Treatment of cardiovascular manifestations of human scorpion envenoming: is serotherapy essential?

Death due to sting by the red scorpion is not uncommon in parts of India. Extensive clinical study has shown that fatality is due to cardiovascular involvement as a result of autonomic storm. Prazosin and nifedipine helped to counter the physiological actions of scorpion venom on the sympathetic nervous system. Scorpion antivenom remains unobtainable for human scorpionism in India.

[Pharmacological correction of stress reactions after hyperthermia].

It was demonstrated experimentally that aminazine, phentolamine, nuredal, sapilent, teperin, dopegyt improved significantly the health status of animals exposed to high temperature and in many cases prevented their death. This gives evidence that the sympathetic nervous system plays a leading role in heat stroke and that sympathotrophic drugs can be used for protective purpose during acute overheating.

Differential cardiovascular effects of pharmacological agents in chickens selected for high and low body weight.

This study was conducted to investigate whether there are differences in the autonomic nervous system function of chickens from lines select … ine was given following atropine. This pattern of response suggested that HWS birds had greater parasympathetic nervous system activity in order to maintain cardiovascular function. These results demonstrate that selection for HWS or LWS has resulted in greater parasympathetic and sympathetic nervous system tone in birds from the HWS and LWS birds, respectively, and suggest …

[Pharmacologic modification of ureteral activity].

The renal pelvis and the ureter represent a functional system with myogenic excitation generation and conduction. The activity of this system is modulated by the autonomic nervous system: alpha-adrenergic and cholinergic substances stimulate, beta-adrenergic drugs inhibit the pyeloureteral activity. Besides the sympathetic nervous system with adrenergic postganglionic excitation conduction and the parasympathetic nervous system with cholinergic transmission, non-adrenergic, non-cholinergic systems appear to exist. Vasoactive intestinal peptide (VIP), e.g., markably decreases the frequency and …

Anhidrosis after anterior retroperitoneal approach for L4-L5 artificial disc replacement.

Description of injury to the sympathetic nervous system after the anterior approach to the lumbosacral spine remains restricted to reports of retrograde ejaculation in males occurring in 2% to 20% of patients. The authors describe a 46-year-old female who presented with low back pain for several years due to degenerative disk disease at L4-L5 that had been treated with an L4-L5 artificial disc replacement though a left anterior retroperitoneal approach. She has had an excellent outcome with complete resolution of her back pain, but noticed the development of dryness of her left lower limb with…

Sympathetic and cardiovascular activity during cataplexy in narcolepsy.

Autonomic nervous system activity changes have been described during cataplexy as playing a role in triggering it. To confirm these previous findings, we investigated the time course of sympathetic and cardiovascular activities during cataplexy. We made for the first time microneurographic recordings of 10 cataplectic episodes in three patients with hypocretin-deficient narcolepsy. During microneurography, muscle sympathetic nerve activity (MSNA) was recorded simultaneously with heart rate (HR), respiratory movements, arterial finger blood pressure (BP), electroencephalography, electro-oculogr

[Relationship between cardiac 123I-metaiodobenzylguanidine scintigraphy and cardiovascular autonomic function test (standing test) in Parkinson's disease].

We investigated the correlation between results of 123I-metaiodobenzylguanidine (MIBG) myocardial scintigraphy and those of cardiovascular autonomic function tests in patients with Parkinson's disease (PD). 123I-MIBG myocardial scintigraphy and a 5-minute standing test were performed in 50 patients with PD and in 19 control subjects. The value of the basal plasma noradrenaline (NA) level was used as an index of basal sympathetic nerve activity, and %NA was used to assess the response of sympathetic nerve activity. In addition, the parameters of DeltaBP and DeltaHR were evaluated to assess the

Angiotensin-sympathetic system interactions in cardiovascular and metabolic disease.

BACKGROUND: Blood pressure, as well as blood volume homeostasis, depends to a large extent on humoral influences stemming from the renin-angiotensin axis and the sympathetic nervous system. Evidence has been provided that a large part of this homeostatic modulation is effected by the complex interactions between the two systems. OBJECTIVES: The present review will focus on three major issues. First it will examine the physiological and pathophysiological relevance of angiotensin-sympathetic crosstalk discussing possible sites, mechanisms and effects of the interaction. It will then address th…

Neural mechanisms of atrial fibrillation.

PURPOSE OF REVIEW: The autonomic nerve system is a potentially potent modulator of the initiation and perpetuation of atrial fibrillation. This review will briefly summarize the neural mechanisms of atrial fibrillation. RECENT FINDINGS: Complex interactions exist between the sympathetic and parasympathetic nervous system on the atrial electrophysiologic properties. Direct autonomic recordings in canine models demonstrated simultaneous sympathovagal discharges are the most common triggers of paroxysmal atrial tachycardia and paroxysmal atrial fibrillation. Also, intrinsic cardiac autonomic ner…

Autonomic regulation during sleep and wakefulness: a review with implications for defining the pathophysiology of neurological disorders.

Cardiovascular and respiratory parameters change during sleep and wakefulness. This observation underscores an important, albeit incompletely understood, role for the central nervous system in the differential regulation of autonomic functions. Understanding sleep/wake-dependent sympathetic modulations provides insights into diseases involving autonomic dysfunction. The purpose of this review was to define the central nervous system nuclei regulating sleep and cardiovascular function and to identify reciprocal networks that may underlie autonomic symptoms of disorders such as insomnia, sleep a

[Markedly decreased cardiac uptake with 123I-MIBG scintigraphy in a case of pure progressive autonomic failure].

A 70-year-old man with about 25 years' history of autonomic nervous system disturbances including orthostatic hypotension is presented. He had had no extrapyramidal and cerebellar signs, and was diagnosed as having pure progressive autonomic failure. His cardiac imaging with 123I labeled MIBG (metaiodobenzylguanidine) showed extremely diminished uptake, but the imaging with 201-thallium showed a normal cardiac uptake. Those results indicate non-cardiac disease-associated (neurogenic) postganglionic sympathetic fiber abnormality innervating the heart. Also, adrenomedullary imaging with 123I lab

Lack of temperature control in infants with abnormalities of central nervous system.

The temperature control of 11 infants with severe abnormalities of the central nervous system has been investigated. 4 were normothermic and 7 poikilothermic. Necropsy of the latter showed that either the hypothalamus was absent or disorganized, or the long tracts through the brainstem were unidentifiable, or in one case interrupted by a cervical myelocele. The lateral horn cells, sympathetic nervous system, and brown fat appeared normal. Where the normal reflex responses to warm and cool stimuli were absent, direct stimulation of the effector organs was attempted. Noradrenaline both stimulate…

[Vascular changes in severe carpal tunnel syndrome: a differential diagnosis of vasculitis].

Carpal tunnel syndrome may present with skin lesions. This may lead to a differential diagnosis with vasculitis. Sympathetic nervous system perivascular involvement and recurrent injuries secondary to sensory loss are probably part of the mechanism of injury. In this case, we also comment on the pathogenic role of persistent median artery associated with a bifid median nerve.

Detrusor inhibition in suprasacral spinal cord injuries: is it due to sympathetic overactivity?

The authors report five patients with detrusor inactivity in suprasacral spinal cord lesions with perineal spasticity. In all of these cases with cervical or high thoracic lesions treatment with an alpha-blocking drug (Phenoxybenzamine) allowed bladder activity to return. These results suggest that the sacral parasympathetic centre is under the inhibitory influence of the sympathetic nervous system.

Control of delayed-type hypersensitivity by ocular- induced CD8+ regulatory t cells.

The immunoregulatory pathway from the eye to the peri - pheral immune system is comprised of the iris, ciliary body, circulation, thymus and spleen, and is influenced by the sympathetic nervous system. At the splenic end of this pathway are antigen-specific CD8+ regulatory T cells (Tregs) that mediate directly the suppression of T cells that effect delayedtype hypersensitivity (DTH). Here we review investigations that demonstrate: (i) the injection of antigen into the anterior chamber (AC) attracts circulating monocytic cells to the iris/ciliary body that recirculate to the thymus and spleen. …

Cervical spinal cord lesions disrupt the rhythm in human melatonin excretion.

To determine whether spinal cord lesions disrupt the diurnal activity of the human pineal, urinary melatonin levels were measured over 24 hours (4 or 8-hourly intervals) in male patients with clinical evidence of cervical spinal cord transection. During the waking state, levels of melatonin in these subjects ranged from 3.2--13.5 ng/4 hours; during sleep and darkness, values ranged from 1.8--10.5 ng/4 hours. Levels of serum cortisol, aldosterone, and growth hormone showed rhythmic variations in these subjects. The absence of significant nocturnal melatonin increases distinguishes quadriplegic

The sympathetic neural hyperalgesia/edema syndrome, a common cause of female pelvic pain, manifesting as a pseudopheochromocytoma with marked clinical improvement with sympathomimetic amines.

PURPOSE: To show that a common but not well-known disorder of the sympathetic nervous system can present with symptoms suggesting a pheochromocytoma. MATERIALS AND METHODS: The standard treatment of this disorder (which is characterized by an abnormal water load test), i.e., sympathomimetic amine therapy, was given to a woman with paroxysmal tachycardia and hypertension. RESULTS: Over a period of six months, the treatment eradicated the paroxysmal symptoms to which all other therapies had failed. CONCLUSIONS: This condition recently named as sympathetic neural hyperalgesia edema syndrome ca…

A study of changes in the status of autonomic nervous system in primary open angle glaucoma cases.

Control of Intra Ocular Pressure (IOP) is influenced by both divisions (sympathetic and para-sympathetic) of Autonomic Nervous System (ANS). The present study was conducted to confirm existence of any association of ANS with primary open angle glaucoma (POAG), if present. Systemic autonomic function tests were assessed in fifty patients of POAG with fifty normal subjects matched for sex and age as control using several well established tests based on cardiovascular reflex responses to standardised stimuli viz. Resting heart rate (RHR), Corrected QT Interval (QTc), T-wave amplitude, Standing to

Cardiovascular autonomic insufficiency in a patient with metastatic malignancy.

A case of cardiovascular autonomic insufficiency is described in a cachectic 70-year-old man with widely metastatic small-cell tumor. The patient experienced disabling syncopal episodes in association with severe postural hypotension. Tests of cardiovascular autonomic insufficiency were abnormal. The introduction of fludrocortisone and the use of elastic stockings produced palliation of the symptomatology, but his condition deteriorated and he died. Autopsy demonstrated unexpectedly extensive tumor invasion of autonomic nervous tissue. Although cardiovascular autonomic insufficiency (CAI) has
