Preclinical alveolar ridge preservation using small-sized particles of bone replacement graft in combination with a gelatin cryogel scaffold.

BACKGROUND: Particulate anorganic bovine bone matrix (ABBM) is extensively used for alveolar ridge preservation (ARP). This study evaluates the potential of ABBM/gelatin cryogel scaffold of regular (250 to 1,000 μm) and small (50 to 100 μm) particles for ARP. METHODS: ABBM was either condensed in a gypsum ring to simulate filling of ABBM in a defect or incorporated into a gelatin cryogel scaffold to disperse ABBM. Condensed regular-sized or small-sized ABBM (rABBM or sABBM) (± bone morphogenetic protein [BMP]-2) were subcutaneously implanted in rats to evaluate biocompatibility and osteogenic

Clinical relevance of dimensional bone and soft tissue alterations post-extraction in esthetic sites.

The key to achieving pleasing esthetics in implant dentistry is a thorough understanding of the biological processes driving dimensional bone and soft tissue alterations post-extraction. The aim of the present report is first to characterize the extent of bone and soft tissue changes post-extraction and second to identify potential factors influencing tissue preservation in order to facilitate successful treatment outcomes. The facial bone wall thickness has been identified as the most critical factor influencing bone resorption and can be used as a prognostic tool in order to identify sites a

Part II. Minimizing alveolar bone loss during and after extractions. Protocol and techniques for alveolar bone preservation.

Alveolar ridge resorption accelerates following extraction of teeth and the residual defect varies from socket to socket. This article proposes a new treatment oriented classification of extraction defects. It also reviews several graft materials and membranes that aid in the decision for selecting an appropriate socket preservation technique. The algorithm developed by the authors helps design a potentially successful treatment plan based on the classification of extraction defects, with choices ranging from no treatment to complex grafting procedures (i.e. allogenic block grafts). In additio

Alveolar bone regeneration in post-extraction socket: a review of materials to postpone dental implant.

Tooth extraction usually involves alveolar bone loss and reduction in height and width of the remaining alveolar socket, owing to the physiological bone resorption. This occurrence may perform an inadequate bone profile, that make difficult orthodontic applications, compromising the functional and aesthetic restoration of dental implants. The present review will provide an update on the biological and clinical profile of materials currently in use and those under investigation, in the recovering of bone margins of edentulous sockets.

Intra- and inter-rater agreement of a new classification system of gingival recession defects.

PURPOSE: Several classification systems have been proposed in the literature, but none of them has been validated by means of an appropriate statistical analysis and, therefore, it is unknown whether these systems work in the same manner among different clinicians. The aim of the present study is to investigate the intra- and inter-rater agreement of a new system to classify gingival recession defects and to evaluate its agreement among different clinicians. MATERIALS AND METHODS: A new classification system was proposed and tested considering the following three factors: the amount of kerati

Buccal plate augmentation: a new alternative to socket preservation.

Alveolar ridge resorption occurs after tooth extraction and has the potential to either complicate implant placement or impair the final esthetic result. Techniques to preserve natural bone and soft tissue contours are of great interest to clinicians and patients because even subtle postextraction buccal plate resorption may have significant clinical effects, particularly in the esthetic zone. Buccal plate augmentation (BPA) is a novel approach for ridge preservation aiming to avoid recession of the facial wall of the socket without interfering with the natural healing mechanism of the extract

The healing socket and socket regeneration.

Tooth extraction is a common procedure in dentistry. The normal healing response to the procedure results in a significant loss of bone and collapse of the surrounding gingiva. In addition to normal healing, a substantial percentage of extraction sites suffer postoperative complications. This article presents histology that supports the concept that the first response to extraction is bone death and resorption of the socket wall. The stages of extraction socket healing also will be discussed. Additionally, the article will present a regenerative method that skips the resorptive phase, the clot

Hypophosphatasia affecting the permanent dentition.

Reports on dental abnormalities in connection with hypophosphatasia almost exclusively describe changes in primary teeth. A 23-year-old man with hypophosphatasia, first diagnosed at the age of 8 months, is described; histologically and radiographically verified signs of the condition were present in the permanent dentition. The findings included a reduced level of the marginal alveolar bone supporting the upper central incisors, which had to be extracted. The molars displayed large coronal pulp chambers. Histologically, the upper incisors demonstrated abnormal root cementum, with areas of dent

Dental pathology in Pongo satyrus borneensis.

The Selenka orangutan collection obtained from 1892 to 1896 from wild-shot specimens in West Borneo, provides an excellent opportunity to examine dental pathology in free-ranging primates. Two hundred and twenty-three skulls from this collection were analyzed for dental conditions, including carious lesions, local infections (infra-alveolar and periapical osseous defects), horizontal bone loss, and premortem tooth loss. Specimens were sexed, divided into three broad age groups, and compared to ascertain sex-related or age-related differences in occurrence of dental pathology. None of the subad…

Decoronation of an ankylosed tooth for preservation of alveolar bone prior to implant placement.

A 12-year-old patient sustained avulsions of both permanent maxillary central incisors. Subsequently, both teeth developed replacement resorption. The left incisor was extracted alio loco. The right incisor was treated by decoronation (removal of crown and pulp, but preservation of the root substance). Comparison of both sites demonstrated complete preservation of the height and width of the alveolar bone at the decoronation site, whereas the tooth extraction site showed considerable bone loss. In addition, some vertical bone apposition was found on top of the decoronated root. Decoronation is

Evaluation of mandibular anterior alveolus in different skeletal patterns.

BACKGROUND: The boundaries for orthodontic tooth movement are set by the bony support of the dentition. This study compares the mandibular anterior alveolar housing in individuals with low, average, and high mandibular plane angles before orthodontic treatment and measures alveolar bone loss and root resorption after orthodontic treatment. METHODS: Pretreatment cone-beam computed tomography (CBCT) images of 75 non-growing individuals, 25 in three groups: low-angle (sella-nasion to mandibular plane ≤28°), average-angle (30°-37°), and high-angle (≥39°), were analyzed. Esthetic restoration with orthodontic traction and single-tooth implant: case report.

This case report describes the use of orthodontic traction to recover soft tissue lost around a maxillary right central incisor with major external root resorption associated with severe gingival recession. Traction of the residual root for 1 month produced a gingival appearance in harmony with the adjacent teeth. After the placement of an implant, a correct emergence profile was obtained, giving an optimal esthetic outcome. After a 3-year follow-up, complete regeneration of soft tissue persisted around the implant-supported crown.

Porphyromonas gingivalis, a bridge between oral health and immune evasion in gastric cancer.

Porphyromonas gingivalis (P. gingivalis) is a gram-negative oral pathogen associated with chronic periodontitis. Previous studies have linked poor oral health and periodontitis with oral cancer. Severe cases of periodontal disease can result in advanced periodontitis, leading to tissue degradation, tooth loss, and may also correlate with higher gastric cancer (GC) risk. In fact, tooth loss is associated with an elevated risk of cancer. However, the clinical evidence for this association remains inconclusive. Periodontitis is also characterized by chronic inflammation and upregulation of member…

Effects of occlusal disharmony on susceptibility to atrial fibrillation in mice.

Tooth loss or incorrect positioning causes occlusal disharmony. Furthermore, tooth loss and atrial fibrillation (AF) are both risk factors for ischemic stroke and coronary heart disease. Therefore, we hypothesized that occlusal disharmony-induced stress increases susceptibility to AF, and we designed the present study to test this idea in mice. Bite-opening (BO) was done by cementing a suitable appliance onto the mandibular incisor to cause occlusal disharmony by increasing the vertical height of occlusion by 0.7 mm for a period of 2 weeks. AF susceptibility, evaluated in terms of the duration…

Intrusion of overerupted upper first molar using two orthodontic miniscrews. A case report.

Loss of the mandibular first molar often leads to the overeruption of the opposing maxillary first molar, resulting in occlusal interference, loss of periodontal bony support, and inadequate room to restore the mandibular edentulous space. Without orthodontic molar intrusion or segmental surgical impaction, restoring the posterior occlusion often entails the need for significant reduction of maxillary molar crown height, with the potential need for costly iatrogenic root canal therapy and restoration. The literature has cited successful maxillary molar intrusion with minor prosthodontic reduct
