances. Furthermore, nocturnal GER is associated with more complicated GERD. Obstructive

Dreams and nightmares in healthy adults and in patients with sleep and neurological disorders.

Dreams are experiences that occur during sleep, while we are disconnected from the environment. Thanks to recent progress in neuroimaging techniques, it is now becoming possible to relate dream features to specific patterns of brain activity. Some conditions occurring in patients with neurological disorders, such as lucid dreams and parasomnias, not only have diagnostic value, but also offer a window into the dream process. They show that dreaming is reflected in physiological signals, behaviours, and brain activity patterns, and that the body can enact dream content. Yet, the dream body can a

[Dental sleep medicine: A further introduction to an emerging dental discipline].

Dental sleep medicine is a discipline traditionally focusing on sleep-breathing disorders like snoring and obstructive sleep apnea. In everyday clinical practice, however, such disorders rarely occur in isolation. Rather, they frequently co-occur with other sleep disorders linked to dentistry, such as orofacial pain, dry mouth, hypersalivation, gastroesophageal reflux, and sleep bruxism. Therefore, a new definition has been formulated in which dental sleep medicine has been described to include the diagnostic and treatment aspects of all dental sleep disorders. In this article, the reasons why

Medical factors of falling asleep behind the wheel.

Drivers' sleepiness and falling asleep while driving account for a considerable proportion of vehicle accidents (studies show different results from 1% to 30%). Sleepiness is rarely well recognised as a causing factor of traffic accidents. 2.5% up to 20% people suffer from excessive daytime sleepiness (EDS) with sleep deprivation as its most frequent cause. There is a strong association between sleep deprivation and medical problems--especially sleep disturbances. The sleep apnoea syndrome (SAS) has been identified as the most common cause of habitual drowsy driving. Patients with SAS (apart f

Excessive twitch movements in rapid eye movement sleep with daytime sleepiness.

A man who showed excessive twitch movement, such as fragmentary myoclonus (FM) and periodic movements in sleep (PMS) predominantly during REM sleep, is reported. He complained of excessive daytime sleepiness (EDS). After examination, his twitch movements were shown not to accompany narcolepsy, and his EDS were considered to originate from nocturnal sleep disturbance caused by FM and PMS.

Phase-Amplitude Coupling in Theta and Beta Bands: A Potential Electrophysiological Marker for Obstructive Sleep Apnea.

BACKGROUND: Phase-amplitude coupling (PAC) between the phase of low-frequency signals and the amplitude of high-frequency activities plays many physiological roles and is involved in the pathological processed of various neurological disorders. However, how low-frequency and high-frequency neural oscillations or information synchronization activities change under chronic central hypoxia in OSA patients and whether these changes are closely associated with OSA remains largely unexplored. This study arm to elucidate the long-term consequences of OSA-related oxygen deprivation on central nervous

Causes and consequences of age-related steroid hormone changes: insights gained from nonhuman primates.

Similar to humans, rhesus macaques (Macaca mulatta) are large, long-lived diurnal primates, and show similar age-related changes in the secretion of many steroid hormones, including oestradiol, testosterone, cortisol and dehydroepiandrosterone (DHEA). Consequently, they represent a pragmatic animal model in which to examine the mechanisms by which these steroidal changes contribute to perturbed sleep-wake cycles and cognitive decline in the elderly. Using remote serial blood sampling, we have found the circulating levels of DHEA sulphate, as well as oestradiol and testosterone, decline markedl

Quantitative Electroencephalographic Analysis Provides an Early-Stage Indicator of Disease Onset and Progression in the zQ175 Knock-In Mouse Model of Huntington's Disease.

STUDY OBJECTIVES: Patients with Huntington's disease (HD) show a high prevalence of sleep disorders that typically occur prior to the onset of motoric symptoms and neurodegeneration. Our understanding of the pathophysiological alterations in premanifest HD is limited, hindering the ability to measure disease modification in response to treatment. We used a full-length knock-in HD model to determine early changes in the electroencephalogram (EEG) and sleep that may predict the onset and progression of the disease. METHODS: A 10-month longitudinal study was designed to determine the effect of t

Higher frontal EEG synchronization in young women with major depression: a marker for increased homeostatic sleep pressure?

STUDY OBJECTIVES: Major depressive disorder (MDD) is often associated with disturbances in circadian and/or sleep-wake dependent processes, which both regulate daytime energy and sleepiness levels. DESIGN: Analysis of continuous electroencephalographic (EEG) recordings during 40 h of extended wakefulness under constant routine conditions. Artifact-free EEG samples derived from 12 locations were subjected to spectral analysis. Additionally, half-hourly ratings of subjective tension and sleepiness levels and salivary melatonin measurements were collected. SETTING: Centre for Chronobiology, Psy

Genome-wide gene by environment study of time spent in daylight and chronotype identifies emerging genetic architecture underlying light sensitivity.

STUDY OBJECTIVES: Light is the primary stimulus for synchronizing the circadian clock in humans. There are very large interindividual differences in the sensitivity of the circadian clock to light. Little is currently known about the genetic basis for these interindividual differences. METHODS: We performed a genome-wide gene-by-environment interaction study (GWIS) in 280 897 individuals from the UK Biobank cohort to identify genetic variants that moderate the effect of daytime light exposure on chronotype (individual time of day preference), acting as "light sensitivity" variants for the imp

Heat of the night: sleep disturbance activates inflammatory mechanisms and induces pain in rheumatoid arthritis.

Sleep has a homeostatic role in the regulation of the immune system and serves to constrain activation of inflammatory signalling and expression of cellular inflammation. In patients with rheumatoid arthritis (RA), a misaligned inflammatory profile induces a dysregulation of sleep-wake activity, which leads to excessive inflammation and the induction of increased sensitivity to pain. Given that multiple biological mechanisms contribute to sleep disturbances (such as insomnia), and that the central nervous system communicates with the innate immune system via neuroendocrine and neural effector
