[A case of greater auricular nerve autologous nerve grafting for inferior alveolar nerve injury by mandibular fracture].

The results of peripheral nerve repair have been greatly improved in the last few years following the introduction of micro-surgery and increased application of free autologous nerve transplants. In the field of oral surgery, a rich experience has been made in plastic and reconstructive repair. The inferior alveolar nerve is endangered by a series of mandibular fractures, with fracture lines running along the nerve canal. For plastic repair of the inferior alveolar nerve, we interpose an autologous transplant from the greater auricular nerve.

Relative value unit impact of complex skin closures to academic surgical melanoma practices.

BACKGROUND: Melanoma excisions frequently are associated with significant soft-tissue defects, creating the need for complex closures. These closures could be performed by either surgical oncologists or plastic surgeons. We sought to quantify the relative value units (RVUs) and describe the practice patterns of 2 academic surgical subspecialties after a melanoma excision. METHODS: After institutional review board approval, a retrospective data analysis of a billing database was conducted on all melanoma patients undergoing an excision and closure by surgical oncology and plastic surgery depar

Plastic within renal pelvis on nephroscopy: a potential hazard during percutaneous nephrolithotomy.

The use of adhesive plastic drapes is common throughout laparoscopic and percutaneous surgery. We report a case in which a significant portion of the plastic drape was translocated into the renal pelvis during tract dilation for percutaneous nephrolithotomy. The complication was recognized only on endoscopy and the plastic successfully removed. The potential for morbidity in other patients in the future remains, and we recommend operators be aware of this unusual, but entirely preventable, complication. We highlight some measures to help avoid and check for this complication before the end of

Advances and Innovations in Microsurgery.

LEARNING OBJECTIVES: After reading this article, the participant should be able to: 1. summarize the evolution of perforator, chimeric, and free style flaps; 2. define and give examples of supermicrosurgery as well as understand its application in treatment of lymphedema; and 3. appreciate the development and advancements of composite tissue allotransplantation. SUMMARY: Although microsurgery may seem like a highly specialized niche within plastic surgery, it is more than just a discipline that focuses on small anastomoses. It is a tool and a way of thinking that allows us to embody the true

Outcomes of Upper Lip Mohs Surgery Repairs Using Standardized Scar Scales With Particular Emphasis on Vermillion Border Involvement.

BACKGROUND: The lips are a common location for skin cancer, and thus, a common site for Mohs micrographic surgery (MMS). As an important cosmetic and functional facial unit, MMS defects and reconstruction can affect patient perception on functional and aesthetic outcomes. OBJECTIVE: The objective of this study was to compare aesthetic and functional outcomes after upper lip MMS between patients with vermillion sparing repairs (VSR) versus vermillion crossing repairs (VCR). MATERIALS AND METHODS: Patients from a single institution from 2018 to 2022 undergoing MMS of the upper lip with linear

Expert Consensus on Navigation-guided Unilateral Orbital Fracture and Orbital Floor Reconstruction Techniques.

Orbital fracture and orbital floor reconstruction surgery is characterised by limited surgical exposure and high risks. Before the advent of digital technology, the design and implementation of the surgical scheme for orbital floor reconstruction surgery mainly depended on the intuitive understanding of imaging and the clinical experience of surgeons, with strong unpredictability and experience dependence. Surgical navigation systems with real-time positioning and imaging functions, when used in orbital reconstruction surgery, can detect the real-time intraoperative position, avoid risks, and

Guides through Transformation: Virgil, Starbuck, and the Ethical Role of the Plastic Surgeon.

Guidance during transformation is a recurring theme in literature and medicine. In Divine Comedy, Virgil successfully guides Dante through suffering toward understanding, whereas in Moby-Dick, Starbuck recognizes Captain Ahab's destructive obsession but fails to prevent catastrophe. This conceptual humanities analysis examines these contrasting literary figures through the perspective of plastic surgery ethics and explores their relevance to reconstructive and aesthetic practice. The literary roles of Virgil and Starbuck were comparatively interpreted using narrative and conceptual analysis. T

Hypospadias: The lay of the land.

INTRODUCTION: Hypospadias surgery is uniquely split across the specialities of paediatric surgery urology and plastic surgery. The division may restrict best practice as there is a paucity of cross-speciality forum for discussion and dissemination. Separately achieving training competence is a challenge. The authors present a travelling fellowship as a template future programme to address some of these issues. METHODS: A fellowship was established visiting seven UK hospitals. Surveys were sent to mentors and plastics trainees via the BAPRAS Specialist Interest Group and Plastic Surgeon Traine

Multidisciplinary Management of an Anterior Skull Base Teratoma in a Neonate.

Teratomas are embryonal neoplasms composed of 2 or 3 germ layers. We present the case of a female neonate who had a rare, mature, skull base teratoma that was managed surgically by fetal surgery, neurosurgery, otolaryngology, and plastic surgery. Imaging revealed an anterior skull base teratoma with extension into the nasopharynx, resulting in gross enlargement of the right nostril and sinonasal cavity. An ex utero intrapartum treatment procedure was performed to establish an airway before placental support was removed. Subsequently, neurosurgery and ENT performed endonasal resection of the te

[Treatment of abdominal lipocutaneous excess in massive weight loss patients].

In massive weight loss patients, abdominal lipocutaneous excess is a frequently encountered complication with major psychological, medical and functional consequences. A wide variety of corrective techniques exists, each with its advantages and side effects, and the choice of the technique must be made in conjunction with the patient, taking into account his unique anatomy, his wishes and his risk factors, in order to obtain the best aesthetic result while minimising the per- and post-operative complications. Liposuction, if prudently executed with the correct technique, permits a further harm

Four-step mandibular lengthening to correct a bird face deformity. A case report.

A technique for extreme lengthening of the mandible is presented. In one operation it combines several procedures already known, modifying some of them. Its advantage is that the movement is subdivided in 4 steps. Thereby the need for grafting is reduced or even absent and the amount of relapse is probably less important than if fewer but bigger movements were applied.

Robotic Harvest of a Rectus Abdominis Muscle Flap After Abdominoperineal Resection.

INTRODUCTION: As multidisciplinary treatment modalities for rectal cancer continue to evolve, neoadjuvant chemoradiation then surgical resection is a common approach. Robotic-assisted abdominoperineal resection is becoming more prevalent in part because of better visualization and instrument mobility within the pelvis. After abdominoperineal resection, postoperative perineal wound complications remain a significant risk. Pelvic reconstruction lowers this risk, and a pedicled rectus abdominis muscle flap is frequently used to achieve this. Traditional flap harvest requires laparotomy, resulting

New-Onset of Herpes Simplex Keratitis After Blepharoplasty, Case Series and Review of the Literature.

PURPOSE: To report 3 cases of new-onset herpes simplex keratitis (HSK) after uncomplicated extraocular plastic surgery and discuss potential risk factors. METHODS: This case series includes 3 patients who underwent uncomplicated blepharoplastic surgery. Within 2 weeks postoperatively, all patients reported ocular discomfort, and their ophthalmic examinations revealed corneal lesions suspicious of HSK. One case was confirmed as an active herpes infection, and the other 2 cases were clinically diagnosed with HSK. The patients were treated with oral acyclovir and followed up for up to 6 weeks.
