INTRODUCTION: Research on rotator cuff tears in Japan was initially characterized by studies clarifying prevalence, asymptomatic tears, and natural history. It subsequently developed toward evaluation of outcomes after arthroscopic repair, postoperative tendon integrity, and long-term results of conservative treatment.
METHODS: This article reviews clinical and basic research on rotator cuff tears that has been conducted in Japan.
RESULTS: Representative examples include arthroscopic superior capsule reconstruction (SCR), rotator cuff reconstruction combined with small-head hemiarthroplasty,

The surgical management of rotator cuff (RC) tears has progressed considerably in recent decades. Arthroscopic procedures now represent the … of truly irreparable RC tears, superior capsular reconstruction is a promising alternative to open tendon transfer and arthroplasty procedures. The purposes of this concise review are to: (1) explain our preference for a DR repair configuration in almost all circumstances; and (2) advocate superior capsular reconstruction as a viable treatment tactic for truly i …

Posterior tibial tendon insufficiency (PTTI) is nowadays considered to be the main cause of adult-acquired flatfoot deformity (AAFD). The purpose of this study is to report the outcomes of tendoscopic treatment of tibialis poste- rior tendon (TP) in eleven patients with stage 1 or 2 PTTI and failed prior conservative treatment. Tendoscopy was carried out as a solitary procedure in 8 patients, while in 3 patients additional procedures such as ,,mini-open" tubularization of TP or anterior ankle arthroscopy were necessary. In a single patient transfer of flexor digitorum longus tendon was perform

An irreparable cuff tear is defined as the inability to achieve direct repair of native tendon to the great tuberosity despite intra- and ex … nts if the muscle is still trophic with a fatty infiltration less than 3. It can be combined with a tendon transfer. In irreparable posterosuperior tears, latissimus dorsi or lower trapezius transfer has been reported to improve active elevation and external rotation. In anterosuperior cuff tears, pectoralis major or latissimus transfer has been used. If the lack …

PURPOSE OF THE STUDY: The authors report 24 cases of revision in recurrent club foot deformity. They assessed Cahuzac's procedure for treatment of the residual forefoot adduction. This procedure (opening of the first cuneo-metatarsal joint and proximal abduction osteotomy of the second, third, and fourth metatarsals) was generally associated with postero-medial, plantar release, and split tibialis anterior tendon transfer.
MATERIAL: Twenty four procedures for 20 children at mean age of five years have been performed. The revision chart comprised 12 clinical and radiological items as proposed …

BACKGROUND: Injury to the spinal accessory nerve (Acc) results in loss of motor function of the trapezius muscle and leads to severe shoulder problems. A vast number of surgical strategies have been proposed to reinnervate (suture, grafting, and nerve transfers) or compensate (tendon and muscle transfers) the lost function of the trapezius muscle.
OBSERVATIONS: The authors report a successful case of Acc reconstruction 5 months after the injury with the anterior C3 levator scapulae motor nerve branch transfer in omotrapezoid triangle of the neck.
LESSONS: The advantages of the proposed techn

There are several treatment options for recurrent shoulder instability. The Latarjet addresses bone loss via coracoid transfer, and creates a "sling effect" on the humeral head with the conjoint tendon. It does however carry a high potential risk of complications including graft resorption, hardware failure, and neurologic injury. It is hypothesized that the long head of the biceps can function similarly, without the donor site morbidity of a coracoid transfer. We present a case of recurrent instability and musculocutaneous nerve palsy following primary arthroscopic stabilization three years p

CASE: A 39-year-old man with a chronic Achilles rupture status post (1) failed primary repair and (2) secondary xenograft repair with graft rejection, resulting in a 12-cm Achilles tendon defect, which was reconstructed utilizing an Achilles bone block allograft and flexor hallucis longus (FHL) tendon transfer. At 15-year follow-up, the patient reported good functionality and satisfaction with the repair, with positive patient-reported outcome measures. Physical examination revealed excellent strength and range of motion. Magnetic resonance imaging confirmed the integrity and incorporation of …

Shoulder dynamic anterior stabilization (DAS) is an efficient and well-established glenohumeral stabilization technique, offering an arthroscopic alternative to Latarjet and glenoid reconstruction with distal tibial allograft or the iliac crest autograft. DAS is essentially an augmented Bankart procedure and can be performed using transfer of either the long head of the biceps tendon or the conjoined tendon. Both result in similar and acceptable rates of recurrences, complications, return to sport, and subjective shoulder function. However, the effectiveness of Bankart repair on shoulder stabi

RECOMMENDATION: The initial treatment of an infected Achilles tendon reconstruction should include thorough debridement of all infected tissues with the removal of retained sutures or foreign material. Cultures should be taken at the time of debridement, and antibiotic administration should be dictated by the result of culture and continued until inflammatory markers and clinical symptoms normalize. If significant soft tissue defect in the overlying area remains, the choice of tendon reconstruction and/or transfer with soft tissue coverage should be left up to the discretion of the treating su

UNLABELLED: Lesser digital deformities may present a surgical challenge to even the most skilled foot and ankle surgeon. Multiplanar toe deformities, including the crossover toe, are especially difficult to correct with reproducible results. Undercorrection, pain, stiffness, and recurrent deformity are well reported throughout foot and ankle literature. The goal of this article is to describe a method of correcting digital deformity utilizing the extensor digitorum brevis tendon transfer and a biotenodesis screw. The controlled tension established with the extensor tendon provides the necessar…

Spastic forearm, wrist, and hand deformities in isolation or collectively can markedly hinder function and/or provide challenges with hygiene. When muscle contracture is the main contributor to the spastic deformity, muscle-based and tendon-based procedures can effectively correct the abnormal posture. Tendons from expendable muscles can be transferred to the weak or paralyzed muscle to improve the static position of the joint or to restore active function. When employed for the correct underlying pathology, muscle-based procedures can effectively correct the spastic deformity, which can lead

Chronic lower extremity (LE) wounds, especially in diabetic patients with peripheral neuropathy, are a leading cause of nontraumatic amputat … y plantarflexion contracture. Varus or valgus deformities of the lateral or medial foot may require tendon transfers or joint fusion, and Charcot neuroarthropathy often necessitates osteotomy and/or arthrodesis. Hallux and lesser toe wounds, commonly due to digital contractures, are treated with tendon transfers, arthroplasty, or fusion. In some cases, transmetat …

BACKGROUND: To evaluate the operative method and clinical outcome for the treatment of posttraumatic equinus deformity and concomitant soft tissue defect of the heel.
METHODS: Between June 2006 and May 2010, seven cases of posttraumatic equinus deformity and concomitant unstable scar or ulcer of the heels were treated by using a hinged Ilizarov apparatus and reversed sural fasciocutaneous island flap transfer. Achilles tendon lengthening was also done in all patients. The average duration of follow-up was 21 months. The sizes of sural flaps were from 7 cm × 6 cm to 10 cm × 9 cm. Two weeks aft

Muscle-based procedures have been the mainstay of surgical treatment of spastic upper limb deformities, in particular when muscle contractures are a main contributor to the deformity. The type of muscle-based procedures that are utilized to correct a spastic deformity is dependent upon the presence of volitional motor activation and the magnitude of muscle contracture. These procedures can markedly improve patient's quality of life and enhance upper limb function. However, comprehensive multidisciplinary care and establishment of shared patient-physician or caretaker-physician decision-making

Management of the upper limb in children with cerebral palsy is complex and requires a multidisciplinary approach. Non-surgical interventions such as physiotherapy, occupational therapy, orthoses and botulinum toxin are first-line treatments, but surgery remains the only option to durably correct contractures, rebalance forces, and improve posture, function, hygiene or aesthetics. Surgical procedures must be tailored to the anatomical segment involved, the degree of spasticity and contracture, and the functional goals defined with the patient and the family. The surgical techniques and indicat
