Otologic manifestations of Langerhans' cell histiocytosis.

Eighteen of 62 children diagnosed with Langerhans' cell histiocytosis at the Children's Hospital of Pittsburgh (Pa) between 1970 and 1986 demonstrated ear and temporal bone involvement. In six children, such otologic disease was their sole presenting manifestation. Common signs and symptoms included aural discharge, postauricular swelling, and conductive hearing loss. The otologic findings in these children, if not investigated properly, could easily be attributed to acute or chronic infectious ear disease. Computed tomography with contrast enhancement proved to be particularly valuable as a d

Construction of virtual airway model to assist surgical correction of velopharyngeal insufficiency with posterior pharyngeal flap.

OBJECTIVE: Posterior pharyngeal flap (PPF) is one of the most common surgical technique to correct velopharyngeal insufficiency(VPI), during which controlling the sizes of the lateral pharyngeal ports(LPP) is the key to outcomes. One innovative procedure was developed to well control the size of LPP. MATERIALS AND METHODS: 40 patients with repaired cleft palate were collected from June 2022 to August 2023. All patients were diagnosed with VPI, and treated with modified PPF surgery. For each patient, upper airway model was reconstructed, and the virtual airway model of PPF was designed. The na

Machine learning surrogate forward models for biomechanical laryngeal control.

Accurate modeling of laryngeal motor control is key to understanding typical and disordered voice production. Traditional biomechanical plant models based on ordinary differential equations can be computationally demanding in closed-loop control settings, particularly when repeatedly initialized and evaluated over dense parameter grids. Runtime depends strongly on solver implementation, hardware, initialization strategy, and numerical optimization. This study evaluates feature-driven machine learning (ML) regressors, specifically random forest (RF), multilayer perceptron neural networks (NN),

Staged treatment with arterial reconstruction and flap transplantation for ischemic mangled extremity: An observational study.

Patients with traumatic ischemic mangled extremities first undergo arterial reconstruction using autogenous vein grafts, followed by flap transplantation as a staged treatment for soft tissue reconstruction. This study aimed to report the outcomes of such a staged treatment. Thirteen patients underwent arterial reconstruction between February 2015 and April 2017 due to damage to the major blood vessels by the traumatic mangled extremities. Of them, 6 patients (5 males and 1 female with a mean age of 51 years, age range: 36-60 years) who underwent soft tissue reconstruction due to necrosis were

Reasoning in machine vision by learning fast and slow thinking.

Reasoning is a hallmark of human intelligence, enabling adaptive decision-making in complex unfamiliar scenarios. In contrast, machine intelligence remains bound to training data, unable to dynamically refine solutions at inference. While recent advances have explored machine reasoning - trading inference-time compute for improved performance - they focus on verbal domains such as mathematical problem-solving where explicit rules govern step-by-step solution generation. Many tasks lack sufficient labelled data and require alternative performance improvement mechanisms, such as inference-time c

A bioinspired photoelectrochemical synapse for neurotransmitter-mediated in-sensor computing.

Photoelectrochemistry (PEC) presents an emerging paradigm for aqueous neurosynaptic emulation. Here, a bioinspired PEC synapse integrating optical perception and biosensing of real neurotransmitter glutamate (Glu) is devised for visuomorphic in-sensor computing. Short-term and spiking-dependent plasticity with Glu-tunable excitatory and inhibitory signals are realized, making possible Glu-tunable convolutional kernel for in-sensor preprocessing and convolutional neural network. This work achieves PEC in-sensor processing and real neurotransmitter-mediated visuomorphic computing for the first t

Intelligent fire detection in agriculture using machine learning and embedded systems for risk prevention and improved sustainability.

This research proposes the design of an autonomous fire detection system based on a Raspberry Pi 3 B+, combined with smoke and flame sensors, enabling real-time monitoring and increased reactivity to fire hazards. To improve detection accuracy and risk prediction, Machine Learning (ML) algorithms, including Random Forest and Linear Regression, were applied to classify hazard levels and anticipate critical situations. The implementation of these models has enabled accurate classification of risk levels, guaranteeing real-time alerts and rapid decision-making. In addition, anomaly detection tech

Management considerations of ameloblastic carcinoma in soft-tissue: case report and literature review.

BACKGROUND: Ameloblastoma is a benign yet locally aggressive odontogenic neoplasm characterized by a significant propensity for local recurrence. Soft-tissue recurrence following mandibular resection and reconstruction represents a distinct clinical entity, often posing diagnostic and therapeutic challenges. In contrast, ameloblastic carcinoma may arise de novo or through malignant transformation of a pre-existing ameloblastoma, reflecting a spectrum of biological behaviour. We report a case of post-resection soft-tissue recurrence with subsequent malignant transformation, underscoring the unp

Well-Differentiated Extraosseous Chondrosarcoma Arising of Gall Bladder: A Detailed Case Report.

This article reports a rare case of extraosseous myxoid chondrosarcoma originating from the gallbladder. The patient, a 50-year-old female, presented with recurrent right upper quadrant abdominal pain for over two months, with worsening symptoms in the month prior to admission. Before hospitalization, enhanced computed tomography (CT) of the upper abdomen performed at a local hospital suggested a malignant gallbladder tumor (gallbladder cancer) with lymphadenopathy near the hepatic portal, as well as chronic cholecystitis with gallstones. Upon admission, whole-abdomen enhanced CT and liver-enh

Immune microenvironment of intimal sarcomas: Adaptive immune resistance with potential therapeutic implications.

OBJECTIVES: Intimal sarcomas are rare, aggressive neoplasms that arise from large blood vessels. Characterization of the tumor immune microenvironment may suggest new treatment strategies. METHODS: Seventeen specimens from 7 patients were labeled by immunohistochemistry for programmed cell death 1 ligand 1 (PD-L1), CD45, CD8, CD4, FOXP3, CD20, CD68, and LAG3. The immune cell density was scored as a percentage of the tumor area (1+ [<5%], 2+ [5%-50%], 3+ [>50%]); PD-L1 expression was scored on tumor cells and on intratumoral immune cells. Immune marker density was quantified using image analys

Unrecognized difficult airway management during anesthesia in two brachycephalic dogs with narrow cricoid cartilage.

Difficulty in airway management during anesthesia was noted in a 10-year-old, castrated, male Pekingese dog and a 13-year-old male French Bulldog. They showed strong resistance during tracheal tube insertion through the subglottic lumen. Therefore, the airway was secured by using a small endotracheal tube or supraglottic airway device. Computed tomography scan revealed a markedly narrower vertical dimension of the cricoid cartilage compared to that seen in common brachycephalic breeds. Posterior glottis was relatively more accessible for translaryngeal intubation in the present cases. Our find

The first case of acute myeloid leukaemia/myeloid sarcoma with cytokeratin expression on blasts diagnosed on urine specimen.

Here we describe a rare case of renal myeloid sarcoma, first discovered incidentally on routine urine analysis, which is one of the first of it's kind to be reported. We describe the vanishingly rare phenomenon of renal myeloid sarcoma presenting without haematological manifestation while also highlighting the often inevitable transformative nature of the disease. We also describe the unusual immunohistochemistry findings, in particular the cytokeratin expression, that is not usually typical of such cases. We briefly discuss use of PET-CT for the disease response monitoring. We summarize treat
